there is difference between assessment and evaluation but we use it as synonyms as well. So for this question you can consider it same, means ans will remain same for both. yes if they ask for difference then you have to explain. already uploaded difference between assessment and evaluation.

long duration mein hai like 6 month wale than its summative if week later or month later hai than it will be formative
