Do you think you have clear idea for sounds before starting, or they start to take shape while designing sounds? Also editing: Funny to notice that as in UI design, different buttons etc. have different levels of visual importance (referred as visual hierarchy), same idea applies to sound design.
It's a mix of the two! Sometimes I know exactly what I want, but often, I figure it out as I go. The trick, though, is to at least have some sort of plan (even if it's not fully formed) on how you're going to go about designing a sound. Even if you abandon that plan, it saves a lot of time and lets you pivot to a new plan quickly.

Usually, but not always. Sometimes it's simply a lower volume, EQ to hide it more, panning to bury it in the mix, etc.
