The focus is both on attraction and gender and how people who don't conform to society's ideas of normality get judged. That includes trans people but also gender non confirming, gay and bi people. Holding hands in Japan in and of itself goes against socital standards, PDA in general is frowned upon, so add being LGBT to that and you are bound to be judged by those around you. It's a commentary on how living as yourself openly is still not really posible, especially with the current political views in Japan. The song is intentionally being vague so anybody in the community can relate to it. If you aren't straight and Cis just living peacefully can feel impossible because the whole world tells you you are the villain.
As someone who's described myself as feeling like a villain just for existing for some time now, for reasons similar to the song's topic as well as some others, this hits close to home... Wonderful delivery as usual, too. Will be listening to this on loop for a good while~

There's so many great lines in this version, I'm amazed.
