To answer Aidan's question, sometimes to run a different defense or offense the team will bring on entirely new personnel that have practiced that one play for crucial situations. Sometimes in the heat of a game the communication breaks down and players don't realize who all has come on the field for the play,. That's when you get too many players on the field. Five or six come on, four or five leave and suddenly one too many players.
There was another 5th down game in 1940. As in 1990, it was just a case of the refs losing track and everyone trusting the ref's count over their own. The 1940 game was Cornell vs Dartmouth, and Cornell entered the game as undefeated national title contenders while Dartmouth were winless. It was Cornell who benefitted from the error to the tune of a come from behind game winning touchdown. However, times were different in 1940, and when they finally saw film of the game and realized what had happened, the Cornell team unanimously voted to forfeit the game to Dartmouth. Class.
I was at the LSU-Tennessee game in '10, standing in the lower part of the student section just off-screen of the end zone in that clip. The penalty flag was the same color as the end zone paint, so I was screaming like mad to bring the flag to everyone's attention while the crowd was seething over the apparent loss. The Coach Les Miles weirdness was outdone by the Volunteers math that day.
Having too many (or in some cases, too few) players on the field is a coaching mistake but it’s not super uncommon, especially on defense. Defenses always make substitutions to try and counter the offense’s own substitutions, and in situations where time is very limited it can become pretty easy to get confused among all the subs.
I absolutely love the weird games/strange endings in non-power four college football games. May not have the most talented players, but those are the games that end up being the most entertaining just because of all the odd/unusual things you see in those games.

This would have resulted in Georgia Tech (#2 in the AP Poll) to have been the #1 team in the AP Poll, and the undisputed national champion in 1990.
In the Central Michigan v. Oklahoma State game the rules don’t allow for an untimed down, the officials were suspended for two games for their mistake but the game result was allowed to stand. Total BS.

The one rule in nearly all of these is that the game cannot end on a penalty.
The game can't end on a defensive penalty. It can end when the offense commits the foul.

I was at the Michigan versus Michigan state game. I looked like the kid with his hands on his head! Unbelievable.
As a Defense captain it was my job to count the players every play. Once we had 10 and I called timeout. Coach ripped me a new one for that. 😂😂
