The thing that i don't understand yet when i asses a hemiplegic patient in stage 1 (flaccid) and start treatment plan he should go through (progress) all stages with physical therapy intervension in the order ? Stage 1 - - - stage 2..... Stage 3 stage 4...and so on Thank you
Dear doc, Don't think about progressing the patient through stages. Just focus treatment of restoring near normal motor patters and normal sensory experiences related to motor performance. The progression of exercises is the key. I hope you have watched both videos of Neurorehabilitation secret series. 😊❤️

One is able to do movement with legs like waliking but there is lot of stiffness while walking or excercise with weight..and pain after stretching or weight excercise which stage is this ? Is it a postive recovery sign
Hi. Thank you so much for this video. My gran has recently had a stroke (2 weeks ago) and has gone from being completely independent to bed ridden. She hates it and is desperate to get better but she is still in stage 1. She has had about 7 physio sessions (45 mins long) and they are helping her to sit and stand but her left side doesn’t move at all. What are the chances of her regaining movement in her limbs? Even if it’s just to walk a few steps? Does anyone have any experience or know anyone that’s got past this 1st stage. I know every stroke i different but any help would be appreciated! Maddie XO...i.suffered a ischemic stroke on 2017,wch left me a left hemiplegic patient. Based on the video presented on Brunnstrom Stages of Recovery,i am on stage 3, spasticity is still so evident, i can now walk wd a quad cane ,bt my upper left portion is still so spastic that i have no voluntary movements on my arm and hand including my fingers. I am on physical therapy for 5 yrs now,and aiming to reach stage 7 on recovery.pls tell your gran to never give up on aiming for full recovery physical therapy shoul continue untill she has reached stagee 7 in the Brunnstrom Recovery.Best of luck and God bless your gran.

Hello sir I am physiotherapy student.. I have 1 doubt.. in stage 2 you said that there is spasticity begins first in biceps and quadriceps is there any reason behind it..?or they are anti-gravity muscle so that it happen..plz explain sir.. thank you..
Both biceps and quadriceps are the strongest components of the flexor and extensor synergy respectively. I have also given the neurophysiological explanation in the 3rd part of Brunnstrom series for increase in flexor tone in UL and extensor in LL. Also the Number of muscle spindles in these muscles are comparatively much more than rest of the muscles and so a stronger hyperactive muscle stretch reflex develops here first.
Both muscles are linked strongly in the upper limb extensor synergy ( shoulder adduction and elbow extension). And so to dissociate the above two actions is important to break the extensor synergy pattern. Stroke patient finds it difficult to keep the elbow extended during shoulder abduction movements when triceps and pectoralis are strongly linked. So when therapist utilizes strategies that trains the patient to maintain elbow extension in varying ranges of shoulder movements, this results in dissociation and gaining of new movement pattern. Hi Doc, im a physio student and my patient is 2months post stroke and he develops voluntary movements. His Legs can bend and extend gravity eliminated, and today he started moving his fingers ext and flexion. Do you think he will develop spasticity?
Finger Movements indicate preservation and intact functioning of corticospinal tracts. He will have good recovery. Follow the NDT therapy concepts along with CIMT to have good functional recovery. The above is true for the upperlimb and in lower limb the extensor motor patterns recover first as compared to the flexors. The explanation to this lies in the brainstem motor control centers and I have explained about them in detail in previous Brunnstrom therapy videos. One has to understand the influence of Rubrospinal and vestibulospinal pathway to answer your question. Watch Upper limb and lower limb synergy pattern videos to know more.

Hello, It varies from patient to patient but commonly persists from few days to few weeks in stroke patients. Can last longer in pure pyramidal strokes ( white matter lesions) where flaccidity can persist for months.
Sir why does calf muscles keep aching after cases if hemiplegia. Patirnt complain of calf pain after recovery n how to deal with it. Is it due to weakness
I have chronic pain from stroke in my arm that started in my shoulder. After I got through the phase that I had the highest spasticity (which included claw foot), it moved to my elbow. It's extremely frustrating that it's untreated because nobody knows the cause.
Treatment guidlines for stroke are not to be strictly adhered. Regarding EMS, it can be given as FES to facilitate desired Movement even upto stage 5 and 6.
