If someone claims they never had failures, they havent done enough implants in their career. Implants can fail with improper planning or majorly wrong selection of cases. The failure rates trickle down to a large extent with experience. And also remember no medical treatment can claim 100% success and yes implants come to a close 98% 2. For highly atrophic maxilla, zygomatic implants are the implants of choice. Not BASAL. Read this consensus of more than 11 year data. Int J Implant Dent . 2023 Sep 12;9(1):28. doi: 10.1186/s40729-023-00489-9. ITI consensus report on zygomatic implants: indications, evaluation of surgical techniques and long-term treatment outcomes. For mandible , shorter implants have worked in a great fashion. There are numerous case reports which you can find in PubMed. 3. The comorbidity you are mentioning , Diabetes, Implants are contraindicated for uncontrollable diabetic patient. I may advise you that even basal implant surgery too is contraindicated. Bone grafting ranges from 3000 upwards to even upto 25000 based on the extent of grafting for a person. Titaniums are non magnetic, hence has no effect in MRI scans. All you need to be scared is about any loose metal crowns in your mouth if you are to take MRI, cos they may fly out. And current MRI machine images doesn't get distorted by implants or crowns. It’s practice in European countries ce approved and India cdsco approved I suggest you doctor don’t make unnecessary judgments on other implant practices on basal you like conventional so you do conventional . If some pus is draining, there must be some infection, which will warrant some deeper cleaning or surgery.
Basal implants are connected via cements and that is again a primary problem.

There is definitely a chance for infection if a broken tooth is left untreated in your mouth. Many a times, it may not give even pain until a bigger infection happens.
Consult a doctor, get an xray done and make sure the broken tooth is fixed.

Mini implants are typically suggested only for upper lateral incisor replacement and lower incisors replacements. However we do have conventional implants of up to 3.3 mm diameter which can be used in such scenarios.

Implants can be placed usually once the growth spurt of the kid is over. However in certain cases, it can be placed earlier, but the crown on the implant may require a change post 20 years.

Doctor, cortico basal implants are very successful, its completely technique sensitive. Please don't give wrong information because of having inadequate knowledge of the system. 1000s and 1000s of cases are being operated and successful.

The video was not against any practice or anybody practicing basal implants. The video explains why Dr Prem does not practice Basal implants.
Despite any advantages claimed by basal and one-piece implants, one-piece dental implants do have a disadvantage in that they are less flexible than the two-piece implant due to their single-unit construction This lack of flexibility prevents more detailed adjustments once placed.
Dr Prem reiterates that though the basal implant looks simple for a practitioner to place and give a prosthesis, it immediately does not supercede the disadvantages it has interms of prosthesis capabilities or a long term success. It makes it tough for patients to also have multiple issues once the implant is performed on them.
And why is that not FDA approved and why is it not practiced across the globe. Why isn't there enough literature in basal implants?
We practice evidence based implant dentistry and not a basal implant course given implant dentistry.
Does any of the BDS or MDS curriculum teach exhaustively about basal implants or even teach?
Kindly analyze merits and demerits before practicing on patients who trust us that we give them the best treatments. Also, Dr Prem has had multiple cases where basal implants have been brought by patients in their hands and got it replaced with endosteal implants.

It’s practice in European countries ce approved and India cdsco approved I suggest you doctor don’t make unnecessary judgments on other implant practices on basal you like conventional so you do conventional . You use RUclips video for better awareness about why required missing teeth need to replace by implants not why I don’t do basal this is not making any point but you are making wrong awareness rest is upto you dear dr Prem .
