The government website says I need to have my birth certificate as well as a photocopy of my birth certificate and that the copy has to be one sided, but show both sides of my birth certificate. I am confused cause I know they take my actual birth certificate and mail it back later
My wife has not changed her name on SSC, but her DL has my last name. Her birth certificate and SSC have her maiden name. Which name do I put on the application?

My husband's name doesn't fit in the spaces on the application. Much less the III (the 3rd) notation. It's on his previous passport, what do I do?

This is just happened to me I have to sent my actual birth certificate but where did you sent it? Is it possible to sent it through fedex?

Not for mail in. I think that's if you apply in person.
