Your video has ignored and downplayed many important aspects of clinical research on the subject and you have just put across your biased opinion in favour of embolization. I'm neither saying surgery is the best nor going to agree that embolization is the best. Both procedures have their advantages and disadvantages. Interventional radiology has a bright future in tackling many medical & surgical issues and will hopefully make new such strides in future also. Having said that, I need to point out that some of the complications you mentioned (like incisional hernia, genitofemoral nerve injury) are unheard of following microscopic subinguinal varicocoelectomy (which is the current surgical gold standard). Either your comparisons have pitted all surgical modalities combined against embolization or your statements are outright as silly as you claimed the risk of renal vein injury in embolization! Please do an honest comparison with fair data and not over emphasize the role of embolization in varicocoele treatment. Also, what was not addressed was the failures and recurrences in the long run due to not addressing the gubernacular veins.
Hi. Agreed with you. But this wasn't a total fair comparison between embolization and microsurgery. It was reading of a letter made by SIR CIRSE society for insurance companies. Will make a fair comparison in future. Also, just read 2 meta analysis on data concluding that gubernacular veins and external spermatic veins don't make a difference and shouldn't be ligated. Not saying that it's an end conclusion. We have seen data change with higher powered studies. But sharing as a thought buster.

Your video has ignored and downplayed many important aspects of clinical research on the subject and you have just put across your biased opinion in favour of embolization. I'm neither saying surgery is the best nor going to agree that embolization is the best. Both procedures have their advantages and disadvantages. Interventional radiology has a bright future in tackling many medical & surgical issues and will hopefully make new such strides in future also. Having said that, I need to point out that some of the complications you mentioned (like incisional hernia, genitofemoral nerve injury) are unheard of following microscopic subinguinal varicocoelectomy (which is the current surgical gold standard). Either your comparisons have pitted all surgical modalities combined against embolization or your statements are outright as silly as you claimed the risk of renal vein injury in embolization! Please do an honest comparison with fair data and not over emphasize the role of embolization in varicocoele treatment. Also, what was not addressed was the failures and recurrences in the long run due to not addressing the gubernacular veins.
Hi. Agreed with you. But this wasn't a total fair comparison between embolization and microsurgery. It was reading of a letter made by SIR CIRSE society for insurance companies. Will make a fair comparison in future. Also, just read 2 meta analysis on data concluding that gubernacular veins and external spermatic veins don't make a difference and shouldn't be ligated. Not saying that it's an end conclusion. We have seen data change with higher powered studies. But sharing as a thought buster.
