The trapdoor to the maze is always in a random Black Powder Mine room. The room with the trapdoor has small rat critters which won't attack you. Then, look on the floor for the trapdoor (it's very well hidden). When you find a trapdoor outline, "dance" on it for a while (walk on it over and over again). Then, use a regular key to open the hatch. At the end of the underground railroad, use a blank. Dodge roll into the corridor, walk to the end of it, and use another blank. Use the Gnawed Key to open the large gate. Fall down the huge rectangular pit in the new floor. Then, follow the directions on the Infuriating Notes (the cheeses are arrows). If you forgot them you can always look in your Ammonomicon. They are randomized for each player so don't look them up. You know you're doing it correctly if you don't encounter any empty rooms. (EDIT: Very rarely, you can encounter empty rooms but still be on the right way. Simply continue to follow the cheese arrows in this case). If you fail to find your way in the maze an elevator to the Hollow will drop down, and you'll have done all that for nothing. Once you have gotten past all of the extremely dangerous rooms with probably half a heart remaining, gather your pickups and head for the boss room where you'll probably die instantly. I wish you good luck.

Oh, and in metal gear solid, you could break its weapons to make it less crazy. Don’t forget to shoot ratty in the face when the cockpit bugs out and randomly opens! Otacon put in that feature to prevent the pilot from causing NUCLEAR ANNIHILATION!!!
Yep. The cannon fires before the crosshairs attack, the drum clip thing shakes before the Missile barrage, and both of them flash when the bot takes damage.

I only died to it once. And I think the punchout took me 6 tries or so.

It's mostly just practice and learning bullet patterns. Good reaction times, making split second decisions and such is also important though.
