On an EFI system, Windows must create a few extra minor partitions in order to be able to install properly. The EFI partition is an example, although that specific one already exists because of OS X. So, what I would have done is, I would have booted in EFI mode, then I would have selected that ~80GB partition created in OS X and deleted it. Afterwards, I would click the "Create new partition" and let Windows do it's thing. It would automatically create the partitions it needs and finally allow you to proceed with the installation process. The "delete" and "new" options for managing the partitions are all visible within the window you can see at 10mins00secs of this video.

Next, let the computer reboot on its own. Do everything you did and then when you get to the disk select, select the last disk and format it. Then, you should be able to proceed.
does that actually work now? when i did it in 2012 if you let it reboot with default settings it just loaded osx again. or.. loaded the cd/dvd part again and again instead of properly booting off the partition at the appropriate time? it's been like 5 years. all i remember is i did have to interrupt the process by holding alt at some or all of the points.

I think you could start up the restore process without installing Windows, since the system image contains a entire copy of the Windows and EFI partitions. So, you could had just booted up the Windows Setup disc, navigate to the System Image Backup wizard, and restored it from setup, NO WINDOWS INSTALLATION REQUIRED!!!

If you look at this whole 3-part arc, you will see that actually didn't work. It was a completely separate problem. But thanks for the suggestion.

The formatting failure might be due to the fact that the Mac OS uses Core Storage: en.wikipedia.org/wiki/Core_Storage

You could of used Hiren's Boot CD so you can use Mini XP or GParted to partition the drive.

The real way to fix that is to boot the windows cd from efi boot and restore using its boot option
About the issue you mentioned at the end, are you running High Sierra? Try upgrading to Sierra.

If MacOS is in GPT that means every partition you create needs to be in GPT hence you need to install Windows 10 BootCamp in EFI MODE !
