- If you've had an MRI for your shoulder pain, you might be familiar with the term "supraspinatus tendinosis", which is a very common finding.
The supraspinatus muscle is one of your 4 "rotator cuff" muscles and is a common cause of shoulder pain.
In this video, I explain what "supraspinatus tendinosis" means, and I also explain the anatomical location of this muscle.

It's been 3 months since I got my mri report it says the same supraspinatus tendonosis, pain is gone almost sometime I feel it when I sleep on affected shoulder
but m not able to gain my old strength back there is weakness I do physio exercise sometimes but m confused how frequently I should do exercise and how much time it can take I really want to get back to the gym😢

Thanks for the explanation. I recently got the results of my MRI and it states I have Supraspinatus tendinosis. But my physio therapist also says I have frozen shoulder. (I have yet to be back to my GP to discuss this development.). Is it correct that I will have to wait for my shoulder to thaw before really tackling the tendinosis? Thx
I would do my best to treat both the tendinosis and the frozen shoulder at the same time. There's no need to wait for your shoulder to thaw before improving the healing of the supraspinatus tendon, at least in my opinion. Good luck!

I am going to talk to my therapist about this strategy. Right now she just wraps me in towels with heat pad. No excercises and pain just getting worse.

Does tendinosis cause popping. I have been diagnosed with mild tendinosis in the supraspinatus and infraspinatus. I have popping in the back of my shoulder or side of my upper arm/shoulder.
It doesn't necessarily cause the popping, but you can certainly have both, especially if you have some swelling in/around the joint as well.
@@GordonPhysicalTherapy I did have an MRI recently and it showed mild tendinosis. I was told by a good p t that tendinosis doesn’t cause popping and he said that mri’s are notoriously wrong and it’s more than likely not the tendinosis causing the problem. So, my physiatrist is sending me to an orthopedic shoulder surgeon. If tendinosis doesn’t cause inflammation, it seems to me that tendinosis is not the problem for popping/grating and something else would be causing it?
BINGO!!! Your video reminded me of an earlier injury (24 years ago) that has flared up chronically for about a year. What about steroid injections?
In MRI it shows 2.0 to 2.5 mm tear in suprastainus tendons....is it heal by physiotherapy...or i need to go with orthoscopic surgery...
If you could help with your interpretation of my MRI results, that would help greatly:
1. It says: suprapinatus tendinosis "with intersitial delaminating tearing and it's insertion"....what is your interpretation of that last part? (It sounds like a peeling away of some layers of the tendon, to me, and is that considered a rotator cuff tear?)
2. Also, I have: "truncation of the free edge" of the posterior superior labrum. Is that the same thing as a tear, or some other abnormality?

Wow! I’ve been binge watching all the videos on your rotator cuff playlist. It has been so helpful to say the least. I had an MRI to rule out a rotator cuff tear. The findings were tendinosis @ the intersection of the supraspinatis & infraspinatus. Now I know why resting my shoulder has made my pain worse. Ready to start my physical therapy much wiser thanks to you. A million thanks!!! 👍🏻

doctor told me i got micro tears in my supraspinatus , cause by training over use a and he told me it's caused by my shoulder deformity , but it's been a more than a year and it didn't heal , i rested for 3 months without PT or pills and went back training and the same problem occus , my problem is that i don't have pain just slight pain but when i raise my shoulder or use it is feels weak , stiff an annoying
That's tough to take 3 months off and then try to return to training... the rotator cuff muscles don't heal very well on their own, so rest doesn't typically work very well for overall healing. At this point, I'd try to slowly improve your rotator cuff strength without causing pain, which will help with your healing process. Are you familiar with rotator cuff strengthening exercises?
@@GordonPhysicalTherapy nope i didn't try them but the problem is that whenever i lift my shoulder up or hold my hand in my face level my shoulder starts feel annoyingly stiff , weak and tired with slight pain
@@jemahboulmaztoulmaztoul1514 try starting with gentle rotator cuff strengthening with your arm in the neutral position at your side. As you build your strength, lifting your arm and having your hand at your face level should get easier.
@@GordonPhysicalTherapy sorry i didn't understand what kind of exercices you mean ,if you have an example , im wearing one of those bracers that pull shoulders back would that help recovery ? cuz doctor told me my main problem is that i have a bone deformity so that when i box tendon make friction with bone

Thanks Dr. Gordon for the clairification of terminology. Since my rotator cuff diagnosis (xrays only) I have studied the shoulder joint intensively and see a physio 2x per week and they treat me but Provide little or no info. In my case if I open a box in front of me with a knive and draw it across my body from medial to lateral I feel intense pain. That tells me it is infraspinatus or teres minor but I keep hearing deltoid. Such confusion!
I have a question please. Can this be diqgnose by ultrq sound and x-ray. My result is there is a partial thickness tear of the subscapularis tendon, supraspinatus tendinosis and proximal biceps tenosynovitis. What does this mean. Thank you if i get a reply from you. Yes, an ultrasound can diagnose the rotator cuff tears you mention. Partial thickness tears are pretty common, and most don't require surgery. Have you had any treatment yet? And if so, has it helped?

I believe I have tendinosis from overuse. I was doing well with PT then Reinflamed it golfing. Ever since anytime I use my shoulder like throwing a football or lifting weights it tightens up really badly. I’ve had several MRIs and there isn’t anything structural damaged according to the MRIs. This all started 9 months ago. Is it too late for PT to fix it? My doctor said he could do an exploratory arthroscopy but I’m afraid it won’t fix anything and will be unnecessary. I’m worried I’ll be stuck like this permanently :(
By the way the initial inflammation was from doing too many pushups.
Thank you Kindly for this video, I recently went to PT for my shoulder. They did a little ultrasound and had me do a LOT of exercises. By the next day the pain got a LOT worse, as did my range of motion. Luckily, I don't have a tear but My MRI showed
Mild degenerative change AC joint. Small effusion glenohumeral joint.
* Moderate edema subacromial subdeltoid bursa.
* Moderate tendinosis with undersurface fraying distal supraspinatus tendon similar to the prior study.
* Tendinosis without tear subscapularis tendon.
* Tendinosis distal infraspinatus tendon without tear.
* Reactive degenerative cysts in the humeral head at the infraspinatus tendon insertion again noted.
I'm not sure what to do next. I don't want to return to the same PT place. I called several times and they haven't returned my calls, nor have they sent the exercise list they promised after my first visit. I don't have confidence in them since they have not followed up with me. Any suggestions as to what direction to take. I appreciate you.

Also: find a better PT! You shouldn't feel worse the next day, which in my opinion means the exercises they gave you were too aggressive. And I'm guessing they didn't do any hands-on treatment. That's not good. Find a better one :-)
My mri doesn’t say supraspinatus, only “rotator cuff tendinosis”. Can you tell me why?
That's not very specific, and I'm not sure why. You could probably request that the company who wrote the report give you more details. There should be a "detailed report" of findings, as well as a summary.

Thank you for this explanation. The mri showed its supraspinatus tendonisis. I have no problem lufting the arm. I cant bend back right arm. Back of my fingers touch my hip. Thats it. Hows this gonna heal? I cant use thearm properly .
I am in Turkiye. I had 2 injections, one was hyauronic acid called MID SHOULDER . Other one was a different one. I cant remember . No help. The Doctors dont know anything here.

It sounds like you have what I described in this video, and I have several other videos on how to improve it. I hope those help! The best sleeping position varies from person to person. My favorite one for shoulder pain is to sleep on the non-painful shoulder with a large pillow(s) in front of the body to support the painful shoulder from dropping in front of the body.
Hi @Gordonphysicaltherapy, Thanks for the reply. Is this problem recoverable, Is this critical problem ? Currently I am using shoulder immobilizer for arm, along with taking some inflammation medication.
I have this with a partial 50% tear. My doctor is allowing me to try conservative treatment 1st, but it's not working. I am a personal trainer and already know how to strength train. I'm very muscular. It's more likely caused by overuse over time. I'm 49 years old, been lifting since 24 years old. I also teach group classes.
My PT was just giving me exercises I already do. No massage at all. Not really working on bringing blood flow to the area. I did have elbow tendenosis last year. (I know I'm a mess!!) The PT I had I liked better. She's just a bit out of my way. It is difficult to fit in my work day. She massaged my arm wrist to shoulder. My elbow did heal on its own! My Sports med doc told me the next steps would be surgery or the option to try PRP. I don't want to do either. I only get paid when I work. If I can avoid canceling clients, I will. Do you think maybe try to go back to my old PT. See if that works 1st before forcing myself to decide between prp or surgery? Does prp work?😮

My mri says minor attrition of infraspinatus. What it mean
I'm not familiar with that term, "minor attrition". I'm guessing it's similar to "mild tendinosis" but I'm not positive.
Been experiencing shoulder pain for 3-4 months. Just got MRI & results show: partial-thickness bursal-sided tear, supraspinatus tendinosis, subacromial-subdeltoid bursitis, & osteoarthrosis of the glenohumeral joint. I'm being referred to a sports medicine dr for injections & therapy. Would really appreciate hearing your thoughts & opinions regarding this specific combination of injuries.
Your combination of imaging findings is very common. I prefer people start with PT before having an injection, but that’s just me 😉. A good PT should be able to help you quite a bit. Good luck!
Can a car wreck cause this? My ac joint was sticking up after the wreck then it was hard holding up. My car wreck was 5 months ago and just did a mri yesterday. And i have this. I neve have hadva swore shoulder

Is it normal to also get burning pain the trap area from this?
Yes, you can get burning in the trap region, especially if your trap muscles are compensating for the weakness in the rotator cuff muscles.

Could this cause pain even in the under arm as well?
Pain under the arm wouldn't typically be due to the supraspinatus. Could be a subscapularis issue instead.

I think I have something like this but I don’t get pain on the outer shoulder. I get passion towards my forearm and in my hand at some point later in the evening.
If you're getting pain in your forearm and hand, it's probably not coming from your shoulder...
@@GordonPhysicalTherapy read somewhere that a rotator cuff injury can cause you to “pinch” your nerve that runs down your arm. That’s what it feels like….although, it’s mostly gone away in the last day or so.

Hello, I was recently diagnosed with severe supraspinatus tendonosis in both shoulders and mild infraspinatus tendonosis in my left shoulder. The doctor believes it's caused by arthritic changes in my shoulders. He also stated that at this point there is no treatment options. How common is it for arthritis to cause these issues? Also while I am not an athlete I do kayak recreationally and work in aviation sheet metal. Is it safe to continue these activities and profession? This is the second time a doctor has told me I had the body of a professional athlete. Unfortunately it's not in the good way . I just have the injuries lol.

Hi I was diagnosed withthickening of the subacromial bursa mild supraspinatus 4 years ago I had 2 injection but that didn't help I'm feeling lots of pain from my shoulder to my elbow and makes some cracking noise

How much time does it take to heal? And hot or cold fomentation? What should be done
Good question! Healing times vary quite a bit from person to person and depend on several factors, including things like: how damaged the tissue is, how long the pain has been an issue, what activities continue to aggravate the tendon on a regular basis, what types of treatments are being done.
I know that might be a bit confusing, but those factors are all important to consider. That's why I often recommend people work with a therapist if they aren't making progress on their own.
In regards to heat or ice, I tend to prefer heat, as it improves blood flow to the muscles. However, if the shoulder is really sore and inflamed, using ice can also be helpful. And it's okay to use both heat and ice, depending on how your shoulder is feeling.

@@GordonPhysicalTherapy hi. Thanks for the reply. I have been going for physiotherapy for a week. They are just giving Russian waves, short wave diathermy, usg. It's the same. Restriction of movement terminally is there due to pain. Don't know how long will it take to heal.

Also, are they doing any hands-on treatment to help reduce the pain and improve the pain-free motion?

It's really difficult to find a good physiotherapist. I am a infectious diseases resident. I was quite impressed by your video as tendinosis is a less well understood term and u had touched upon these areas.

Hey... So i was diagnosed with supra-spinatus tendinosis (right shoulder)2months ago. I used to gym 3days a week, i was a beginner.I used to feel the pain only when i used to do Db press(that too very minor pain, rest of the time it was all good(a little bit pain started because i did a intense shoulder workout did pushups pullups etc) . I decided to consult a doctor. I was a badminton player. Can i start playing badminton(after 4yrs)( i am right hand dominant). I don't feel the pain now(haven't tried bench press yet and never gonna) . I am undergoing regular physiotherapy for my shoulder. So will it worsen the situation or its okay to continue gym and badminton?( MY FORM WAS NOT CORRECT WHILE DOING DB PRESS THATS WHERE EVERYTHING WENT WRONG)
It sounds like you're in a good position to start getting back to the gym and to badminton, especially if you can ease your way back in and aren't experiencing pain. If you're still working with your PT, I'd get their opinion on how to ease back into these activities without making your pain return.
@@GordonPhysicalTherapyI read on google that it takes minimum 3-6 months for full recovery. Badminton as a sport is very hard on the shoulder. I fear that this temporary injury would get transformed into a permanent or a surgery. Its been just 2 months😐. Idk i am very confused. Anyway. THANk YOU VERY MUCH FOR REPLYING. @@GordonPhysicalTherapyHey, I asked my doctor he said i can start gym and badminton but i wasn't convinced i told him that i felt some pain while i was driving then he said dont play badminton for now.... After 1month you can start badminton for now you can work out. So the other day i did some bicep curls shrugs... I felt a tight pull in my shoulder area. I am really not convinced with my docs advice as he hardly evaluated me he saif raise you hand ohh ok you dont feel the pain its goood.

As long as you're not training through the pain, no, you're not necessarily doing anything dangerous. Just listen to your body.
