    Thank you so much. My son has this on his ultrasound and since our doctors wouldn’t tell us a whole lot, we’ve been freaking out. He’s our fifth and we’re so excited, but sincerely, thank you for be interested a voice of calmness and rationality on RUclips.

    I'm 44 and pregnant with my 9th child. Never had this show up before, but some of my babies never had an ultrasound, and my oldest is 21, u/s wasn't as advanced then.
    I did the maternity21 test at 10 weeks, and we are very low risk for defects.
    I'm a believer in Christ, and I fully trust him, so this is just information and I will not be too concerned or stress about it.
    It is not a defect by itself. Just associated with defects that have already been ruled out in my case.

    This video makes me feel so much better! They had found 2 markers on my ultra sound in which I just found out today. Next I am going to go get a pediatric echocardio done to follow up. All my genetic and trisomy blood tests came out normal. Should
    I still be so anxious or take a bit easier!?

    • @DrNari
      @DrNari  7 лет назад +2

      Many times in an otherwise normal ultrasound, these soft markers are low risk. With two markers present, working this up further isn't a bad idea since the risks are higher. It looks like they are already doing that and have offered you a blood test such as a quad screen or cell free dna and this can help reassure you to decrease the anxiety a bit. best of luck.

    Hello doctor, we did cell free dna test (NIPS test) around 5-7 week and the result for all three chromosomes came back negative and low risk but in our 19 week anatomy scan we found that our baby has 1 focus and our OBGYN is not worried as my wife is 27 years old and everything else on ultra sound looks normal. Can you please let us know what do you think.

    ●Left ventricle show echogenic speck. it is soft marker for chromosomal anomaly, however isolated are commonly seen and not very significant.
    ●Bilateral renal pyelectasis is noted measures 5.0mm.follow up is suggested.

    Thank you for explaining this so clearly. I am currently 26 weeks pregnant. At 20 weeks, an isolated EIF was found. At 12 weeks, I did the NT scan + blood work and got a probability of 1 >10,000 chance of my baby having an abnormality. My 18 week quad screening came back negative with a probability of 1 in 8,000. I just got blood drawn for the Maternit21 test because I am very anxious about the possibility of my child having downs syndrome. Is it possible that the Maternit21 test comes back positive after all the negative blood work? Thank you for being available for questions.

    • @DrNari
      @DrNari  7 лет назад

      Your testing has been very reassuring given the probabilities you were given and you are likely very low risk.

    • @devinhall5937
      @devinhall5937 7 лет назад +1

      Thank you for your response. So the first trimester test just looks at my hormones that are produced by the placenta and the Maternit21 test actually looks at cell free DNA. So is it possible that the first test could have been a false negative since they didn't actually test fetal DNA? I can't seem to find how reliable those first trimester tests are.

    • @DrNari
      @DrNari  7 лет назад

      So each test is slightly different and with different false positive and negatives depending on the patient scenario. While the quad screen does look at 4 different hormones (AFP , hCG , Estriol, and Inhibin-A) and the cell free dna looks at circulating fragments of fetal dna, both are considered screening tests and not diagnostic tests (same goes for first trimester screening). Many feel the cell free dna can be more accurate in most scenarios but it is possible that one test is false negative or positive or vice versa. We usually don't do multiple screening tests unless one is positive and we don't want to go on to a diagnostic test like an amniocentesis so we try to get reassurance other ways.

      An echogenic intracardiac focus is a soft marker for T21. In an otherwise normal ultrasound, it can be a normal variant. I would relook at the video since I discuss this in detail in it.

    I had a small white spot found on my baby's heart at 19 weeks. The sonographer the called it a normal variant. Everything else was perfectly normal I'm 31 years old. My NT scan showed a measurement of 1.3mm. I've just opted to take the AFP4 quad test. If the AFP4 quad test comes back negative, do you recommend having the amniocentesis?

    • @DrNari
      @DrNari  7 лет назад +3

      Sounds like an echogenic intracardiac focus. NT measurements are based on gestational age so that value just depends on the normals they provided you. In an otherwise normal anatomy ultrasound that is low risk, the quad screen provides additional reassurance. The decision whether to go on to a confirmation diagnostic test such as an amniocentesis is a personal one based on your desire for confirmation; however, often in cases like this, the risk of a chromosomal abnormality is less than the risk of complication from an amniocentesis so most patients are satisfied with the reassurance of a screening test such as a quad screen.

    • @jenniferporter4407
      @jenniferporter4407 7 лет назад +4

      DrNari Thank you for replying. I did end up getting the Quad test results. They came back normal. My risk factor for Down Syndrome was 1:6618. My doctor sent me to a specialist. I had a second anatomy scan. The EIF was still there, but the baby was perfectly normal in all other ways. I opted to take the NIPT for my peace of mind. I'm currently waiting for my results. I'm super nervous. I had a first trimester miscarriage about a year ago. The baby had Trisomy-14. It's very much scaring me that I've already had a pregnancy with a chromosomal abnormality.
