- Multiple bilateral symmetric gray-matter lesions should always suggest a toxic or metabolic encephalopathy. Carbon monoxide (CO) intoxication may mimic the flu or drunkenness. CO selectively damages the medial lenticular nucleus of the basal ganglia of the brain. It may also affect the cerebellum and the hippocampus. Rapid diagnosis and treatment is successful for most patients. The most important therapy is oxygen, and if possible hyperbaric oxygen therapy (HBOT).
