Since you mentioned that this might not be an option for you, the best advice we could give you to help avoid pain in the future would be to stick to a soft food diet (so you take it easy on your jaw joints), avoid stressful situations (because this will cause you to clench) and to do jaw exercises, stretches and massages on your jaw joints to loosen the muscles. i was given a mouthguard with a ridge that goes toward the top teeth and also a mouthguard for the top teeth. since using this i have had worse tmj issues. is an appliance with a ridge on the bottom normal?
Thank you for your question. We do not use mouth guards in our office because we believe it does not fix the root of the problem which is the jaw joints. We use modified splints (also known as modified gelb appliances) as well as custom ALF appliances for TMJ problems. Our lower appliances do not have ridges because we want your lower jaw to be able to move to a comfortable position. With ridges, it locks you in a bite which restricts cranial motion. I have a friend of mine who has been endentulous for over 12 yrs (no dentures) He has had no support in his mouth. So he is riddled with neck/shoulder pain, hearing loss as well as sleep apnea. He has postural issues as you mentioned in this video rounded shoulders and forward head posture that I believe stems from the TMJ. His left condyle is worn out. His insurance is why he is in the state he is in Medicare has no dental and he recently got Medicaid 2 yrs ago and where they pay for the very least treatment and ignores the TMJ completely. I would imagine if the denture is made without the TMJ taken into consideration it could make his pain worse. Do you know what kind of dentist he should follow up with a TMJ specialist or a prosthodontist??

We too wish insurance would cover more and stop skipping over TMJ benefits considering the jaw joint is one of the most used joints in the body. Regarding your friend, you are correct. There is a possibility the underlying problem is a TMD/TMJ issue. If any appliance or guard is made without taking the jaw joints into consideration, it can cause many uncomfortable symptoms and could make symptoms worse. We always recommend having a TMJ MRI taken of the jaw joints so you can get a better understanding of what is happening internally. If there is an underlying TMJ problem, we do not recommend getting braces as it will not treat the problem. Our goal is to get the joints working properly and then straighten the teeth.
Great question! Braces are used to straighten the teeth. If you have a TMJ problem, you need to address that first. In our office we always treat the jaw joints first. Once the jaw joints are in a stable position, we then will consider braces, ALF appliances or aligners to move the teeth.
