Paolo Carta it varies by law. most states in USA, volunteer firefighter and paramedics are allowed to with permits from the state. In Canada, as far as I know, only one province allows lights and sirens on private cars.

Perché purtroppo in Italia si ragiona con troppa burrocrazia, e soprattutto loro usano un sistema operativo fantastico. Non fanno servizio in sede come noi, ma abitano vicino o comunque raggiungono la sede solo quando c'è una chiamata ecco perché hanno le sirene sui mezzi privati. Arrivano con il mezzo privato e si fondano sull'ambulanza, preparata da 1/2 volontari fissi. Tra l'altro hanno i tempi di risposta più brevi al mondo

Sir, it doesn't make a difference what type of car they are driving. They respond to medical emergencies with their personal vehicles and therefore it's totally legal according to NYS law for them to outfit their cars.
As well as their own vehicles are registered as ambulances
