I'm not one hundred percent sure, but I think the eyes the Cagney has is the gift for two reasons. One, isn't there a part in the boss battle with Cagney where his eyes are just eye sockets, that would mean, depending on what happened after or during the battle along with information about Cagney that we may or may not know, he doesn't have his eyes anymore. And two, even if it is not obvious the Blind Spector's eyes that he has in his hands are gone; plus the eye color of both bosses are kinda golden but the eyes that Cagney has here have a soft neon tint to it.
....
I may not be right, but there is a lot evidence pointing in that direction (including the fact that the comic was pretty dark before Blind Spector gave Cagney his gift).

If your talking about cagney being blind (since you didn’t specify) basically after defeating cagney in cup head cagneys eyes roll into the back of his head leaving empty eye sockets and in the game you don’t know if cagney ever got his eyes back or something so this person thinks cagney turned blind and since blind spectator (the ghost) can pop out his eyes which are on his hands he just popped his eyes into cagneys empty sockets (I explained the whole thing sorry)
