33:40 I interpret this line as signifying that Andrew did not regress, he sees through his own delusions, yet pretends to still be suffering from psychosis so that he will be lobotomized. In other words, he would rather lose his cognitive function rather than live with the grief and guilt of what he has done.
This movie has a lot of rewatchable value, it has so many hints hiddens since the beginning, for example how Andrew always needs Chuck to light his cigarette, because obviously mental patients cant carry any fire sources
11:45 Actually she was drinking the glass of water. But Andrew as an aversion to water, so by his point of view the scene show that there is something at that time with him and water, later confirmed by the lake and the trauma that put him into.

I think he realized the truth but also realized that he wouldn't be able to live with himself with what he did (and didn't) do. I think he pretended to regress, or just let his other personality take over because he knew they would lobotomize him and he would be "free" from it all.

Regarding Andrew's crime, you're right to question it landing him in the hospital instead of prison. We don't know how his psyche presented at the time the police arrived or for the duration of the trial, possibly he could have presented as insane, but certainly at the time he shot Dolores he was fully aware of what he was doing. It was a crime of passion rather than a product of an insane mind.
