Terraria is definitely a game that requires a lot of time and dedication to. While it's gameplay loop is similar to Isaac runs in that there are clear points in progression you can declare the end point... it takes weeks. Like, if you are new it WILL take you weeks to reach hardmode, and even experienced players know that speedrunning is masochism. Also, nobody plays on Mediumcore, there is no shame playing on Journey Mode, and the only difference between Expert mode and Master mode is that one is painful and the other is more painful.
FYI: all music that doesn't feature chiptune is from Terraria Otherworld, a scrapped sequel to the main game that had more detailed graphics, a more orchestral music direction, and a broader emphasis on purging infectious biomes such as the Crimson and Corruption.

Samson’s chains and binge eater. Samson’s chains item is a reference to the early hardmode frail weapon, dao of pow. And binge eater is just various food items from terraria, like steak. 13:00 the health up items are just food items from terraria. That’s all. 13:04 the skeleton key item is a reference to a pet item dropped from the dungeon guardian, the bone key. 13:22 2 resprites. Placebo is the water bottle item, and the compass item is, well. Terraria is a worth while game. If you feel stuck ever, literally no one will blame you for using a wiki. Also when/if you do, some of the music here in this mod came from popular Terraria mods, and some of the mechanics are outdated simply cause they fit better.

Some of Terraria's items just don't make sense in isaac tbh. there are pretty much no "status effects" to protect isaac from, so the mod creator probably grabbed a generic OP item, which turned out to be the Ankh Amulet and slapped it on an another OP item, the Rock Bottom.
The items that DO make sense tho, such as Psy fly and Cat o' nine tails, those ones go hard. idk if the spirit sword is changed to Meowmere (because the regular terrablade is mom's knife, it seems), but it would be a nice fit.
The Rock Bottom change is called Ankh Charm. In Terraria, it's crafted by MANY items that make you immune to Status Effects, and it's basically an amalgamation of them all. You can further upgrade it with the Obsidian Shield, crafted by a Cobalt Shield found in the Dungeon and an Obsidian Skull made by 20 Obsidian I believe, and it becomes the Ankh Shield, which gives the Immunities, Knockback Immunity and 4 Defense
Honestly, the Ankh Charm, the sprite used for Rock Bottom, should have been the Evil Charm instead, since both share the effect of making you immune to a wide variety of debuffs.
I do enjoy how at the end, delirium was teleporting like an endermen in the rain and it was a health race on whether either it or lavencas computer would die first

A lot of the resprites make perfect sense! Rock Bottom: The Ankh Charm protects you from most debuffs. Psy Fly: The Terraprisma is a summon that moves incredibly quickly, usually taking out weaker enemies before they have a chance of reaching you. Like the fly with projectiles! Mom Transformation Knife: The Terra Blade is a powerful weapon you get close to endgame, and requires crafting several other weapons together, like how transformations work. The first key piece is an old key mold from pre-1.3, and the second is what you combined it with to create a key - in this case, a Hallowed Key, which fits perfectly for getting it from angels. There's several others I want to talk about, but I'm tired at the moment, so it'll have to wait. My only real complaint is that most of the soundtrack swaps don't really make sense to me, minus Downpour having the rain music.

As an experienced Terraria player, the actual comparison of Terraria Item to Isaac Item is... varying. Most of them make at least some sense. Psy Fly becoming Terraprisma isn't EXACTLY on point, but it's close enough to where you can see where it came from. Some of them are incredibly accurate. You didn't pick it up, but Spoon Bender became Chlorophyte Bullets which, yeah, that's what those do. A couple make no sense at all??? Why did Guppy's Tail become Nectar, which only summons a little hornet pet? Why did Rock Bottom become the Ankh charm when evil charm, which also grants immunity to debuffs, was RIGHT THERE? Also: If you're more the type to watch people play games rather than actually play the games yourself, might I recommend a channel named gromek999? He has a show called Terraria Veterans, where him and some friends play on the hard difficulties. Hijinks ensue. Might be interesting for you to watch?
The Rock Bottom resprite is Ankh Charm. Which is a truly awful choice, since Evil Eye Charm exists, and gives you status immunity, just like the Ankh Charm in Terraria, but NO, it's Rock Bottom apparently.

Mom Transformation Knife: The Terra Blade is a powerful weapon you get close to endgame, and requires crafting several other weapons together, like how transformations work.
The first key piece is an old key mold from pre-1.3, and the second is what you combined it with to create a key - in this case, a Hallowed Key, which fits perfectly for getting it from angels.
There's several others I want to talk about, but I'm tired at the moment, so it'll have to wait. My only real complaint is that most of the soundtrack swaps don't really make sense to me, minus Downpour having the rain music.
Underground Crimson for Womb is another good choice, I'll admit.

The secret room soundtrack is SO GOOD
As an experienced Terraria player, the actual comparison of Terraria Item to Isaac Item is... varying.
Most of them make at least some sense. Psy Fly becoming Terraprisma isn't EXACTLY on point, but it's close enough to where you can see where it came from.
Some of them are incredibly accurate. You didn't pick it up, but Spoon Bender became Chlorophyte Bullets which, yeah, that's what those do.
A couple make no sense at all??? Why did Guppy's Tail become Nectar, which only summons a little hornet pet? Why did Rock Bottom become the Ankh charm when evil charm, which also grants immunity to debuffs, was RIGHT THERE?
Also: If you're more the type to watch people play games rather than actually play the games yourself, might I recommend a channel named gromek999? He has a show called Terraria Veterans, where him and some friends play on the hard difficulties. Hijinks ensue. Might be interesting for you to watch?

The Rock Bottom resprite is Ankh Charm. Which is a truly awful choice, since Evil Eye Charm exists, and gives you status immunity, just like the Ankh Charm in Terraria, but NO, it's Rock Bottom apparently.

If you do the correct amount of damage you can erase tainted Esau with the eraser I’ve only ever seen it been done once
You can't do that anymore, you cannot damage Esau anymore in first place
I don’t recognize a lot of the music used in the music mod. Either it’s all new music or it’s some sort of remixed version of some of the tracks.
Edit: I think much of the music is actually music that was weitten for Terraria: Otherworld
