You're giving defibrillation doses for aflutter?

Sorry I can see how that can be confusion. Basically it is rhythm dependent. The problem with afib and a flutter is when the rate is 150+ it may just looks like either a regular narrow complex tachycardia or an irregular narrow complex tachycardia. So I would treat accordingly. 50-100 for regular narrow and 120-200 for irregular narrow. But aha doesn’t give specific doses for sync cardioversion anymore they refer to the manufacturer recommendations. So whatever monitor someone is testing with or using in the street would be the correct dose.

@@medictestsecrets Right. You hit that well in the video but on your pop ups it says "Synchronized cardioversion: 200j, 300j, 360j". Could be me not listening well and just noticing that though lol. But very informative video. I wish I would've found your videos while doing cardiology and acls in medic school!

Yea I tend to use the higher energy dose recommendations so when I was making the video I was probably thinking of an irregular R-R which would be 120j-200j with escalating doses which was why I put 200,300,360. But I appreciate the feedback and letting me know that segment is a little confusing! Also I am happy you think the video is helpful and I appreciate you taking the time to watch!

Unstable A-fib, is it meds or cardiovert? It was unclear to clear the difference based on your video…or am I just missing something obvious?

unstable pts always get electricity, stable pts get meds

Unstable (CASH) will always be synchronized cardioversion, Adenosine is given if the pt is stable.
