Yes it works for everysize, the only difference is the distance between your neckline to where your design is placed in the front. Example for children it's between 2 to 2.5 inches down the neckline and adult is 3 to 3.5 down from neckline in the front

No it’s not because a vneck is low already and some are lower than others. You would have to determine how low your vneck is to determine placement. Some would be placed immediately after the v while some would require an inch or two after the v. It all depends on how deep your v is.
It is too close to your left arm and it’s too low about 1 inch think you should bring it up and bring it towards the centre and it will look a lot better
I will tell you why it looks like that, it's the size of the logo(4.5inches) compared to the size of the t-shirt(Medium). Not placement. The placement is good
I have been doing this for over a decade fulltime and I would definitely say you are close but missed... yes, I generally follow the collar seam (unless it is an oversized collar such as a scoop neck in which case I would eyeball 4" and keep as close to that as reasonable without interfering with the collar), however 3 1/2" down is clearly wrong on most situations. On a lady's small shirt your round logo would become a bullseye on her left bosom. Also, just as the shirt you wore had the logo into the armpit which it should never be... caution must be used on the downside measurement especially when shirts are to be worn by women as one doesn't want to have a target there. When in doubt, head up a bit...
I had forgotten this! I used to do heat press logos on shirts at my family’s athletic store. I am 70 and that was in high school!

how far down would you go for a 6x shirt? I have a client that is quite large and wants embroidered shirts. is it the same for larger sizes?

Yes it does. Just make sure you know the middle of your logo.
