    Shoulder pain is common. The presence of pain around the shoulder does not always mean it is a shoulder problem. Pain can come from the structures inside the shoulder or from the structures outside of the shoulder.The pain can be referred from the neck problems ( cervical spine pathology) or from other distant sites. Elbow pain may be rarely transmitted to the shoulder.
    The shoulder is a common area for referred pain and examination of the shoulder should include an examination of there other areas. One of these areas that can cause pain referred to the shoulder is the neck, such as a herniated cervical disc or cervical spine pathology. The muscles and the nerves that arise from the neck area will pass through and around the shoulder on their way down the arm. The pain may radiate to the shoulder of the scapula. The pain is usually at the superomedial aspect of the scapula.
    This is an example of the most common area of referred pain from neck pathology and disc herniation. The pain originating from the neck region may overlap the shoulder. Usually, the pain in the neck is felt on top of the shoulder over the area of the trapezius muscle. Pain from the shoulder itself is usually felt at the top of the arm. If the shoulder moves freely without pain, then the neck is the source of the pain.
    Sometimes these conditions are confusing and difficult to differentiate (they usually overlap), especially when neck related shoulder pain causes weakness of the rotator cuff muscles, causes the shoulder to become stiff or causes shoulder impingement. Pain arising from the acromioclavicular joint can radiate into the neck itself.
    Sometimes MRI is the only accurate study that helps in differentiating between neck pain and shoulder pain. When cervical spine problems are suspected, the physician should examine the patient for findings of gait disturbance and long tract signs which will indicate that the patient may have cervical myelopathy.
    A positive Spurling's test will indicate neck pathology, especially nerve root irritation.
    A positive shoulder abduction test usually indicates neck pathology. The patient’s symptoms are relieved by shoulder abduction, by placing the hand over the head.
    Heart problems such as myocardial infarction or angina may cause pain radiating to the left shoulder.
    Irritation of the diaphragm may present itself as shoulder pain. The diaphragm may share the same C4-C5 nerve root irritation.
    The chest and upper abdomen should be examined carefully for possible pathology.
    A Pancoast tumor is another condition which causes referred pain in the shoulder region. A Pancoast tumor is a type of lung cancer that forms at the very top of the lung. Shoulder pain may be the first symptom of pancost tumor. The post tumor may invade the brachial plexus, the ribs, the vertebral bodies, the pleura and the chest wall, causing shoulder pain.
    Sometimes spine fractures may present was shoulder pain. The fracture may irritate the rhomboids muscles and transmit pain to the scapula.

    This was very helpful. It verified the x-ray and doctor's diagnosis of inflammation/nerve pain in my upper neck and right shoulder blade. I was prescribed 75 mg Diclofenac 2X per day for pain and to reduce inflammation. What also brought relief was sleeping on right side with my arm raised (as shown in this video). Thankfully the Diclofenac knocked out most of the pain and reduced inflammation after just 1-2 days. So I was able to stop taking it ! I did restart Flax Seed pill and Fish Oil pill that I had run out of 2-3 weeks before. Years ago I had found that after taking flax seed (or fish) pill for 2 weeks my 'tennis elbow' pain (inflammation) went away ENTIRELY. Hopefully, restarting the Omega Oil will help prevent a recurrence of the severe neck pain. And thank you Dr. Ebraheim for this very useful information.

    I will be watching this perfect production again and again. I will also make an appt. with my Dr. next week. I have lived with this radiating shoulder, neck, arm, pain, and lack of range of motion for two weeks hoping it would go away after one extensive overuse all-day strenuous activities of my arms and shoulders. Sometimes I wake up with the pain, other times not. I dread this complicated diagnosis and wish I could see you, Dr. Ebraheim. I so appreciate the rhythm in which you articulated the information. I am otherwise in great health...no health conditions. I do think my injury stems from my years performing massages in my day spa. I had weakness in this shoulder area then but never like now.

    I thought that I would add that cervical spinal segmental dysfunction (but short of actual herniation) is also an extremely common cause of referred pain into the shoulder and arm - via scleratomal pathways. I treat this many times each week in my chiropractic Applied Kinesiology practice. In no way do I wish to hijack this fine video.. but if anyone is curious, I did make a short video on this.     long arc reflex is exaggerated in case of CNS/Stroke and well emphasis here also by Myelopathy,
    and on the other hand diminished with nerve root compression/Radiculopathy

    I had end up in the hospital since January 1st of this year with a horrible pain. At 1st my arm was numb I tough i was having a heart attack since I have heart problems. Thank God wasn't the issue. But since then my arm had been weak my fingers roll in like if am making a fist n they can't give me a diagnosis. I do have heart issues .I was also having a horrible neck pain I was so stiff few weeks ago...now my wing and the inner part of the rotator I have all the symptoms in this video...GOD BLESS US ALL . I WISH U ALL TO GET BETTER BC I KNOW THE PAIN IS DEBILITATING 🥺 ITS A DIFERENT PAIN..ALSO BOTHERS ME WHEN MOVING I FEEL MY ARM ITS STOP WORKING LIL BY LIL N I NEED MY ARMS..     Very helpful. Ive just been diagnosed with arthritis , disc bulging and facet arthritis with radicular nerve . I have had symptoms for 5 years but finally they did an mri.

      @@divyanipatil2391 pain in neck, shoulder and down the arm. Pins and needles and stinging prickling sensations. Also weakness in arm and loss of range of movement. I have now had an acif operation about 10 weeks ago but not much improvement yet. In physio now.

    I have severe pain in my left shoulder and shoulder blade when I life my arm high and to the back. Also have tingling like pins and needles in my back in the spine areas that feels like it is more to the left side of the spine. My doctor had been checking me for problems with the pancreas, possibly cancer but so far has found nothing. I have abdominal problems and that is why I was checked by my doctor to begin with. They still have not subsided. She is at a loss and so am I to figure out what is going on. Thanks for explaining all about referred pain because it seems like that is what is happening in my left shoulder.

      I have armpit pain for nearly two years it could be just a muscle strain as there is a big muscle in that area it could be costacondritis or just coming from your shoulder see your gp get referred to a physiotherapist and in the meantime put a hot water bottle on it but be careful and wrap a cloth around it good luck

    I've been so much pain on my left shoulder. The pain, that I feel is alot of pressure on my entire arm. and it Starts on my shoulder n neck area. it also goes to my chest and jaw. the pain was so bad yesterday, that I went to the E.R n thought I was having a heart attack or something. But thankfully I wasn't. They did a bunch of test n also a nuclear x-ray, to see if I might possibly have a blood clot ,somewhere in my chest. But no blood clots , thankfully. So the diagnosis was, that I have , "Paresthesias" on my shoulder n neck. something to do with my nerves n maybe a dislocated disk.. I have to now go see another doctor n also a neurologist 😩 oh God, i just hope they this pain to go away 😮‍💨 it's been 3 months that I started n have been having this pain n it's just getting worst 😭

      Girl tell me about neck and shoulder pain. Worst pain I have ever been in, including two natural births.
      Once I picked up a concrete planter in the yard, moved it a few feet. The next day, I woke with what felt like a simple crick in my neck. It worsened as the day progressed. By 6 PM i could not lift or turn my head, only held it forward.
      I had tears streaming. My husband took me to ER.
      MRI showed a bulged disk in my C-spine. Not sure which one. Now it comes and goes.i feel it is directly related to my scapular dyskinesia or “winged scapula” bc they always seem to aggrieve one another. I feel like cold water dripping on my dorsal scapula.

      @@alabamajenny8751 omg something very similar happened to me!! A few years ago, I was working on thanksgiving and was trying to hurry and finish my workload so I could go home and have dinner with my family. I was stocking big, heavy boxes of juice and being careless with the way I was lifting them. At one point I felt a sort of “snap” in my lower back and didn’t think too much of it. The next few days the pain started getting worse and worse… The next couple years my lower back was almost constantly in excruciating pain, no matter what I did, or meds I took. So an MRI revealed that I had damaged my L-4 and L-5 vertebrae, and have a bulging disc, as well as arthritis and mild scoliosis. I’m super limited in the kind of work I can do now, and have found relief in a mixture of pain meds, frozen water bottles, and heating pads. But I wish I could go back to that day and tell myself to stop trying to rush and focus on proper lifting😖
      I hope everyone here has some relief to their pain!! Oh lord help us all lmao

    Very informative video! I have gone to many Dr’s that never gave me a proper diagnosis, I have a lot of pain between shoulder blades, pain never goes away. Don’t know what to do with this problem any more.

      @@heartmomma6460 have you found a diagnosis of your pain? Mine started in 2018 and the past 6 months it's getting much worse, so far my orthopedist can't diagnose and told me to go to primary care and get some cardiac tests. My cervical spine MRI, EMG and nerve conduction test on left shoulder are all normal. I don't know what else to do but to live with the pain every day

      @@artbyagema actually yes. I ended up in the ER with a Gastrointestinal Perferation that was leaking causing my intense pain ironically. The septic toxins had almost reached my heart I put it off so long. I was life lighted the Fri before mothers day last yr & was told a half hour longer the acid would have basically eaten my heart. This happened to my 57 yr old cousin in June 2019 & he didn't survive. It's so hard for me to give advice because I have so many other health issues. Drs passed off the buck contributing it at 1st to my Lupus, severe Osteoporosis, 4 failed back surgeries, Fibromyalgia, Anxiety & Panic attacks, being severely underweight (medically induced anorexia) which causes pain everywhere from having weak muscles & no body fat as protection/padding as well. I just always feel so cruddy that I don't even like going to all the numerous Drs who specialize in ea individual body part or ailment. It's so taxing when a person battles issue after issue then not get the proper attention, treatment & even diagnosis. I also live 1.4 hrs one way from subpar Drs at best in rural Iowa. I tend to put off going since it has become kind of hard to make the 3 hr total at commute. I don't sleep well due to pain, have Narcolepsy, severely anemic/malnourished from being underweight. All combined makes me a road hazard, hence the Dr visits have become less and less. For me I say if it's not 1 thing it's something else for sure. I'm ready for a break & be my old overactive nonstop self again. Please don't be like me if possible, push your Drs for answers & don't put off going. Big Hugs, keep me updated on yourself.     Doctor, u explain it soo well. I have right side cervical radiculopathy. I can't go for surgery . Regular itching , tingling in right thumb, index finger. My rt hand goes totally numb. Any treatment ??.pls help
