  • Transient tooth sensitivity after filling is fairly common, tooth pain weeks after filling is not a common occurrence. This video addresses the possible reasons for tooth sensitivity months after the filling is done. Biological reasons such as the depth of the cavity, closeness to the nerve and plaque contamination. Physical reason has to do with the micro movement of the remaining tooth structure wall, affected by the microscopic hairline fracture, shrinkage of the composite or white filling material known as polymerization shrinkage, sleep apnea, C-factor, void in the filling material, uncured composite and micro gap.
    If the tooth is still sensitive after one or two bite adjustments. than the bite may not be the problem. the filling may need to be replace.
    If the pain persists for more than 90 days, then the tooth may need Root Canal Therapy.

    Hi David, I have just had cavity filled in my tooth but I look in the mirror and can still see a little bit of brown patches on the tooth my dentist says this is not a cavity but just staining does that sound correct?

    Hi Dr,
    I have a shallow Class I filling on an upper posterior molar. Two months ago, I accidentally bit down on a clam shell fragment. It was small, sharp and hard. I heard a loud crack noise but no pain.
    A few days after, I noted the area where this filling was (on the occlusal) was sore when I chewed on food. The soreness went away after a week or so. No sensitivity to cold or hot.
    Four weeks later, this molar throbbed at night and I could sense the pulsating sensation. This occurred a few times before it settled down. It was not overly painful and I didn’t have a headache or fever. About a 3-4/10 I’d say. No sensitivity to cold/hot or pressure when chewing.
    Another four weeks later, so two months after the incident, I notice an ongoing tingling sensation at random times when I’m not chewing. It’s almost as if there is a fine needle/pin that is pricking up and into the root. It’s not overly painful, more of an annoyance which draws my mind to this tooth. About 3/10 in terms of sensitivity.
    X-rays have indicated nothing abnormal and fracture tests have been passed with nil issues.
    From intraoral photos I have noticed two small indentations in the filling itself from wear and tear, quite possibly from the trauma of biting into the clam shell fragment.
    Would you have any advice as to what might the cause of my sensitivity?
    So far I’ve come up with the following:
    - damaged filling
    - underlying small fracture, possibly underneath the filling
    - secondary caries underneath filling (the occlusal became sensitive to cold out from nowhere 6 months ago. It eventually went away). The trauma just reignited the nerve and pulp.

    Hey I went for a full up last month and never felt any pain until last week , it's starts at the bottom and then to the top very much imbearable I went to the dentist and she said I must extract the same tooth which got filled last month, should I get a second opinion?

    I have done my all filling process today....and now it is some type of sensation on my tooth.....and very difficult to chew anything...is it normal??please reply..¿

    Hi great video!
    I've had attachments done to my teeth for invasalign 5 days ago and all of them are extremely sensitive when I brush, I can't even pass a brush it's excruciating pain! However eating is ok only sweet things irritate the teeth. I'm using sensodyne and colagate sensitive toothpaste. My question is will this pass or should I contact my dentist? I had a routine dentist check up before I had treatment and all my teeth were fine. I had fixed braces as a teenager and never had this problem is it due to age I'm in my late 30s or the process of having the attachments as some teeth felt sensitive
    but I was able to manage it. I'm dreading have the attachments removed! Many thanks

      Thank you. Sorry to hear that your teeth are sensitive after the attachments. For the attachments to stay on the teeth, the tooth surface has to be conditioned with a mild acid, on the enamel surface, this is not a problem at all. However, if you have gingival recession with exposed root surface, the root surface can be sensitive after exposure to the conditioner(has to do with the pH). You may wish to stay off an acidic diet for 1-2 weeks, that includes coffee and wine. Use alkaline (pH > 8) toothpaste and mouth rinse. If your teeth are still sensitive at the time of attachment removal(let’s hope not), you can ask the dentist to give you a local anesthetic to alleviate the pain and anxiety.

      @@drdavidchengHello Dr. I had a front tooth filled 2 months ago. It was a deep cavity. Now it just started very bad pain and sensitivity. The cavity was close to gum on one side. Any idea what could be causing such pain after 2 months? Thank you

    Did you have toothache after filling, it's been a week since filling today, but the pain is not gone, I went to the doctor today.
    He did x-rays and said that there is no infection of any kind, just use sensodyne toothpaste, but I am very tired of the pain.

    I have a cavity in my tooth with little sensitivity to hot and cold. My dentist suggested I could get a filling which will remove the decay and the sensitivity will not aggravate or other option is to get RCT. I was thinking of getting the filling as the sensitivity is not much. Kindly advice. Thanks

      HI
      Little sensitivity can be because the cavity is small or the nerve is dying, therefore the dying nerve may not respond ‘normal’ to hot and cold. If your dentist is talking about the possibility of RCT, then the cavity is quite deep already.
      You can always start with the filling first and if the sensitivity remains or worsen, you can do the RCT later.

    I have a terrible back of head and neck pain / Strain after my tooth filling it was fine after 3 days then the tooth sensitivity started again and so did the neck pain. Going back to the dentist she said neck pain has nothing to do with the filling.

      There are no documented studies linking filling to neck or back pain. In my own experience as a dentist, I did not see any evidence of linking the two. My suggestion: wait for a week or two for things to settle down.

    I have a question so I had my fillings in my upper left where my molars are it’s been over a month I had some sensitivity but then it started slowly going away then I put down on a butter knife little too hard and some piece of my tooth came off very minuscule I went back to the dentist they said no damage was done but I’m still having sensitivity and a really weird achiness on top of it when I eat hot or cold foods or drinks is this normal and should I wait a couple months? To just have it poured if it continues I will never do a root canal after having an almost infection Causing me severe pain but now it’s just a weird achiness and really bad sensitivity even when I breathing I also had a really bad problem on my right side where the route was dying and Caused me really bad pain so I had it poured I’m afraid that I’m repeating that

      @@User77989 Yes. even what happens when I don't eat there is a strange sensation in that area. I think the fillings are destined not to be used for chewing hard food

    I started few filling fr last year July to October, am having sensation on the last one that they did ...first they use the white filling i started getting sensation, they change it to silver still getting sensation.
    I went back there, he started a root canal procedure n told me to come back in a week...during that week am still feeling that sensation...so i told him to remove the tooth because the pain was unbearable.
    When he removed the tooth he told me is 4 roots was there n he only inject 3 roots that what was causing the pain and sensation...am still having that problem he send me to do a CT scan they said they haven't seen anything wrong.
    Now the sending me to pulmonary, it's my upper wisdom tooth close to the back...the r saying the can't figure what the problem ... am sensitive to hot r cold i ave to stop eat alot of things,can someone please tell me my next steps to take ...am 38 the only time felt a toothache was fr 2004 1 time,since this dentist went in my mouth fr 2021 its a disaster.

      Sounds like there are few issues involved. My suggestion is to seek a second opinion from another dentist. Replacing from white filling to silver filling may or may not solve the problem, depending on the problem, for example if you have a fracture tooth extending to the nerve, most likely the filling material will not change the outcome. Generally, sensitivity can be from acidic food group such as wine or citrus food with pH around 3, you can get CTX3 rinse with a pH of 8.3 (citrus flavor is milder, mint is a stronger) from the Carifree company to neutralize the acidity from the food. Fracture root, need extraction, bone graft and implant placement. Sleep apnea or upper airway resistance can lead to bruxism related to microarousal, and can in turn cause teeth sensitivity. Daytime clenching from anxiety can also cause teeth to be sensitive. I hope this helps. Thank you.

    Hi Dr. Cheng! I just got my tooth filling done yesterday. Ever-since, I’m feeling sensitivity on that tooth when I chew food, which is making me chew the food only from the other side of my mouth. I experience no sensitivity with hot/cold drinks. Please let me know if that’s normal or should I consult my dentist.

      Hi Ayesha,
      As with any filling, the tooth usually goes through a certain amount of trauma, whether it may be hot or cold sensitive or biting down sensitive. It is best to wait and see for one to two weeks for the tooth to settle down before any decision.

      Hi Dr. Cheng! I hope you’re doing good. An update on my case, the pain during chewing after my composite fillings didn’t get any better after a month. I visited my dentist again, who says I have no high points on my fillings, thus only applied a desensitising gel on my teeth. It didn’t help me at all. Eating food has been a struggle ever-since I’ve gotten my fillings. I’m planning to visit a new dentist now. Can you please give me your opinion on what should be done now? Should I get my fillings replaced?

    Had no sensitivity before filling but have sensitivity after filling. Been weeks now, ready to be done with my dentist. I just had black lines, dentist didn’t even give me injection but it’s so annoying. Can’t chew on the right at all.

      @@mikepaquette1245 hi👋🏾 , I recently went to the dentist & he filed it down & it’s much better. It’s not perfect but can chew, it was a bit too high. Certain things still irritate it so I think it needs to be filed down a bit more so you can ask your dentist about that.

    So it's not normal that a filing done 5 years ago is still sensitive? That doesn't sound good...

      Depending on the type of sensitivity you are experiencing. You can have sensitivity from an acidic diet, for example wine, lemon, lime or citrus fruits. You could have sensitivity from the gum recession. You could have sensitivity from tooth fracture.
      It is important to determine what is the cause of the sensitivity. It is best for you to see your dentist to determine the cause.

      @@drdavidcheng I do drink a lot of monster energy none sugar version, my teeth are filled and they are super se service too cold and candy is this normal?, I feel they hurt more now than when they were with the cavity, how does they make sense? I’m really upset this is even happening.

    My issue is that the filling was sensitive the. They put a desensitiser then everything was fine but then the pain transferred to my bottom teeth . I can’t take this anymore I can’t even sleep straight , I have to sleep sitting up . 😭😭😭 I wish I had not done this , I was perfectly fine before . God please help me

      Generally speaking if you have to sleep sitting up means the nerve of the tooth is dead, the tooth is going to need root canal therapy. If the nerve is dead, in my clinical experience it is possible to feel the pain transfer from the top to the bottom on the same side. Once the original tooth problem is solved, then the pain on the bottom should go away.

    Hi, I got my general routine checkup from Hutchinson Metro Dental at Bronx, and they said everything looks good I had some fillings before that they said need to be refilled and the day doctor touched my tooth I am having this super cold n hot sensitivity Which I never felt before. He did the X Ray and the tooth does not even need root canal as the doctor said and as I am saying it was never bothering me. But now I only regret my decision going there and have spent so much money with only worse outcome. Please suggest what should be the next step

      Hi Rama,
      The tooth usually takes two weeks to settle down after the filling. X ray is only one of the metrics to determine the need for a root canal. If the tooth has spontaneous pain after two weeks, a root canal may be necessary.
