This is most likely a GC issue. The GC should be going over the plans with the subs and the subs should be asking the right questions. Sometimes the prints are wrong especially with kitchens and baths and changes are made by the homeowner. These things need to be relayed to the subs. There really should be a punch list carpenter onsite or available to catch these things before the electricians start poking holes all over the place. This is a disorganized mess, and I blame the GC for not running a tight ship. Some things are unavoidable...it happens, but the way they went about addressing the changes is preventable.
Have you ever worked in construction? The subs get a set plans and should be able to read them as a retired plumber, I have seen it get more and more like this before I was forced to retire and in my opinion well I better not say due to youtube standards

I've seen remodels in better shape, after the demolition. This builder knows damn well they're not even close to being ready for inspection.
