Evaluation encompasses assessment. When a teacher/academician does the evaluation (e.g. final marksheet) at the end of the academic year, s/he includes all the assessments (quizzes, projects, assignments etc) done throughout the year in it.
The purpose of the evaluation is to make a final judgement of all the assessment activities done around the academic year.
