  • As women approach their mid to late thirties, they often suffer symptoms that are ignored or dismissed by conventional medicine. Insomnia, weight gain, irritability, reduced memory, and brain fog are all symptoms that can drastically change the quality of life for women. Today’s guest is here to show us that there is hope and that it doesn’t have to be this way.
Today on The Dhru Purohit Podcast, Dhru sits down with Dr. Sara Gottfried for a deep dive into women’s health, specifically the changes that occur in perimenopause and menopause. Dr. Gottfried shares the symptoms women in these categories face and the science behind why they occur. She shows us the critical tests that need to be addressed, offers practical tips women can implement to address these changes and balance key hormones, and inspires women to empower themselves on their road to longevity.
Dr. Sara Gottfried is a board-certified physician who graduated from Harvard and MIT. She practices evidence-based integrative, precision, and Functional Medicine. She is a Clinical Assistant Professor in the Department of Integrative Medicine and Nutritional Sciences at Thomas Jefferson University and Director of Precision Medicine at the Marcus Institute of Integrative Health. Her three New York Times bestselling books include The Hormone Cure, The Hormone Reset Diet, and Younger. Her latest book is Women, Food, and Hormones.
In this episode, Dhru and Dr. Gottfried dive into:
-The metabolic crisis that women in their 40s go through and the hope that Dr. Gottfried offers on a different way to be and live
-How conventional medicine often dismisses the symptoms that women feel and makes them feel it's a normal part of aging
-The top misconceptions or misunderstandings about the interplay between hormones, including how long before menopause symptoms can start
-Women with high cortisol levels in their 40s had decreased brain volume and memory
-Common symptoms of perimenopause and menopause as they relate to aging: mood swings, depression, insomnia, and loss of libido
-The environmental and lifestyle factors that impact women’s hormones including stress, ultra-processed foods, and exposure to endocrine disrupters
-The top 3 hormones to balance and correct: cortisol, estrogen, and thyroid
-The importance of testosterone as the most abundant hormone in women’s bodies
-How Dr. Gottfried started her hormonal journey by testing her cortisol and how prioritizing meditation and yoga reduced her stress response
-Dealing with insomnia during perimenopause when it’s especially critical to prioritize sleep and how progesterone can help
-Testing thyroid levels at least once a year and the connection between thyroid and autoimmune disease
-Bringing hormones in harmony by increasing protein, reducing ultra-processed foods, increasing fiber, and quality fat
-The key to feeling better in menopause is measuring and addressing your glucose levels
-What drives belly fat and the relationship between belly fat and cortisol
-Micronutrients, including Vitamin D and copper, and how they impact thyroid levels
-The role of post-traumatic stress and the use of psychedelics
-Helping women over 40 and the importance of healing states of consciousness
-Dr. Gottfried’s message of how it doesn’t have to be this way

Women are also so time strapped these days, managing careers and family. And it's increasingly harder to make ends meet. Our kids are entering a broken world and we carry all the emotional strain of this. It's so hard to have time to make homemade dinners and on a budget or to have time to exercise or the money to join a gym / class.

Absolutely agree about being time strapped. I genuinely don’t think this modern fast paced life, working full time and running a house is suited to women. We need that softer, slow life so it’s like we’re constantly running uphill and getting nowhere.. a treadmill on incline and trying not to fall off.
In terms of homemade dinners, I do a lot of one pot meals with extra portions and freeze them. If it’s a meal that takes more than 15 mins to prep and cook, there’s no way I’m doing just one portion!
I utilise the exercise videos on RUclips for free. I refuse to pay a gym membership and spend even more precious time driving to and from the gym… sometimes my exercise is walking on the spot after a meal to lower the glucose spike or dancing around my bedroom to a few of my favourite songs to raise my heart rate. The best thing is, you don’t even need to look presentable! Sometimes I exercise in my pajamas that are going to be washed that day…again saving time and less laundry.
Hope this gives some inspiration 😊

I will never ever forget - I was 45. Starting menopause and bleeding like crazy. Had a D and C. Nope. Had to have a uterine ablation. Bleeding stopped. BTW, before that surgery I was given the drug Mifepristone with zero warning. OMG. I thought I was giving birth seriously. There was no follow up from dr after ablation. I was thrown into menopause. The symptoms. I asked and asked and asked - that entire OB GYN team - help me. I was drying out. Sweating and losing my sex drive. You know what my male doctor told me? "Get into bed naked. That's all you need to remember." HUH??????? No advice on biodenticals. Nothing. This was 17 years ago. I am now in my mid 60s. Scared to death. Aging rapidly because I was advised - no hormones. They cause cancer. What happens to my generation now? I know our grandmothers did not have hormones. But my grandparents also slept in separate rooms. I never saw affection. They looked old. Kinda feel like I was sentenced to a slow, painful death as I await osteoporosis, dementia, my husband leaving and more. Just wow ......

I knew what I needed to do but literally was not allowed (by society and family) to do what I knew in needed to do. We are expected to perform at work like men now thanks to the ‘feminist’ movement. Society does not give a perimenopausal or post menopausal woman a break. We have to act our way through decades. It’s exhausting. I believe this is a big factor in raised divorce rates. Women are absolutely exhausted and literally want a divorce to get some sleep, peace, and space to be how they actually are. To be authentically female. The whole of society need education on menopause, not just women.

FOOD is definitely a big part of the hormone issues in women (and men), people eat fast food every week, if not every day, even grabbing a muffin or donut at a coffee shop counts. Back in the day, eating out was a "treat" and you went out to eat as a family, never alone. Women started working, and raising children, and household chores, etc; whereas before women ran the household (very well I might add), because they didn't have the added stress and time that work takes up. The traditional "nuclear" way of life was WAY healthier for the entire family. Yes women went through menopause back then too, but they didn't have all of these stresses on top of it.
You would be lucky if you have a spouse who helps with raising the children and the household chores (cooking meals, laundry, cleaning, doctors appointments, soccer practice, pay bills, etc), it's hard to be 50/50 ALL the time, but if it's give and take on a regular basis, that can take a TON of stress off the woman or one of the spouses.

Unfortunately, this "brushing off" of women's health issues by medical professionals is SO prevelant. I was dismissed for months due to all of sudden heavy heavy bleeding non-stop and extreme pain, worse than child labor by a thousand times, went to Emergency several times, went to family doctor, went to walk-in doctors, all brushed me off saying I'm just getting older, it's just hormones changing. Family doc put me on birth control to control bleeding, it somewhat worked but not a hundred percent, I was still bleeding very heavy on the 7 days off birth control. All this while I was waiting to see a gyno, because the wait list was SO long at any gyno I called. Minimum 2 month wait. Emergency room doctors put me on other meds to reduce bleeding, which didn't help at all.
All they kept doing was ultrasounds and it kept showing fibroids/cysts, so the docs kept chalking it up to these things and nothing else.
Finally got in to see the gyno, and she immediately said she thinks it's cancer and I need to get biopsied immediately. I was at the oncologist a day later and diagnosed with cerival cancer. No one in the months prior bothered to check me internally, and if the wait list for gynos wasn't so long, I wouldn't been diagnosed earlier and started treatment sooner.
DO NOT ALLOW doctors to brush you off, I wish I had been more insistent. Although I didn't suspect cancer at all, I KNEW something was not right, and it's wasn't "just hormones". GET your pap tests regularly as early as age 30.
Thank you for this episode, I wish more doctors/healthcare professionals watch these types of videos/podcasts, but most of them think they know it all already and/or don't really care about patients, they will do bare minimum.

The root of all changes and problems is a lack of proper nutrition, perhaps this is the first problem to solve.
All commercial food goes after more profits first, and competition is fierce, and because of this, all food is advertised as better than the competition.
However, almost any can, box, or bag is not fresh food, and maybe that is an indication of what to eat or not.
Another good indicator of something odd is the constant increase in cloth sizes.

I found 50 to be incredible! Even took a trip through Europe. Could wear my adult daughter’s outfits and felt great. By sixty I’d had a few injuries and my work load was even heavier than before. Changed my diet because I was finding my body was reacting to foods that I’d had no problem with before. Some weight gain. Now, at 72 I am finding myself wondering if the stress levels associated with running our own business has added up. I work out almost everyday being sure to have recovery times and I watch my diet yet my weight has increased and have some weight distribution. I think I seriously need to get my hormones checked. I find it very difficult to find information for older women. Will this change over time? I hope so.

    • @TheTriciaLife

They say not to do cardio or workout everyday. That alone can cause stress in your body or cortisol levels to rise. Walking is ok, you can do cardio but maybe once a week. HIIT training is better for older women. Especially for aging. Maybe incorporate meditation/breathwork. A nature walk can be considered meditation.
You should get your hormone panel taken. The Doctor says she gets hers done 4 times a year or once a quarter. She said her cortisol was 3x what normal was when she was stressed out. That was her late 30’s.

I went full carnivore for a year to cure insulin resistance and pre-diabetes. Although I lost 15 pounds and slept better, I did my labs and to my surprise my levels got worse! A1C went up, Uric acid was high, ferritin high, cholesterol high, etc. I don’t think carnivore is for everyone. I’m a clean eater, haven’t had any simple carbs in years. NO processed food whatsoever, NO sugar ever yet my health on paper doesn’t look good. I wish I could find a diet that works for me. Went on a two month vacation, drank wine often and the 15 pounds came back. So weird. Adding, Veges, nuts, yogurt was all I did and now I’m back to base. This doesn’t seem right. It’s got to the hormones. I really wish more people would talk about this so us mid-life women don’t have to throw darts trying to figure out how to navigate this period of our lives. I was told by my Health coach not to IF but I still do it because that’s what’s natural to me. I found a new doctor last week and he says it’s my hormones so we are working on a plan to get me on HRT after my labs. He also advised not doing a meat based diet at this stage of life. Mind you, I’m not over weight- never have been, but insulin and A1C was on pre-diabetic levels so that’s what made me do carnivore to help reverse it. I just want to get back to my comfortable size which is 10 pounds less and reverse my levels to normal. I know Dr. Mindy, I’m very familiar with all the doctors in the Carnivore/Keto space and IF doctor like Dr. Fung. I’ve leaned diet is very individual and not one size fits all. I’m not restricting anymore. Eating more whole food fiber like Dr. Mary Claire- Haver promotes. Praying HRT will help with all my concerns and issues.

It happens with hormone fluctuations... if a young woman is getting on and off birth control or just has a problem with her hormones naturally.. it can happen.
Now this proves the importance the medical field should give to women and women's hormones.. specially as we get older... we live longer and there is no excuse we aren't treated with more respect. There is no reason we should be suffering from so much arthritis, osteoporosis, dryness, and over all quality of life. If men had been the ones that were getting mostly autoimmune diseases, degenerative arthritis, cancers .. we would have probably seen a lot more interest..

I completely cut off sugar, sweeteners (no diet sodas, nothing) and white flour off my diet. I mainly eat salad, vegetables, proteines like fish and poultry and protein powder. Good fat like olive oil, avocado, nuts and seeds. I still work out 4-5 times heavy lifting in the gym and I shed 5 kilos of pure fat in 2,5 months. It works but it takes a lot of dedication.
