What are Skews and Index Offsets in PAUT?

SKEW90 & SKEW270 on Screen is right, but on the probe weld piece is wrong, the SKEW90 on your left, and SKEW270 on your right. -Index value is on SKEW90, + Index Value is on SKEW270. the SKEW Define on “Introduction_to_Phased_Array_Ultrasonic_Technology_Applications 4th Printing 2017”，p169.

My friend it's better to use Omni PC setup and perform a scan plan firstly making a group for covering the root and another for cap and that's better for adjust the index offset and then save your focal low in a file

Ideally yes. But sometimes the scan plans don't work on the field because there are some physical restrictions. In that case a technician has to use his judgement and come up with the offset.

Creating a scan plan, using an appropriate software such as ES Beam Tools-Olympus Setup Builder, follow the procedure .. build the setup, calibrate, trace TCG curves .. ever heard of those? Not even cowboys do it like that.

Absolutely right!!! Skew 0 and 180 for transverse defects!
