      Quality of life is important too, so I totally understand this isn't easy, make the decision together with your family, the surgeon and possible second opinion. Your surgeon sounds very experienced though and he has the full picture which nono of us here on the internet have, even if we're you're friends and want the best for you. I'm sure sure will come to the best desicion for you.

    Hi,
    I commented on your video after seeing the surgeon last week, I am a nurse, my husband is a colorectal surgeon specialising in complex cancer surgery.
    Listening to the information today I know it is all very overwhelming for you, but I honestly think these results of negative PET scan/ MRI and biopsies is the best possible news given your diagnosis. It sounds like they are classing you as having no evidence of disease at present which is really amazing ( I would specifically ask if you are No evidence of disease).
    If this is the case I can totally understand the surgeon’s reluctance to perform exenteration surgery now, it really is such a huge operation that has risks related to both morbidity and mortality and would be being performed as a preventative measure against reoccurrence (but with no certainty that a reoccurrence wouldn’t occur elsewhere (metastasis)
    If you are NED then you do have a little bit of time, you can get the second opinions, ask all the questions and just have time to think about what is the right decision for you. There will be consequences either way that you will have to live with. You need to ask both the surgeon and oncologists about risks v benefits specifically for you and then decide which of the options you feel most at peace with.
    The difficult thing with medicine is that isn’t an exact science and every individuals body and their cancer can react differently, so no one can give you concrete guarantees , they can only give you probabilities based on the research available.
    You are in a really difficult position, but at the same time the fact that you can make a decision is so positive. Before this last set of scans you were under the impression that surgery was essential to your survival, that there was no choice. So my advice is gather all the information, ask all the questions and get a second opinion and based on that information make the decision that is right for you, even if that takes a couple of weeks you need to be sure that you are will be ok your decision whatever happens in the future.

      Yes!!!! Its so obvious that this is really hard for him too!! He deserves our support, not criticism. He is such a blessing in Tiffany's life!! He is walking this difficult path with her. His deep love for her shows in his actions by the many things he does for Tiffany and he is always there for her, even in the hardest of times.

You are both so perfect for each other, and I've only ever seen Matt being loving and supportive. As you say, he is going through this too.     Tiffany, the most important question you need your surgeon to answer is “Are you saying the huge tumor that you said did not respond to chemo or radiation is now gone?”
    What happened to it?!
    That surgery is no joke and completely life changing.
    Quality of life is absolutely an important factor, but only YOU can decide what defines quality for you; no one else can do that.
    Ultimately, you - and only you - has to live with the outcome of these important decisions.
    I absolutely wish you clarity and only the very best on your journey forward. ♥️

      I agree 100%! I don't understand why surgery is still an option if the big tumour isn't there any more, why would it be necessary? With only the limited information we have it doesn't make sense. I really hope Tiffany gets the answers she needs soon 💙💙

      @@debbiebarker1105 If you google Deferral of Surgery you will see. In UK specifically if someone has had a complete clinical response to neoadjuvant chemo therapy they offer deferral as an option. However surgery still has to be offered by the doctor as an option too.
      What does complete clinical response mean?
      Complete clinical response is defined as an absence of clinical, endoscopic or imaging evidence of a rectal tumor during the restaging of a patient with locally advanced rectal cancer after neoadjuvant chemo therapy.
      He would have never offered her deferral of surgery unless she was a candidate. To be a candidate she clearly had a complete clinch response to neoadjuvant chemo.

      @@ourcorrectopinions6824 hello, she did say much earlier that she is stage 3, the stage 4 diagnosis was a mistake. If it has progressed I think the surgeon would have said so. Whatever it is , she will have a clearer picture when she sees him next and hopefully share with us. Meanwhile, lets not assume the worst and voice that as it will only frighten and discourage Tiffany. She needs to remain positive and hopeful. Miracles do happen. This surgery is life changing, some patients regret how it affects their quality of life. Maybe he wants her to be sure this is acceptable to her.

    You and Matt were thinking out loud, processing, when you left the surgeons office after the two choices were given. If you weren’t including us in this very personal journey, we wouldn’t have even know the process you both are going through. We LOVE Matt! He is a one in a million great guy! Thank you for giving the mild, gentle, correction to those who need to be reminded to show love and kindness to all.

    We've seen how supportive Matt is, so I can't imagine that anyone could say anything acting as though they know what you two are truly going through. No need to defend Matt, we see how he feels about you. Those comments are not helpful and not acceptable. Your quality of life is a major consideration. I can't imagine trying to make that decision. Prayers for you, Matt and your family and hopefully God will guide you in the right direction.

    Tiffany, first of all, I just want to say that I am so sorry you are going through such a difficult diagnosis. Being in the medical field, I understand how life altering pelvic exenteration is. Colostomy, urostomy, loss of fertility and everything else that may or may not come with the operation is shocking to say the least. However, life with potential terminal cancer is also shocking. You, your family and the doctors are going to have to make the best decision for YOU. We as your tiffanythinks family are praying for your recovery. The only thing that I do not understand is how maintenance chemotherapy has gotten rid of your stage T4b cancer, especially after harder hitting drugs did nothing to change the mass? My advice is get a second opinion from another leading oncologist and colorectal surgeon. Also, do not waste time, as everyday counts when dealing with cancer. Stay strong, you got this!

      The surgeon said in the first meeting......when you will live you must do the surgery, the cancer spread without surgery in 6 month.....What I dont understand, why do surgery when there is no cancer? In the pet scan is now no cancer showing up? I think that is a misunderstanding. When there is no cancer the surgeon would say.....There is no cancer we didn't need the operation, it's all good.....After the chemo the docter said that the tumor did't shrink at all. And than she had the chemo because she must wait so long for the biopsie Op, this chemo was to prevent spreading. I didn't think the tumor is gone, I think the biopsie is without cancer cells. Sorry about my englisch

      I wholeheartedly agree. If the chemo didn't shrink the tumors, then how it is possible they are all gone now? I know Tiff will do what's right for her but I think a second opinion, if possible, is a great idea.

    Matt is an amazing human being, he chose to be supportive, compassionate and kind when you gave him a choice to walk away when you were diagnosed, he wanted to stay. Do people understand how absolutely remarkable a quality that is in a man these days? That’s a beautiful, spiritual and superb loving and profoundly human human being! How lucky Tiffany is to have found someone like Matt!

      Maybe ,,,a small suggestion,,,maybe u can take a white sheet of paper with the pro and con' s of surgery ,,,and the pro and con's of not having it,, the main question is, how U , only U want to live your life! The quality of your life , i mean,, there seem to be no certainties here,,no guarantees which makes this very hard for u to take any decision,,, put it straight to your surgeon: if i were your daughter, what would u recommend? What would u do????? Does this makes any sense for u? Think about it, and hold on pretty lady!!

    Tiffany, I have faced cancer twice, faced some of the same life changing issues you face. What I would have appreciated, and was never offered, was to talk to one or two patients who experienced exactly what I was facing. It might have been helpful to join a patient support group, both with patients who have had surgery, and others who didn’t. Try to find young patients, dear.
    A doctor and a nurse have their points of view. They may have difficulty discussing the emotional effects of cancer.
    They will sugar coat it, because they too fear cancer.
    They do not want to be the bearer of all the side effects and complications of surgery.
    If they did, that would make you too anxious to face surgery.
    While another patient will tell you the real hard facts, and prepare you for what’s coming.
    There are a lot of unknowns. No one can tell you the future with any certainty.
    Often, there is pressure to do something. The surgeon wants to do something. And the family wants something to be done.
    But doing nothing may be the best solution for quality of life.
    Isn’t quality of life the goal?
    Surviving with complications, constant anxiety and depression, and just not being happy is not living a quality life
    Just take things as they come.
    It is a good thing you are including all your family in your decisions. This is a personal journey, but also a familial one.
    Try to keep the positive attitude you always have. I know it’s hard.
    I am on a similar journey.
    One thing my journey has taught me is that here in the US, we need a lot better genetic counseling. I had a hard time telling my family and asking for help

      She should also speak with her Radiologist and Oncologist as they are usually brutally honest and do not sugar coat anything. They are part of her team not just the Surgeon.

    I felt so strongly that I had to stop the video a third of the way in to say something. No one who saw the worried look on Matt's face the morning of your hospital appointment could ever doubt how much he loves you. He was trying to be brave for you, but it was obvious just how much he was hurting too. I love the little exchanges between you. You are such a cute couple. He is doing exactly what a supportive partner should do. He is supporting your decision and will be there for you whatever you decide to do.       My dear child,
      Matt is worth his weight in gold and his devotion to you is obvious. Shut out the naysayers. There will always be nasty and disgruntled people. Pity them, and move on. BTW, being that Matt is such a good cook, has he ever been a contestant on the Great British Baking show? Maybe a few years back?? He looks familiar. He is beautiful, inside and out. An all around terrific guy. Chin up Tiffany, don't waste your time or energy on this issue. You've got bigger fish to fry.

    Matt seems so much like my husband. I’m going through breast cancer and my sweet husband is the stoic and quiet type. People don’t need to underestimate how hard it is on our loved ones to watch us go through cancer. Matt seems like your sweet, strong rock and I’m so glad you have him to lean on.     As a 10 year cancer patient, I've learned that you have got to do whatever is right for you! Listening and receiving opinions is great, but advocating for yourself and shutting out the negatives is almost as important as treatment. I've found, at the end of the day, people are more upset by not following their own instincts than they are about being "wrong." I understand how people can become very invested, but, ultimately, you and your family are the ones who have to live with your choices. Absolutely no one else knows what you need, how you define your quality of life, or what you are capable of achieving. Hang in there. Ignore the distractions (including negative comments) and live your life ... one day at a time. I wish you well.

    Tiffany, I am also under the care of the Marsden and have had to make difficult decisions with every part of my treatment which could potentially have long term life debilitating affects.
    In my experience, the surgeons at the Marsden have an incredible success rate compared to national figures, if your surgeon is hesitant to go ahead with surgery it is for a very good reason.
    As much as people online who are doctors or nurses or cancer sufferers or know someone in a similar situation, your team at the marsden know 100% of your cancer situation and they are who you should be guided by.
    They may not be a loving family member but I believe that makes them even better equipped to judge the situation as they remove the emotional aspect and deal with the facts.

    Please tell Matt, he is deeply respected by lots of people he’s never met, myself included. My spouse is my caregiver (chronic long term autoimmune) and I know just how hard that is. It’s often grueling and so difficult. You two will make the best decisions for you both. You shouldn’t have to give one seconds thought to anyone not supportive. I pray for you both. 💜🇺🇸🌵

      @@eunchaneunchan I have Neuroendocrine. I had a 3cm tumor removed from my small intestine and it has spread to the lymph nodes. I still have one area of cancer and it is being watched. I do get injections for symptom management (Sandostatin) and took a break for a few months but start back up in October.

    I think people forget this is matts journey too, taking away all of your pelvic interior means you’ll always need medical intervention one way or another, risks of infection, body image, loss of fertility, it’s not the future you or Matt imagined. Having the surgery, is still no guarantee it’ll never come back, not having the surgery doesn’t mean it’ll come back either! What’s important is life, but being alive is not the same as living. You do what your gut tells you. What ever the future holds live what ever life you have left with no regrets, never look back and go.. but what if. Because none of us have the gift of hindsight. Massive of loves to you all

    There is approximately a 41% false negative when diagnosing mucinous adenocarcinoma with a PET scan. The best way to check for that cancer is with a Diffusion Weighted (DW) MRI. Your surgeon/oncologist would be aware of this. My best to you and your family.

      It is not surprising that the PET scan did not light up, because the neoadjuvant therapy transformed the adenocarcinoma to mucin pools. Scanning is difficult. Prognosis is difficult. So many unknowns

    I've been where Matt's been and no one knows what it's like until they've been there.     Remember to ask the surgeon “ If it was his daughter or wife, would he have them do the surgery” that really is the ONLY question you should ask.

    I just don't understand two things. 1.) How the tumor which has invaded the uterus has disappeared and no longer shows on a scan. And 2.) If there is nothing there and you are healed from chemo and radiation, why is he offering you a choice of surgery or a deferred surgery? I think he needs to get on the phone and spend 30 minutes explaining himself. Much love Tiffany, you're going through so much!

    Matt is a GEM!!!!! He dropped everything and moved in with you to support you and Ama. He has been there every step of the way.     💙Hi Tiffany - (from USA) I’ve been following you for almost the whole year since your diagnosis. I also have cancer - stage 4 lymphoma and now aggressive breast cancer. It’s so much to process! People with negative comments just have NO idea unless they have been in your shoes!! I absolutely understand what you’re going through and I’m glad you and Matt are getting more info from your doctor. Whatever you decide to do will be right for YOU and it’s YOUR decision of course with Ama & Matt’s input too, because they ARE going thru this with you daily!)
    I am in a completely different situation (I’m much older and have other medical problems also) than you and have decided not to even do chemo nor further diagnostic testing, but to live my life each day the best I can with whatever time I have….it was MY decision and it’s what’s best for me, whether people understand or not. Just do what is best for YOU - your surgery would absolutely be life-changing! It’s a HUGE decision!
    Matt is such an AMAZING and supportive partner and you are blessed to have him. I don’t have a “Matt” in my life, so believe me, I know the value of a partner like him! …..please don’t even think about the negative comments! Bless you for standing up for him!! Having someone to go thru all this with you is HUGE and Matt is truly an angel! So loving and supportive!
    Just know that there are so many watching who have not commented before, but we watch, we laugh with you, cry with you, celebrate with you and worry with you. And some of us are on a cancer journey of our own and understand the difficult decisions on a completely different level.

    My opinion has changed from last video. immediately I was like Get the surgery straight away but now I realise its your decision and you alone who has to do what's right for you, what feels right for you.     Hi Tiffany, I would advise having regular private scans to stay on top of the cancer returning. This way you are ahead of the game as oppose to relying on NHS scans and their times between scans. Full of admiration for you, Matt, your Mum and family. Keep going ! We are all so proud of you!

    Tiffany, I’m so sorry that you and your family are going through this. Matt is amazing. I’ve been dealing with cancer for about 11 yrs now. I’ve had cancer 5 times and now it’s possibly in my lungs. I understand totally what you are going through. Just really wanted to say that, YES, we’ve got this!!     I think getting a second opinion is a great idea. It's all so overwhelming I think talking to another doctor and seeing what he thinks could help. That's obviously just my opinion. My heart breaks that you had some bad comments about Matt. I think most of us love your Matt. He's such a keeper and a true sweetheart. 💙💙

    Yes girl we’re not putting him down we’re just saying he’s not a doctor I have I had colon cancer a year ago and I know it’s been a year and I’m still dealing with that going to see my doctor every six months and is always going to be like that and I had my surgery and I’m much better thank God but you should think about having your surgery and not wait especially because it would spray everywhere in your body so don’t think that we’re putting your honey down. Just use some common sense for yourself to specially when you share your life.

    I wish you'd just said the PET scan was clear at the previous video, people were so confused and worried for you. Nothing was making sense to us on delaying surgery. A clear scan makes a HUGE difference. So happy for you!💙

    Hi Tiffany, I just wanted to say I’m so proud of you for doing these video during your treatment! I was diagnosed with cancer at the new year and finished my treatment in April and I don’t know how you have been able to still do these videos during your treatment, you are such a strong person!
    Tiffany, you and you alone will choose to make the right decision for you and it doesn’t matter what other people think or say, please just focus on yourself and not what other people say! Each person will choose to do what they think is right for them and you will too!
    At the end of my treatment all I kept thinking was if I needed more treatment I would say no or that I need a break cos I found it so hard and I knew that people around wouldn’t be able to understand but I was very lucky that I haven’t needed any more treatment.
    Please take some time away from other peoples opinions and make the right decision for you!

You must be inspiring so many people without even realizing it! There will always be rude, ignorant people but look how much support and admiration you have!     Hello Tiffany... I'm from Cape Town, South Africa. I want to comment about the negativity around Matt. I've experienced caring for my Mom and uncle during their cancer journey. People will never understand the emotions of the loved ones caring for the cancer patient, especially during their lowest moments. We share all those emotions with you and we cover our emotions because we have to be strong and positive all the time for the patients well being. Your Ammas love for Matt is so evident and that tells us what a wonderful person he must be. Keep strong, at the end its all in God's hands...continue praying for His guidance. Love and prayers 🌹🙏

    I’m glad that you have Matt and he’s so supportive! It’s very difficult to watch someone you love go through this. People shouldn’t comment on things they don’t know. I think it’s very good that you’re talking with the surgeon and getting the whole picture. Decisions are best made once completely educated. Just breathe Tiff, you got this!

    tiffany, please hear this: i went through this with my mom and my best friends husband passed from cancer two months ago. START THE GO FUND ME NOW!! you don’t know what twists and turns are ahead of you, and having it in place and growing will be so helpful and a comfort. and you can use the money to have some fun too! a fabulous mini break, a day at the spa for your mom!

    I just don’t understand how you can from a stage 3 t4b malignant tumour which hasn’t responded well to Chemo and radiation (didn’t shrink) to it spontaneously disappearing into thin air and you having no cancer……

      Even if the tumour wasn’t/isn’t now cancerous, you can’t just leave a tumour to grow through multiple pelvic organs and have it not be taken out….

    I'm currently having chemo for exactly the same cancer as you....T4 tumour. I was luckily (if u can say that) enough to have emergency surgery and have a extended right hemicolectomy, with good margins after removal. Im having chemo as it was in 3 out of 48 lymph nodes. I really can't advise you on what to do but whatever it is...I wish you all the luck in the world.

    Dearest Tiffany, one thing I'm SO confused about (so goodness knows how you must be); is if the scans and biopsies are showing negative for cancer, why is surgery still required? And if your surgeon IS still considering surgery, he must believe some cancer IS still there, so then why is he suggesting waiting 6 weeks? Surely that would risk it getting worse? I don't understand.

      Cancer can be easily missed if it is too small to be picked up on scans, or is elsewhere other than a site that was biopsied so the surgery would be a preventative measure incase of recurrence as far as I’m aware

      If Tiffany has no cancer anymore (except for potential lingering cancer cells), then why is a deeply life-altering surgery still an option?
      Also, why didn't the surgeon say: Tiffany, I have good news, the cancer responded really well to the chemo and radiation and is now gone as far as we can see, the scans are clean and so are the biopsies. This has become a whole different ball game now and you don't need surgery anymore. Now, that doesn't mean the cancer is literally completely gone and that you are cured, in medicine we don't call it cured or cancer-free because there could still be cancer cells in and around the area of where the cancer was, even if I took biopsies that don't show any, plus there could be cancer cells in other organs that we can't pick up in scans. That's why I suggest a couple of other rounds of chemo. I have good faith Tiffany especially considering the cancer is responding so well to chemo. I watch your videos, and it won't be long anymore before you will be able to do a boomerang.
      Why wasn't it a clear story, something like this, in the car and why weren't Tiffany and Matt accordingly happy and relieved? Why were some things unclear like cancer cells that would have changed into innocent micu?
      Why did the surgeon say: the disadvantage of waiting though is that "it" could spread/grow and become inoperable in 6 weeks.
      (Hence people responding: be careful, Matt shouldn't assume the cancer won't or can't dangerously spread in 6 weeks.) "It" referring to cancer, thus the surgeon saying the cancer is still there.
      More importantly, other people responded that Micunous Adenocarcinoma doesn't show up in scans. And to ask her doctor to confirm if this is her diagnosis? And that the micu itself *is* the cancer.
      Yet other people said the cancer cells are hard to detect in biopsy after chemo, and also wondered if this may be the reason the surgeon suggested to wait for 6 weeks without chemo to do another biopsy again. Again? Tiffany isn't a guinea pig. And then why do a biopsy after chemo the first time?
      So .... Does Tiffany still have cancer or not?

    I can’t imagine how hard it is to make a decision like this. Knowing that the pet scan was clear as well as the biopsy, I can understand why the surgeon would suggest waiting. He truly knows the quality of life the surgery leaves people with & I also believe that the last thing he would ever want suggest would be waiting & not doing the surgery if he thought the end result would not be well. Whatever you choose to do, we will support you because the decision is yours & you owe no one an explanation. You got this!! ❤️❤️😘

      @@janneal9156 I don't understand this too. This was the only surgeon who would agree to operate on Tiffany and take on her case because its a complicated one. From what I understand the tumor was quite big too can it really just be gone? After the chemo and radiation didn't do anything and the doctor told her it wouldn't so anything either it was just to give them time to come up with a plan? Non of this makes sense or maybe I'm the one who's not understanding it

      @@janneal9156 she has clean biopsies, clear pet scan and a clear colonoscopy. the treatment worked. her stage of cancer (locally advanced stage 3) has a good survival rate 71%.
      Also if you go back and watch the video of her results video in May with her mom in the car after her second round of chemo and radiation, she was told it did shrink 'some' and that she was told she was responding well to chemo, however the team of doctor's treating her met and discussed whether to continue with more chemo since she was responding to it, or to ship her over to this current surgery who does the very difficult full pelvic removal surgery, they decided to have her see the surgeon who does the complicated surgery at RM in London. He then put her on six more weeks of chemo then did biopsies and scans. They turned out to be clear.
      Maybe her if oncologist team opted for the additional chemo round they would have had the same results. Either way, it seems as if she's been offered Deferral Of Surgery as an option and even a recommendation by her surgeon. This option is only offered to those who have had clinical response to treatment. This is good news.

    I lost My Dad to Stage 4 metastatic Prostate Cancer in 2017 and I now help care for and support my mom who has stage 4 metastatic breast cancer .
