I like the idea of the premonitions coming from the mental illness of each Malkavian. The Malkavian with schizophrenia will have hallucinations where as a Malkavian with anxiety disorder have panic attacks when something bad is going to happen like a painful spider-sense
This was before 5th Edition but I made a Malkavian NPC who was under the delusion that he was created, not by another vampire but by God Himself for the purpose of hunting down and killing "true" vampires. The twist? His delusion was so strong and complete that it gave him the True Faith merit. That's right, this little traitor and hunter could brandish a cross and repel other vampires.

I feel like playing into depression and powerlessness can be much more real than playing wacky Joker style characters. Feeling frustrated at not being taken seriously, or the huge expectations you have to fill the exceptionally rare situations people DO is something i think most normies could relate to(but maybe thats just my MDD and PTSD talking)
I'd say don't be afraid. The important thing is that you try, and that everyone at the table has fun. I'm playing a Malk at my table for the first time and it's very exciting.
My malkavian is a knitter.. She gets her visions from her knitting and gets frustrated when other people look at her work and just see knits and purls when she sees the answers however the questions elude her. When she is asked to tap into her powers she often gives her answers in knitting patterns that she writes
My character is a Malkavian combat Trickster with Obfuscte, Auspex, Celerity and Dementation...I drove mad many enemies, also diablerized one sabbath member... got great fun!
12:40 Hi, next week I ll be playing my first rpg ever and the character that I created is something like this, btw the setting is in Istanbul, Turkey and my character is a nihilist fledgling Malkavian Imam which has dealt with themes like depression, loneliness and melancholy all his life and describes himself as a thrice damned Malkavian which was already fed up with life itself and now he is a freaking immortal that has to continue existing this shitshow for the rest of his unlife.

I adore the clan of the moon. My malky has a haven in a theater so she will quote movies at random that needs some dissecting to piece how it relates to the current conversation/investigation. Her oracle archetype makes her empathetic and on a dime recklessly violent.
My Malk was Sabbat embrace, a devout but flawed Catholic priest, who was secretly having an affair with the house keeper. His sire embraced him with the *intent* on breaking him. "Where is your god, now?" Instead, he thought that God was punishing for his sins, and decided that he was damned for all time... But he *could* save others so he tried to redeem is sire. It didn't work and in the end, he "killed" him and fled.
If you really want some inspiration for a malkavian character, go play disco elysium. I think the main character is good example of one; DiD, strange visions and behavior, yet have moments of sanity. Sometimes it's played for laughs, but also taken very seriously. Hell, your derangement could be your skills talking to you!

My little Malk lady has an apartment with books everywhere. Books in every room. A very messy library with a singular bed shoved in a corner.
