    I have to admit when I had the perianal abscess it was hard to watch your videos because you look so happy and pain free. Now that I have had the drainage and feel relieved I enjoy your videos now.

    Thank you so much for this - it's really helpful. I had a perirectal abscess which was treated by experts in North Carolina. It was big by the time surgery was needed (4 cm x 3 cm), but I came back home and it healed, so I was lucky. I did need a drain, though, and surgeons used a Penrose drain, which worked really well. I was very grateful for their care, and I have subscribed to your channel, because what you are telling people is practical, sensible and reassuring.

      Mine burst I my sleep the night before surgery was scheduled after that nights ER visit..
      They wanted to drain all even after it burst..I felt was healing on its own...
      Just the grape shell of skin was left hanging dry of pain and fluid

    I had this kind of abscess in 1990. I went to the er 3 times and was told it was hemorrhoids. The infection got so bad I almost died. I finally went to a doctor I knew. Got to the hospital had to go straight to surgery. I'm still having problems to this day.

    GOD BLESS you for this information. I'm 42yrs old & never had experience this. Just got an abscess in July. The hospital in High Point NC told me to let it drain then come back after it pops. So I did in August the doctor only put his finger up my butt then told me to go to my primary care doctor after I told him I didn't have one. Now it's Nov took a pic cause I noticed pus on my underwear. I have a hole. Want lie now I'm scared. You helped me to ease my mind & now I will have to go back to the hospital. Just moved to New Ellenton SC still no health insurance but want give up. GOD BLESS YOU ALL & YOUR HEALING

    4 doctors misdiagnosed, they think I have bowel cancer, haemorrhoid etc finally I found very smart Chinese colorectal doctor he said immediately I have high complex fistula, I have 7 surgeries, 2 seton, 2 clips take me 3,5 years to heal

      This happened to me recently and had been ongoing for three months before I went to the ER. The GI doctor initially treated it as an anal fissure and hemorrhoids, prescribing nitroglycerin 4% and some suppositories. However, I ended up in the emergency room last weekend. The ER doctor ordered a CT scan, which revealed a perirectal abscess, and I had surgery the same day. It’s been six days now, and I feel much better. I’ll see the surgeon again in two weeks and may need to consult a colorectal surgeon (CRS) if a fistula is suspected.

    My story, because I'm beginning to suspect I have a fistula.
    Late June, about 6 weeks ago, I thought I had a hemorrhoid. This progressed over the course of a week or so. After treating with Preparation H and no relief, I decided to seek medical attention.
    July 3rd - Went to urgent care. Diagnosed with abscess. Started on antibiotic. Instructed to follow up with GI doc.
    July 5th - Followed up with GI doc. He said "it's huge", informed me I had a surgical condition and referred me to surgeon. Earliest would have been over a week. He instructed me to go to the ER to start an IV antibiotic if it got worse.
    July 6th - Spent majority of day in ER. Received CT scan to judge depth. Residents performed surgical procedure in ER room, cutting into and draining abscess. It was SUPER painful. They packed wound, and instructed me not to remove until 24 hours, after which time to re-insert packing and continue to do daily for a week.
    July 7th - My sister (a CMA) assisted with packing change. Super painful, but getting a bit better.
    July 9th - Packing wouldn't go in, but swelling had gone from between baseball/softball size to fifty cent piece. Doctor said shouldn't be of concern, don't worry about packing.
    July 12th - Had first follow-up. Pain down, swelling down, seemed to be trending in right direction.
    July 12th - 24th - Seemed to be healing, began doing light activity. Walking on slight incline, and very light weightlifting. Still getting overnight drainage.
    July 26th - Felt like I was almost out of the woods, and did almost half my pre-procedure workout. Felt a little bit of pain, but wasn't concerned.
    July 27th - Woke up to blood and significant drainage. Dialed back activity to VERY light walking.
    July 30th - Aside from overnight drainage, seemed the setback was fine. Still limiting physical activity.
    Aug 1st - Pain feels like it did a month ago. No additional drainage this morning, but most pain I've had since before the procedure.
    I've been keeping the wound covered with either bandaids or gauze and tape, and changing twice daily on average.
    I have a followup with GI doc Aug 3rd, and with the residents that performed procedure Aug 9th.
    For context on pre-procedure workout. I'm very active, with 6-7 days of 30-60 minute moderate to intense cardio, and 5 days of weightlifting on average per week. Obviously I haven't been doing that over the past month or so. I'm very nervous that between the continued drainage, especially overnight, more than 3 weeks out, and how hard and painful the site feels today that either they didn't fully drain the abscess or I have a fistula. Would love to hear your thoughts and opinions, as well as any advice on post-procedure activity and healing?

      I have a lump inside hole but it is painless I dont feel pain at all but i will surely go to doctor i dont know how that lump formed on its own previous month i had intense itching and i was stupid i used to scratch with big and dirty nails it totally damaged the skin. I need to worry now😅

      @@Qamarr_6092 Further update. Lots of blood, swelling, and drainage 8/1 and 8/2. Saw GI doc 8/3, and day before was able go schedule appointment with surgeon/specialist for this coming Tuesday, 8/8. GI doc, per thelr recommendation started me on another round of antibiotics. 10 days of clindamyacin. I'm crossing my fingers hoping maybe the residents, not being specialist, didnt get everything out, and I need another incision and drainage, and that combined with the antibiotics will take care of this. Today was day 4 of antibiotics, and pain and swelling is down, but still getting significant drainage, throughout day, but mostly at night. Been taking it pretty easy, aside from a couple walks that seem to increase drainage. Idk if that's good or not, but I'm getting sick of taking it easy, and even sicker of my ass leaking.

      Further update. Saw surgeon/specialist yesterday. He believes I have a fistula, but can't quite confirm it until MRI, which will likely be in next week or two. After which, apparently they wait until 8 weeks after initial incision and drainage of abscess before operating, because if the fistula isn't mature, there is higher risk of complications, including fecal incontinence. In a slight bit of good news, because I'm taking any I can get at this point, he said there are no restrictions on exercise, weight lifting, etc. as they wouldn't affect it one way or the other. I explained that the one day I did do some light weightlifting, he said that's simply because there was stuff in there that wanted to come out, and wasn't a cause for concern. Slightly terrified at what happens next, but at least I have some clarity and answers now.

      @mike692975 I'm not a doctor, but I wouldn't think spicy food makes the fistula return, but rather it's still there, perhaps never fully healed and you can feel it when you eat the spicy food.

      @@sagartripti yes I was only having perianal abcess which was not turning into fistula. The initial symptoms were feeling pain and discomfort near anal and you can feel it getting worse soon, just few days. Then you can feel a lump start growing. It’s not easy to just ignore it, then it’s time to go to hospital.

      @@sagartripti but if you don’t deal with it. It has highly chance to be fistula. So don’t hesitate, just go to hospital. Of course surgery is the only option unfortunately. I am not gonna lie it’s painful for first few days, but you can feel way much better after that. Hope you are feeling better soon

The challenge is I had been getting this nasty yellowish discharge every 2 to 3 days since then. I sometimes change my underwear 3 to 4 times a day sometimes and sometimes I could go 3 to 4 day without any discharge and then I feel this tightness and go through the same cycle.

    There are 2 non-invasive treatments for a Perianal Abscess that are not mentioned in this video and that are important to know if you want to avoid the problems with the old technique of Surgical incision and drainage such as a large open wound that often does not completely heal, narrowed anus, incontinence, plus a higher chance of a recurrence of the abscess and a fistula ! These are: 1) Aspiration (sucking out the pus with a needle) with Antibiotics 2) High energy laser treatment (a small fibre-optic probe will destroy the inside of the infectec cavity) The disadvantages old technique was freaking me out, so i am trying to help someone who is in the same situation.

    When you said they use the drainage tube right off the rip that made my heart sink. I had my first one 2 years ago, dude lanced it and put a drainage tube in and I’ve been having troubles since. Recently got with another dr and they got me on antibiotics and said to go back if an abscess returns, which it has. Hate to know my only option to be rid of these is surgery, but if that’s what it takes 🙃

      Be careful as you may end up with sepsis especially as you have a fever. That is a warning sign.

    Hello Dr. Chung (@yourfriendlyproctologist) - You are doing amazing work here! Your videos are so informative, provides great guidance on what are the right/not-so-right paths to follow and make complex subjects simple.
    How common is it for the pus in the perianal abscess to retrun only3-4 days after draining/surgery? I underwent the surgery in the ER 5 days back after experiencing lot of pain in the back and after a few days now I notice that the area near the operated spot is starting to swell again.
    Is there any laparoscopy option here?

I was able to navigate out of this entire bottom mess with the help of the information u provided within a month . I think am now completely healed though am still careful of what i eat and following best practices to keep the fissures away for atleast another 6 months. So thank u once again

      I drink lots of water and avoid spicy and sour food . Moderate amount of fibre all in all i say a balanced diet

    I had a horseshoe abscess in June of 2021, and sadly, I didn't know what was going on. (Because initial symptoms were a fever and chills, my first instinct was to rule out covid). I thought then that I had a fissure and some bug and that the two were unrelated. I waited a week, not even being sure that they'd take me seriously in the hospital. What a mistake this was (on my part). But because I have a stubborn streak, I was reluctant to check myself into a hospital. When the symptoms didn't improve, and I had a 103-degree fever, I finally decided to drive myself to the E.R. Thankfully, they DID take me seriously, and they ordered blood work and a CAT scan. It turned out not to be covid at all. I had the early stages of sepsis! I was placed on IV antibiotics, got shots of morphine, Tylenol, and Toradol, had the abscess lanced, and went home after a 2 night stay in the hospital. Sadly, I was in the unfortunate group. Because the drainage didn't stop after 4 months, the proctologist ordered an MRI. The abscess had indeed turned into a high transsphincteric fistula. He then decided to perform a two stage seton fistulotomy. However, he stated that a lay open fistulotomy was also a possibility. He simply wouldn't know which technique would be most suitable until after I was examined under anesthesia. The seton was left in place for 7 weeks, and then he pulled it through. A year elapsed, and sadly, my trusted surgeon passed away. But some drainage persisted! I found a new specialist an hour away (a female proctologist) who ordered a second MRI. It was revealed that a blind ending intersphincteric fistula remained, which she treated with a standard, lay open fistulotomy. I'm devoutly hoping that this is finally the end of it all. But I'm not sure. My follow-up is on December 30th, 2022, and I'm nervous. (Today is the 21st). Having pelvic floor disorder is what caused this whole mess. I also know a little more about these conditions than I want to. 😥

    My abscess ended up popping on it’s own. I was barely able to walk during the time I had it and even just standing up was immensely painful. My doctor put me on antibiotics and it popped within 2 days. I was at my friends and all of a sudden something felt different. I put my hand down there and pulled my hand up with just straight sticky puss blood. It looked like slime. It was disgusting. Ran home and got the rest of it out and had a little pea sized cavity. It was insane. 6 months later I’m chilling tho 👍

      @@Elisa-tu1ry They fine if you have boils use turmeric and maybe Castor oil bit on the area everyday it would be normally gone in a week or two gradually decreased or use oinments prescribed by doctor for the boils.

      @@adriadri3624 I’m fine now. It healed within 2-3 days. I was able to go on though without pain basically after i got all of the infected cyst/puss out. Sorry for the absolutely gross description but it was really gnarly. It was fascinating at the same time though to see how the body reacts to something like that

    Hi Dr Chung, I'm so grateful for your channel. I've recently been diagnosed with an anal fissure and anemia that I think has been caused by bleeding from the fissure. I just tried my first iron supplement and it gave me terrible cramps and diarrhea. I understand longer term it is likely to cause constipation. This will of course not be helpful for healing of my fissure, and may easily cause more bleeding, making the anemia worse. I imagine I'm not alone, perhaps you could do a video on anemia complications with hemorrhoids and fissures and how you might manage it. All the best.

      Anemia due to hemorrhoids is rare. I would suggest seeing your medical doctor to evaluate sources of anemia which are many.

      @@yourfriendlyproctologist Thanks for the advice, I will do. Perhaps it's diet. I was bleeding a lot last year, but you're right, I'm jumping to conclusions. On the examination when he diagnosed a fissure he inspected me with the proctoscope and it was perhaps the most painful thing I've ever experienced. On another channel they said if you can see a fissure from a visual inspection you shouldn't use a scope as it will cause so much pain and potentially make the fissure worse. The pain was so bad I can't help but agree with them, so I'm not sure whether I trust the surgeon any more.

    THINK!! Before you listen to doctors. If you are healthy, maybe you should try hot compress or wait 10 days before you have a doctor lance that perianal abscess.
    30% of perineal abscesses are MRSA! You heard that right, that means there is NO chance, none, of further complication fistula. You literally just had an operation on you when you could have tried hot compress or waited 10 days to see if it would make its way to the surface of the skin and discharge.
    Wait! Before you have one of these idiot doctors permanently ruin your body forever. This is whack!!
    I am So sick of these textbook doctors that lack common health sense. Time to get rid of these bad doctors!!

    I have pilonidul sinus. I had it for years with no issue. Because of it being there two doctors misdiagnosed. Finally the third doctor told me my discomfort is from an anal fistula. It opened on its own the day before appointment. He recommended an mri and antibiotics and a conversation before any surgery. But he said we will see what the mri and antibiotics do before doing anything. I’m very glad I watched this. Idk why, but for some reason this whole thing got me very worried. Also very happy for this third doctor.

    Hi Dr. How do you feel about packing or not packing the wound? I have been doing some reading and it says there is no real difference in recovery except for the pain factor in packing. I would like to know your thoughts on this or if anyone has experience in this.

    I just had it from the past 4 days and got the surgery today. The worst part was it happened on my last few days of the vacation trip. During the last 3 days was the worst unimaginable pain which feels like so concentrated at the anal region but whole body can feel it. The worst part it; you cannot go to poo , you can’t walk, sit or lie down properly. Only after the surgery I could feel myself alive. I was in Europe and the surgery was done in Sweden.

    Both my dad and brother had pilonidal sinuses. They had to go into hospital to have them drained and treated. I had to treat a lot of pilots for this problem, in the military.

    by watching ur video i feel like there is a hope .. i live in dallas texas and had an perianal abscess back in june .. went to see a doctor and she made a lil cut and let the pus out and ask me to follow up in 2 weeks .. after 2 weeks which was on august 5th i went and she said its still needs to be open and she insert something and open it again and that time i was traveling back home and she said i m good fr traveling.... i traveled to my country by the time blood and puss was still coming out from that area i went to see surgeon over there and he said i needed a surgery so i went through the surgery under GA for my abscess...and it was on august 21st ..
    i came back to 🇺🇸 on sep 12 .... and had some blood spots coming out from that surgery.. went to ER and they said wound looks good and closed still they did CT scan which came out clear .... ( my infection was 4.5 cm deep ) on the surgery time )
    again i had blood coming out so i went to see the same doc again and she said it looks like delay healing so she told me to follow up in next 6 weeks and she mentioned that there is high chances of fistula :(( i never had fever in this whole process...
    my only question is that blood coming from wound is it normal fir this long time of the period:(
    i really want to visit ur office even though its far coz every time i go to this doc she only talks about fistulas:((
    just worried about my bleeding plz reply me and suggest whatever is best for me
    or what i can do about my bleeding

    I have had an issue for about the last Month and can’t get it figured out. It started with a pain below my tailbone but the tailbone itself wasn’t really hurting. It hurt under the tailbone and between the rectum. It also went up my butt crack. I can feel like what is a firm sack in the rectum area and it feels like a squishy tube going down my crack. I went to my GP but she said she couldn’t feel anything. She could see a little scab area at the top of my crack and slight redness on my cheeks but nothing major. She sent me for a CT Scan with contrast. Nothing showed up on the Scan. No abscess or liquid. Bladder was unremarkable. If I push on the area where I feel like there is a sack it makes a slight gurgling sound and my rectum feels like it is swelling up. Sorry for the length of this but I’m trying to add everything I can think of. Please doctor if you can help me with your thoughts or suggestions I would be eternally grateful. I cannot sit up long and have to keep a shifting from side to side for the short time I’m up. And it hurts my butt crack area to lay on my back. I have to sleep on my side. It burns where that scab spot is on my upper crack. I am at a loss. All I want to do is cry it’s so painful. 😢 Thank you!

      He did say unfortunately antibiotics alone won't get rid of it.
      I was able to get rid of an abcess on my arm with antibiotics and warm compresses. But this may be more complicated.

      I was treated with antibiotics only (for abscess, not fistula!) but I think it doesn’t always work.

    I have a perianal abcess, and it bursted open about a week ago. In the hospitol the doctor said that i don't need surgery. However it is still leaking pus and fluid. Should i insist on a medical procedure or wait until it heals on it's own ?

    Man doc I wish I had seen you from the start. I was so traumatized from the wrong diagnosis to many hurtful and harmful procedures I stopped telling doctors about it. Now I have tracking, inflammation, and chronic continuous abscesses. I'm scared I'll never have it fixed. I've given up on it.

    Do you do CT scans after surgery to see if the abcess has disappeared? My surgeon has ordered a CT scan two weeks after surgery. I have no more pain.

    I just went through a surgery for this a few weeks ago. It felt like it was about the size of a fist. I showed it to the doctor and he told me that I was going in for surgery that day. I was put under general anesthesia and spent a couple of days in the hospital with constant IV fluids and antibiotics. I guess he packed it with 140 cm of packing at the end of surgery. I've been going through daily packing changes now for the least 3 weeks and it's almost completely healed up. Even with all of the morphine they injected me with, those first couple of packing changes were some of the most painful experiences of my life. Something tells me that morphine doesn't work on me.

    I recently went to a doctor in Japan. He said it's a small abscess but said it's nothing to worry about and just have me some weak pain killers (which didn't work), some antibiotics and some stomach settling medicine. The issue has just continued and I can't sit down and/or sleep. My head and eyes are pounding due to the lack of sleep.

    I’ve had an abscess surgery in August and now 4 month later the pain came back and I do feel something a bit. I don’t know if it’s abscess or fistula . And if I’ll need surgery again 😭
    For the 1st time I had a tube inside drainage for 2 weeks and after twice weeks it healed and doctor took it out

    Hi doc. Currently going through this. I have an anal abscess and it ended up popping on it's own. I went to the doctor and told me to just let it drain on its own but it stopped draining and i feel a hard lump still. No pain just a hard lump and some discomfort. What do you suggest?

      I had the same issue. It popped on it's own and had a lump on the side not far from where the old one was. Then 6 weeks later it filled up with puss

    Thank you for this video!❤ I had my anal absec cut open 3 times, cause it keept closing before all of the pus was out. This time it feels like it is empty, at least the skin around it feels soft again. But today the doctor found a fistula in an ultrasound exam. He wants so treat in with suton. Do you think I should wait and see if the abses is gone this time? Can I live ok with an untreated fistula?

    What would you suggest for someone that doesn’t have health insurance? I’ve had this for almost 2 years it’s painful & uncomfortable, but I don’t know a cost efficient way to have it removed. The last time I went to the dr before my insurance ended, she told me it would go away & to take sitz baths. A year later it’s still there & bigger. I got it from underwear rubbing against my butt crack & it caused a boil. It’s also very embarrassing. Please help

    Hi Dr. Chung! I have many questions on this topic!
    1. Can an abscess drain out the rectum?
    2. If an abscess does drain out the rectum is that automatically a fistula?
    3. If an abscess is draining inside the rectum, would a surgery drain it from the same hole (inside) or do they try to drain it externally?
    4. If a patient is prone to sepsis (I've had it 5 times) and other infections (like MRSA), should they be on precautionary antibiotics?
    5. If you do recommend precautionary antibiotics to your patients, at what stages would that be i.e.: prior to draining? Post draining? For how long?
    6. What happens if someone has had an abscess for a long time, like around a whole year....because the doctors didn't notice it? What if they've even had it 2 years? Can one sort of hang around that long without intervention?
    7. If a patient is diabetic, tell us about how an abscess plays into that. What comes first: the diabetes or the abscess (my blood sugar has been rising and rising over the last 2 years no matter what I do and I understand nutrition extremely well)?
    8. Can you go over what might differentiate symptoms of anal cancer vs. abscess? For instance would both cause extreme fatigue, puss, etc?
    9. Should an EUA be done to biopsy for cancer, look for abscess, etc...prior to an actual surgical procedure so the patient can plan and be aware of their up coming conditions?
    10. Can an abscess cause urinary retention?
    11. Can an abscess or cancer or both cause changes in stool?
    12. Is surgery for an abscess riskier for incontinence if you've previously had a sphincterotomy?
    13. At what point of chasing fistulas do you begin to talk to your patients about proctectomy?

    Very complex for western medicine but not for traditional Indian medicine Ayurvedic Kshar sutra that’s how I was healed . Western medicine doctor as well misdiagnosed me too they said it was a horseshoe shoe fistula but I did not . Another problem in western medicine is they can’t properly diagnose fistula with an MRI it doesn’t always show however in Indian they use a probe that’s an ultrasound that can see exactly when the fistula is. I was lucky I had a good CSR that put in a seton properly.
    As well traditional Chinese medicine can cure this there’s a doctor in Mississauga ontario.

    Dr Chung, the surgeon confirmed that I have a fistula. I’m scheduled for surgery next week. The plan is to probe and find the tract and then perform a fistulotimy or insert a Seton . He also said that I may need a LIFT surgery 3 months out if necessary. All of this was done without an MRI . Is that standard ? Would t an MRI at least help confirm everything ?
