you said the trench box needed to be 18 inches above the trench wall. this is only needed when the box drops down below the wall. then a slope can be created on the sides that would be 18 inches below, and the slope must correspond with the soil type. other than this, the box can actually be level with the top of the trench wall, at least by the OSHA standard.
Looks like worker entered a trench with more than 2’ of exposed earth below the system at the 6:06 minute mark . And the ladder wasn’t tied off as required by OSHA
