It's one factor that might modify the patients symptoms as it has an influence on shoulder mobility. Just try to sit in a slouched posture and anteflex your shoulder... Research is controversial if a kyphotic posture contributes to shoulder pain. Yamamoto et al. (2015) say yes: www.ncbi.nlm.nih.gov/pubmed/25441565, Barret et al. The humerus is attached to the scapula which moves upon the thoracic spine. Therefore, any malpositioning of the ribs or thoracic spine can cause alterations in the mechanics of the shoulder girdle as whole.
What is your take on passive hanging for stretching the CAL and therefore increasing the subacromial space over time? I have had anterior shoulder pain (provoked by shoulder flexion) for 4 months now. I tried passive hanging yesterday with partial body weight but had pain while bringing my arms down. Any advice?
A ligament cannot be stretched really. The only thing that we'd expect is that you have some amount of creep after prolonged stretch on a ligament and then we're talking about the toe phase of 1-2% of lengthening. Don't think this is going to make meaningful difference.
hmmmm... have you read about Dr. John Kirsch's study on the CAL during passive hangs? I even bought his book. The testimonials on passive hangs and their positive effect on shoulder impingment is overwhelming. I am still skeptical and am therefore searching alot about this. I am still scared to encorporate passive hanging during my healing phase. However, what I can certainly say is that eccentric loading, which I started 12 days ago, helped me ALOT. I have no problem now holding and slowly lowering 1.5Kg in flexion. My LHB seems to have adapted well to that. BTW, can you show other eccentrics or isometrics I can do for my pathological LHB, for variation's sake? ;)
No, but first of all the mechanistic reasoning behind a new concept has to be logical - which isn't according to what we wrote earlier. On top of that, there is no evidence for passive shoulder hangs in the scientific literature, not even case studies. LHB tendinopathy never comes in isolation, but together with rotator cuff tendinopathy. We will soon publish videos on the rehab for that.
Sir my brother operated acl tear few years before but still he have pivot shift positive and plc problem also occurred in this condition shal we go for surgery? Or it can be recovered by physiotherapy

God I wish these people were available in the United States. I have dealt with scapular issues for years, and they ran multiple nerve tests and said the nerves were fine, but I have been to over 10 physical therapists, they don't run any of these tests they go straight to strengthening and charging, and the end result is always useless.
