  • Meeting Objectives:
    Participants will increase their understanding of the challenges of providing care for persons with substance use disorder in rural settings.
    Participants will understand the benefits of engaging stakeholders outside of the hospital system in addressing SUD.
    Participants will understand the process for making community connections to strengthen the support network for persons with SUD.
    Purpose:
    The Hospital Systems Addiction Care Community of Practice promotes the development of interdisciplinary hospital systems that identify, engage, treat, and coordinate services for people with substance use disorder to improve patient and hospital outcomes in New Hampshire. It is funded by the NH DHHS Bureau of Drug and Alcohol Services and coordinated by the Foundation for Healthy Communities and Growth Partners.
