with due respect, first of all Thank you for this informative video which explains four principles in a brief manner, Now I would like to ask 2 question related to the video presentation. question 1: In the video while explaining the fourth principle 'Justice' you first said that, " all patients with similar circumstances shall receive the best possible treatment." however, later on in the same slide you further talked about resource allocation in two different circumstances such as circumstances pertaining to mental health and circumstances pertaining to accidents. Kindly clarify what is the meaning of "similar circumstances" as per the explanation, because prima facie these 2 are different circumstances and hence, should be dealt differently, however, if there were 2 different cases of accident and both patients were treated any differently then the question of fourth principle of justice would have been applicable in such scenario. kindly clarify. Question 2: In the video while explaining the second principle 'Beneficence' you mentioned a term ' Medical paternalism' and you said that " it never occurs in practice". my question is, say for example there is a mental patient whose condition of mind is as such that he is a threat to both himself and others, however he does not wants to admit himself in the hospital, but the doctors strongly feel that he should be, even then medical paternalism will not be applicable in practice? for instance the patient does not have any gaurdian or relative, can the doctor's decision will overrule the patients decision then? However in the UK variation exists on a regional basis and this isn't always the case. Question 2 - medical paternalism: Correct under the mental health act patients may be deemed of significant risk to themselves or others, and can be detained against their will. However, where they recieve treatment it should be considered in their best interest, utilising the principle of beneficence. Under normal clinical practice, a patient has ultimate right to autonomy.
