Ludicrously, the male character of Joe was played by a female actress, Karen Black - without doubt the worst casting of her entire career (and Karen regretted 'Trilogy of Terror' - when THIS was the real trashy horror role!)

The actor playing a trans woman in this movie IS a woman - Karen Black, in the most ill-conceived casting of her career. So if she 'looks better than' the other women, that's merely your subjective opinion/ranking of these different WOMEN's looks. Casting a woman as a trans-identified man was utterly ludicrous - an insult to women and a brazen misrepresentation of the reality of self declared, male 'trans women'.
