      Hello,
      thanks for watching Physio classroom. Here are some more extra information to help you facilitate moments in flaccid patients:
      1. Use associated reactions
      2. Place the paralytic joint you want to facilitate in mid range and in gravity eliminated position ( suspension is preferable).
      3. Also simultaneously use roods facilitation techniques during irradiation / Imitation.
      4. Keep using the same concept in weight bearing activities too.
      5. Long duration Tens in weight bearing will also help in facilitation of tone.
      Note: only patients of pure cortical stroke will fail to respond to above treatment as they have pure pyramidal lesion.

    Hello Sir,
    How the overflow mechanism works? What's the physiology of it? Is it due to a neural firing happening between two sides of the brain or is it muscle energy transferred side to side?

      We always recruit our muscles or body parts depending upon the load of work.
      If you have to lift a pen and if you have to lift a bucket the muscle work will automatically be adjusted to higher level with more number of muscles participating.
      Over flow simply means to lend or donate energy at the target joint from the surrounding areas.
      Its same as irradiation mechanism which I have explained in PNF principles videos.

    What are the chances of someone who has had a stroke returning to how he was, particularly in overcoming spasticity? Most physiotherapists seem to agree that it is incurable. What a prospect!

    Spasticity in adductors and biceps are very likely to become big problems for hemi patient, such as risk for contractures, atypical patterns for manipulation and gait, poor alignment and posture...why do you exercise these muscles first? How to prevent challenges above?

      Spasticity only becomes a challenge when it is uncontrolled and patient is not trained to voluntary contract and relax different muscle groups or movement patterns.
      Alternate or reciprocal movement training from the beginning of the rehabilitation can ensure that flexor or adductor spasticity donot become strong enough to interfere in performance of upperlimb and gait activities.
      There fore both the agonist and antagonist muscle groups are facilitated and I have mentioned hip adductors and elbow flexors because they are the easiest to be facilitated reflexively to motivate a stroke patient to participate actively in the rehab sessions.
      Also please note that we have to choose the best treatment strategies from different Neurological approaches like Bobath/ NDT, MRP, CIMT, Perceptual motor control, Brunnstrom, PNF etc.

    In raimestes LL shows vice versa ..and if the patient is performing adduction on unaffected leg so it should be abduction on affected leg that is the leg should fall out

      This will become clearer more and more as more Brunnstrom treatment videos will be uploaded.
      We have to take the best strategies from both the Approach and customize it for patient's treatment.
      Brunnstrom never recommend to strengthen synergy patterns.
      They are to be facilitated and new combination of Movement patterns to be introduced ASAP.
      Brunnstrom utilizes concepts some of which have similarity with the NDT and PNF concept of TREATMENT.

      Thanks for your question
      Cognitive and Psychological and perceptual problems are also often associated with stroke and are needed to be dealt with in a holistic manner ( coordinating with Psychologist and Neurologist).
      There are strategies to correct hemineglect and improve the motivational level of the patient.

      Please read the Brunnstrom concept of treatment.
      Elbow flexors are being facilitated in the stage one, i.e. stage of flaccidity.
      They are not facilitated when spasticity develops
