Pre-hab is HUGE! Makes so much difference in your outcome that it is foolish NOT to do it religiously! 7 weeks post-op and doing fantastic. Quit smoking day of surgery and doing well with that as well. Finished formal PT two weeks ago so working out on my down at our community center 4-5 days/week. Best words in ALL of your videos are the ones I believe Dr. Weening said “After I close the incisions I am just a cheerleader. The rest is up to you!” This made a huge impression on me and motivated me to take my recovery in my own hands and make it successful! Keep up the great posts.

Regarding the crack about socialized medicine in CA: the private fee for service medical system in the US pushes you out of the hospital as soon as you can walk: same day surgery and discharge. Overnight? NFW. Discharge instructions and signoff are always obtained before anesthesia is completely worn off.

Thank you greatly for these videos! I have been dealing with knee pain from osteoarthritis for more than 5 years with physio, injections, bracing etc. I have now gone through the assessment and am awaiting an appointment with a surgeon for a total knee replacement. I won’t be having the actual surgery for at least six months and am looking for suggestions as to pain and sleep management before surgery. I have been unable to sleep more than 2 hours for 3 months now due to knee pain at night. I have been to my GP and have tried all medications they have suggested with no relief. The physio that I am working with to keep my knee mobile has suggested that perhaps I should get and wear a brace that would keep muscles from tensing and pulling the grinding bones together and causing pain at night. Would you suggest something like this? Any other ideas? Through the day I find that moving around and changing positions helps manage the pain. I have not had ant significant relief from Tylenol, acetaminophen or naproxen and get temporary relief from CBD and or. Ice/heat. I’m really trying to keep some level of activity ( missing the 10 +hours of tennis I was playing until mid-July!) but with ongoing interrupted sleep I am finding that increasingly difficult. I would g really appreciate any ideas/suggestions 😀. Thanks!
You cannot believe how happy I am to see good videos done by CANADIANS!!!!! We rock. I had both knees done overseas because I had good insurance and didn't have to wait a year or more. Turns out that the left one has 2 different sized parts as it klunks at every step and the swelling hasn't gone down in 6 years. Going to have it refit or something like that in Nov. and hope it works this time. I didn't have anything like this where I was and got no information from the surgeons (mostly because they didn't speak English) but a little from their secretaries. This information is fantastic - I've watched half a dozen so far and have started the prephysio. My biggest question is when my previous doctors indicated that I would be able to kneel after is not true. I cannot kneel on the bed to pull the covers up. Is this common? Also the first one (in the UAE) said it was titanium so no problems with going through the scanner at the airport. He lied. All I had to do was show the scars and the agent let me through but I was disappointed as it was a time when I was travelling a lot. So, what's with the not being able to kneel? Again, thanks for being Canadian.
Hi Vivian. So we tell patients they are allowed to kneel but may not be able to due to pain. A combination of the scar and the lining around your knee and your ability to bend. Hope that helps! Thanks for watching and subscribing

Your info has been most helpful. I’m scheduled for knee replacement in another 4 weeks and I started doing the prehab exercises religiously. My questions concern the bandages and stitches care after surgery. What are the bandages. Do they need to be changed. That info would be helpful for me. Thanks so much. Pam
Hi Pam. Really depends what they use. We use a see through adhesive plastic type dressing that only needs to be changed if it is soiled. They will give you that info. Good luck. Thanks for watching and subscribing
What about pre and post TKR exercises. Pre TKR exercises should help educate the patients with proper exercises to do after the surgery as well as strengthening key muscles groups prior to surgery. I appreciated the over all broad brush approach of what to expect and to do. However, lots of surgeons fail to mention the role of physical therapy in maximizing the benefit to this procedure and gaining maximal range of motion.

The one downside to having good pain control over these years, for me, is the mistake of not having used those low-pain days to strengthen. Anything I can do now?

Hi there, I just watched your video about knee replacement which I really appreciate. I have severe arthritis on both knees and both are never fully straight. Only one knee is bothering me causing me to limp very badly. My surgeon in New York reccomends replacing both knees at the same time mainly because if I did only one, I would have one straight leg and one bent one which would put more pressure on the straight one while walking causing a slower rate of healing. Also having the benefits of going through the whole process only once. At the same time I am very concerned about not having one good leg to stand on and wonder how I will manage. Most grateful to hear what you have to say!

What do you 2 think about getting both knees done at 1 time? I was told both of them need to be replaced? Also, what if the person refuses to stop smoking and for how long?
So some surgeons will do both. We think it is a lot of pain and a lot of surgery so don’t typically recommend it. As for smoking we encourage people to stop but don’t refuse surgery. Ideally 3 weeks or there is no benefit
Had full knee replacement in January…procedure went well….youre videos helped me to see what to expect. I have, however, noticed a “clunking” sound in my new knee…..Is this normal or something to be concerned about.

Why is the additional weight loss required? I have lymphedema in my right leg and weight 300+. I have lost over 100#'s and have kept it off for over 4 years. Dr still wants me down to 225 before tnr. Arthritis is bone on bone and losing weight is getting more difficult!!
Hi Maryann. The main reason your surgeon is requesting further weight loss is likely related to the increased risk of complications and greater degree of difficulty of the operation if a patient has an elevated BMI. Hope that helps.
Having the procedure done on June 4th, 2020. Had 3 previous scopes to repair torn meniscus’s due to martial arts for over 30 years. Previous surgeries include a TRSR (Total Reverse Shoulder Replacement) back in 2012, Left elbow ORIF, spinal fusion of L4-L5, partial right hip replacement in 2018. I agree that the sooner I return home produces a better outcome. We agreed, surgeon and myself, that an overstay for 1 night at the hospital, but if I feel like it, I’d rather go home the same day. I currently live in a mobile home so a walker would be a challenge, I’d rather try to get away with just a cane. I’ll have a CPM (Continuous Passive Motion) device along with an Aircast Motorized cooling machine. Pain management is 10mg of Percocet every 4 hours with the addition of Tylenol & Advil as adjunct therapy.
What is your opinion of Total Knee Replacement surgery being done as an Out Patient Procedure? In this case the patient does not remain in the hospital for any length of time but goes home within a few hours of the surgery completion. I am 72 and in good health.
Hi maginot2u. Outpatient surgery is a very appealing option however we do not have the infrastructure in place here in Canada. The majority of our patients stay one night to control pain, mobilize with physio in the morning and make sure they can pee on their own (as urinary retention is a common problem after spinal anesthetic). In general length of stay has drastically decreased over the 15 years of our practice however we are just not quite there for outpatient surgery. It is being done successfully at many other centers elsewhere. Good luck and thanks for watching.
You guy's are great. TKR here in AZ , even on Medicare, is very expensive with the co-payments. I have to play quite a few gigs to save up for it and I have to sit while I'm playing (I'm a bass player so we're suppose to be transparent (unless you're Getty Lee)).

I was wondering I am going to be having a Knee revision when I have surgery. I am having a revision including both Tibia and Fibula, and I have found some videos showing surgery of knee revision but only with the Tibia involved, how do you revise the Fibula? Hello, I’m 65 years old and in 1980 I had a Jones reconstruction procedure on my right knee. I was able to do anything I wanted competitive sports, recreational ports. 2010 bone on bone pain began to make walking a pain. No more injections I am having a knee replacement in 27 days. My question is will my Jones reconstruction complicate my TKR?

Very helpful video as usual. Thanks. Would it be helpful or important for me to buy and use a icing wrap before my surgery which isnt until 12-7-22? I also have a recumbent bike i could use every day before surgery. My knee is not currently swollen and i have been on an rigorous workout routine involving treadmill use and leg machines for the last seven years. I tore my right ACL and meniscus back in 2014 and didnt want surgery so i have been able to exercise with it since then until recently when the knee arthritis got to the bone on bone stage. I tend to overdo exercise so i dont want to do anything that will disrupt my recovery after surgery. I am a little discouraged by the feeling i get from your videos which suggests I may not be able to return to my pre op exercise routine for a looooooooooong time after surgery, if ever. I am 82 but in good shape for my age and need to exercise due to heart disease. Any thoughts you have on this would really be appreciated. Thanks again.

Thanks for another thorough video. *Regarding swelling*: I had very successful hip replacement surgery this summer, but I had trouble with swelling and also with finding a comfortable position to sleep for the first week. The home care nurse suggested sleeping in a recliner, which I did not happen to own. When I looked for one, I found an *"anti-gravity" style chair*, which is designed to recline past horizontal so that your feet can be positioned totally above your heart, with your body bent forward at the waist and your knees slightly bent and supported from beneath. The feeling of support from head to toe is incredible and allowed me to sleep like a baby, and the elevation of my feet maximized the anti-swelling effects of icing. (Apparently, this style of chair was inspired and invented after observation of the natural shape the bodies of astronauts take when floating in weightless conditions.) I anticipate total knee replacement this December and expect to make good use of this chair again.
Hi Michael. Thanks for watching. Sleeping on your back can be a big challenge for people after hip replacement surgery but it does get better. And the swelling improves, slowly, but it does. Usually we tell people 6-12 months. Good luck with the total knee.
I've never had an iota of pain in the joint itself since the hip procedure, but my leg muscles had atrophied so much in the three years I allowed pain to limit and ultimately halt normal exercise that it has taken time to build their strength back up. At ten weeks post-op, I can report I am pain-free and able to do just about anything I want, including riding my bike, which is proving to be very beneficial. If I had it to do again, I would not have waited so long after recommendation that replacement was necessary to have it done.
Hi Michael. I think that is a very sensible approach. Getting moving as soon as you can is a key factor when recovering from a total knee replacement. But everyone has to do it at their own pace. Continued good luck to you as you get back to normal.
Hi Patricia. All centers and patients are difference but typically 2-3 days in the hospital. Some centers keep you in the hospital for 1 day and then transfer you to a rehab facility.
Very helpful. I'm having TKR Revision on the 30th. Next yr TKR on other knee and last yr 3 failed back surgeries. Super concerned how I'm gonna get thru pt when I can't do much with other knee and with back, but gonna push thru somehow.
Sounds like you have had a pretty tough course. You are right - determination is the main thing. Doing whatever you can preoperatively to get strong and prepared. Good luck.
I know this video was posted 6 years ago, and I have posed a couple questions on 2 of your other videos about TKR (thanks again for your replies), but have either of you ever performed TKR on a patient with transverse myelitis? Or do you know someone who has? My orthopedic surgeon who is performing my TKR in Oct. has not. I would like to get a better idea of what to expect post-surgery considering my unique and rare condition. For example, will the implant itself heat up or get colder with weather changes? Since I am already temperature-compromised I need to know if the implant will exacerbate that sensitivity.
Your videos have been very helpful. I've been dealing with arthritis in my right knee for a year, went in last Dec to have a tear fixed because of the pain and the dr said the tear was not causing the pain it was that I had grade 3 chondromalacia femoral condyle of the medial joint so been through cortisone shots 2 times and synvics and medication and nothing is working seeing my orthopedics doc in a few weeks to discuss surgery, is it normal for someone 51 to have to have a total knee replacement so young? I know the pain is horrible and getting worse.
Hi Lisa. So typically a knee replacement would not be the treatment of choice for grade 3 chondromalacia. Usually it is done for grade 4 - or bare bone. Knee pain is a difficult problem and it is important to accurately diagnose where the pain is coming from and then to exhaust all non knee replacement options before proceeding - especially at a young age. 51 is young but there is no perfect age. We recommend that people should wait as long as they possibly can to have a knee replacement because it is a good but not perfect operation and ultimately was designed for your parents and grandparents not for you. Younger people have the highest chance of not being totally satisfied due to their expectations. Have a long discussion with your surgeon. Good luck
This is a question for one of your shows and I need your thoughts I have a bad knee and hip on the same leg both with arthritis right now I am doing cortisone shots for both how would I go about having surgery on either one with pain in the other probably stupid question but I worry if it comes to that
Just wondering...have you ever done a TKR on a patient who had a stroke on one side, but a perfect knee there (can walk, foot drop with AFO, weakness , etc) but the others leg has the lateral aspect of the femur sitting on the medial of the tibia? Just wondering if there was any chance of a good outcome since the stroke side will never be optimal as a helper. I'm supposed to get this fixed although I have no pain in that knee, but my surgeon has never done it in this situation so I'm his guinea pig. If I can't rehab it, I'll never walk again, I suppose. Pretty scary.

It’s C What you need to do is to prepare people by telling them that they will basically be losing 6 months to a year of their lives. Minimizing the recovery is just a complete disservice to the patient and keeps them making an informed decision.

Hey Docs I am set up for surgery of the new revision with a longer bar in my knee on September 22. I went to rehab and I went to Pre-Op yesterday. Today the called and said my antibodies are hi and need to be checked to see if I will need a transfusion?.. what can you tell me about this? Thanks Docs keep well and thank for information
Hi Darlene. It may be that when they cross matched you for a possible blood transfusion they found antibodies so it means they have to find a more specific blood match. I have Complex Regional Syndrome.. but am bone on bone and in alot of pain because it. With my OA. It was advised by my Orthopedic doctor to have a total knee replacement, however he stated because of my CRPS he stated it's up to the orthopedic Surgeon he is referring me too.. What is your take on this.??? I feel I don't have any other options I want to proceed.
Hi Pamela. Ultimately the surgeon needs to agree to do it but yes if you have advanced arthritis and have exhausted non operative measures it sounds reasonable. I'm just starting my research on knee replacement as it's coming up for me in the near future. I know there is going to be pain after the surgery and I can deal with that after living with arthritis pain too long already, but after the pain from surgery passes and pain from muscles heals, what is causing pain after that? Sorry if that sounds like a really ignorant question, but if the knee replacement parts are not actually part of the body, (nerves and such) what is causing the pain? Thanks for your time.
Hi Juanita. Very good question to which there is not a clear answer - if you can figure that out you would solve a problem for a lot of patients and orthopaedic surgeons. The pain is likely coming from the soft tissues - the lining of the knee, the muscles, the tendons, etc. Some also think that when someone has pain for so long that the joint becomes wired for pain and that some of that is irreversible. Hi bluesmanreturns. After total knee replacement there can often be sounds or clunking throughout range of motion. This can be related to the fact that you have an artificial knee made out of metal and plastic and that your patella (whether it is your own or has been resurfaced) clunks over the femoral component. If the clunk was related to soft tissue getting stuck in the notch arthroscopic treatment has the potential to reduce the clunk. The main indication to do this procedure would be if the clunk was significantly painful. Hope that helps.
The Main thing My Doctor did not tell me my implant would be hitting my patella with every step making it painful and swelling around my knee cap area , its also really annoying and loud If I new this before hand , I would not of had the operation.. And now my Doctor isn’t responding to my pleas, Any solution for this ?

Hi Leonard. It sounds like it would be beneficial for you to be assessed by a physician or other health care professional. It is important that you ensure that whatever damage to your skin that has happened doesn't get infected. Hi Docs I was assessed for total knee replacement on both knees three years ago I couldn't have it done then my right knee is grinding and I can't straighten it out so my foot turns out I am making a new appointment to have it assessed . It's preventing me from walking very far at all
Hi Charlotte. Often as arthritis progresses you will lose the ability to full extend - this is called a flexion contracture. If you have been suffering for a long period of time it is a great idea to get assessed and to discuss all of your options up to and including total knee replacement. Hope that helps

Talking With Docs , my doctor tells me that I need a TKR but I’m procrastinating. Someone in my exercise class was told that stem cell treatment was not going to help her. She found another doctor who gave it to her and now she swears by it. I’m just trying to be informed and exhaust all my options before going through with the surgery. Thanks for getting back to me.

Before deciding on a knee replacement surgery understand one thing: Surgeons do surgeries. Period. These guys are older in the tooth and seem to want to stay with old-fashioned painful traditional knee replacements when modern evidence DOES show that regenerative therapies like PRP actually work. I work out in a fitness class with a lady who worked as a registered nurse before retiring. Her husband is a physician. Before PRP she could hardly do a lunge move without wincing in pain She is now totally pain free and doing every move in the intermediate HIIT class. She had PRP therapy...not sure if they used fetal blood or her own blood. Downside is that in US Medicare wont pay for it because it's considered experimental. It can be quite expensive out-of-pocket. Due your own due diligence and don't listen to just one surgeon before deciding on knee replacement. Younger more open-minded docs will be more receptive to surgery alternatives like PRP therapy. Don't just listen to one doc who says surgery is the only answer.
Hi Joyce. There are many different approaches to the hip some of which involve cutting or releasing the insertion of some of the muscle and reattaching it at the end of the procedure. There are pros and cons to all approaches and recently there has been some enthusiasm for the anterior approach particularly because it gets people out of the hospital quicker. This seems like a somewhat short sighted goal but it does have some potential benefits depending on the patient and the surgeon performing the procedure. Our goal is that you have a hip replacement that functions well for 12-20 years rather than trying to get patients out of the hospital in 1 day rather than 2 or 3. All surgeons and centers are different. Hope that helps

Pre-hab is HUGE! Makes so much difference in your outcome that it is foolish NOT to do it religiously! 7 weeks post-op and doing fantastic. Quit smoking day of surgery and doing well with that as well. Finished formal PT two weeks ago so working out on my down at our community center 4-5 days/week. Best words in ALL of your videos are the ones I believe Dr. Weening said “After I close the incisions I am just a cheerleader. The rest is up to you!” This made a huge impression on me and motivated me to take my recovery in my own hands and make it successful! Keep up the great posts.

Regarding the crack about socialized medicine in CA: the private fee for service medical system in the US pushes you out of the hospital as soon as you can walk: same day surgery and discharge. Overnight? NFW. Discharge instructions and signoff are always obtained before anesthesia is completely worn off.

Thank you greatly for these videos! I have been dealing with knee pain from osteoarthritis for more than 5 years with physio, injections, bracing etc. I have now gone through the assessment and am awaiting an appointment with a surgeon for a total knee replacement. I won’t be having the actual surgery for at least six months and am looking for suggestions as to pain and sleep management before surgery. I have been unable to sleep more than 2 hours for 3 months now due to knee pain at night. I have been to my GP and have tried all medications they have suggested with no relief. The physio that I am working with to keep my knee mobile has suggested that perhaps I should get and wear a brace that would keep muscles from tensing and pulling the grinding bones together and causing pain at night. Would you suggest something like this? Any other ideas? Through the day I find that moving around and changing positions helps manage the pain. I have not had ant significant relief from Tylenol, acetaminophen or naproxen and get temporary relief from CBD and or. Ice/heat. I’m really trying to keep some level of activity ( missing the 10 +hours of tennis I was playing until mid-July!) but with ongoing interrupted sleep I am finding that increasingly difficult. I would g really appreciate any ideas/suggestions 😀. Thanks!
You cannot believe how happy I am to see good videos done by CANADIANS!!!!! We rock. I had both knees done overseas because I had good insurance and didn't have to wait a year or more. Turns out that the left one has 2 different sized parts as it klunks at every step and the swelling hasn't gone down in 6 years. Going to have it refit or something like that in Nov. and hope it works this time. I didn't have anything like this where I was and got no information from the surgeons (mostly because they didn't speak English) but a little from their secretaries. This information is fantastic - I've watched half a dozen so far and have started the prephysio. My biggest question is when my previous doctors indicated that I would be able to kneel after is not true. I cannot kneel on the bed to pull the covers up. Is this common? Also the first one (in the UAE) said it was titanium so no problems with going through the scanner at the airport. He lied. All I had to do was show the scars and the agent let me through but I was disappointed as it was a time when I was travelling a lot. So, what's with the not being able to kneel? Again, thanks for being Canadian.

What about pre and post TKR exercises. Pre TKR exercises should help educate the patients with proper exercises to do after the surgery as well as strengthening key muscles groups prior to surgery. I appreciated the over all broad brush approach of what to expect and to do. However, lots of surgeons fail to mention the role of physical therapy in maximizing the benefit to this procedure and gaining maximal range of motion.

I just watched your video about knee replacement which I really appreciate.
I have severe arthritis on both knees and both are never fully straight.
Only one knee is bothering me causing me to limp very badly.
My surgeon in New York reccomends replacing both knees at the same time
mainly because if I did only one, I would have one straight leg and one bent one
which would put more pressure on the straight one while walking causing a slower rate of healing.
Also having the benefits of going through the whole process only once.
At the same time I am very concerned about not having one good leg to stand on and wonder
how I will manage.

What do you 2 think about getting both knees done at 1 time? I was told both of them need to be replaced? Also, what if the person refuses to stop smoking and for how long?
So some surgeons will do both. We think it is a lot of pain and a lot of surgery so don’t typically recommend it. As for smoking we encourage people to stop but don’t refuse surgery. Ideally 3 weeks or there is no benefit

Why is the additional weight loss required?
I have lymphedema in my right leg and weight 300+. I have lost over 100#'s and have kept it off for over 4 years. Dr still wants me down to 225 before tnr. Arthritis is bone on bone and losing weight is getting more difficult!!
Hi Maryann. The main reason your surgeon is requesting further weight loss is likely related to the increased risk of complications and greater degree of difficulty of the operation if a patient has an elevated BMI. Hope that helps.
Having the procedure done on June 4th, 2020. Had 3 previous scopes to repair torn meniscus’s due to martial arts for over 30 years. Previous surgeries include a TRSR (Total Reverse Shoulder Replacement) back in 2012, Left elbow ORIF, spinal fusion of L4-L5, partial right hip replacement in 2018. I agree that the sooner I return home produces a better outcome. We agreed, surgeon and myself, that an overstay for 1 night at the hospital, but if I feel like it, I’d rather go home the same day. I currently live in a mobile home so a walker would be a challenge, I’d rather try to get away with just a cane. I’ll have a CPM (Continuous Passive Motion) device along with an Aircast Motorized cooling machine. Pain management is 10mg of Percocet every 4 hours with the addition of Tylenol & Advil as adjunct therapy.

What is your opinion of Total Knee Replacement surgery being done as an Out Patient Procedure? In this case the patient does not remain in the hospital for any length of time but goes home within a few hours of the surgery completion. I am 72 and in good health.
Hi maginot2u. Outpatient surgery is a very appealing option however we do not have the infrastructure in place here in Canada. The majority of our patients stay one night to control pain, mobilize with physio in the morning and make sure they can pee on their own (as urinary retention is a common problem after spinal anesthetic). In general length of stay has drastically decreased over the 15 years of our practice however we are just not quite there for outpatient surgery. It is being done successfully at many other centers elsewhere. Good luck and thanks for watching.
You guy's are great. TKR here in AZ , even on Medicare, is very expensive with the co-payments. I have to play quite a few gigs to save up for it and I have to sit while I'm playing (I'm a bass player so we're suppose to be transparent (unless you're Getty Lee)).

Hello, I’m 65 years old and in 1980 I had a Jones reconstruction procedure on my right knee. I was able to do anything I wanted competitive sports, recreational ports. 2010 bone on bone pain began to make walking a pain. No more injections I am having a knee replacement in 27 days. My question is will my Jones reconstruction complicate my TKR?

Very helpful video as usual. Thanks. Would it be helpful or important for me to buy and use a icing wrap before my surgery which isnt until 12-7-22? I also have a recumbent bike i could use every day before surgery. My knee is not currently swollen and i have been on an rigorous workout routine involving treadmill use and leg machines for the last seven years. I tore my right ACL and meniscus back in 2014 and didnt want surgery so i have been able to exercise with it since then until recently when the knee arthritis got to the bone on bone stage. I tend to overdo exercise so i dont want to do anything that will disrupt my recovery after surgery. I am a little discouraged by the feeling i get from your videos which suggests I may not be able to return to my pre op exercise routine for a looooooooooong time after surgery, if ever. I am 82 but in good shape for my age and need to exercise due to heart disease. Any thoughts you have on this would really be appreciated. Thanks again.

Thanks for another thorough video. *Regarding swelling*: I had very successful hip replacement surgery this summer, but I had trouble with swelling and also with finding a comfortable position to sleep for the first week. The home care nurse suggested sleeping in a recliner, which I did not happen to own. When I looked for one, I found an *"anti-gravity" style chair*, which is designed to recline past horizontal so that your feet can be positioned totally above your heart, with your body bent forward at the waist and your knees slightly bent and supported from beneath. The feeling of support from head to toe is incredible and allowed me to sleep like a baby, and the elevation of my feet maximized the anti-swelling effects of icing. (Apparently, this style of chair was inspired and invented after observation of the natural shape the bodies of astronauts take when floating in weightless conditions.) I anticipate total knee replacement this December and expect to make good use of this chair again.
Hi Michael. Thanks for watching. Sleeping on your back can be a big challenge for people after hip replacement surgery but it does get better. And the swelling improves, slowly, but it does. Usually we tell people 6-12 months. Good luck with the total knee.
I've never had an iota of pain in the joint itself since the hip procedure, but my leg muscles had atrophied so much in the three years I allowed pain to limit and ultimately halt normal exercise that it has taken time to build their strength back up. At ten weeks post-op, I can report I am pain-free and able to do just about anything I want, including riding my bike, which is proving to be very beneficial. If I had it to do again, I would not have waited so long after recommendation that replacement was necessary to have it done.
Hi Michael. I think that is a very sensible approach. Getting moving as soon as you can is a key factor when recovering from a total knee replacement. But everyone has to do it at their own pace. Continued good luck to you as you get back to normal.

How long do you normally stay in the hospital after a total knee replacement
Hi Patricia. All centers and patients are difference but typically 2-3 days in the hospital. Some centers keep you in the hospital for 1 day and then transfer you to a rehab facility.
Very helpful. I'm having TKR Revision on the 30th. Next yr TKR on other knee and last yr 3 failed back surgeries. Super concerned how I'm gonna get thru pt when I can't do much with other knee and with back, but gonna push thru somehow.
Sounds like you have had a pretty tough course. You are right - determination is the main thing. Doing whatever you can preoperatively to get strong and prepared. Good luck.
I know this video was posted 6 years ago, and I have posed a couple questions on 2 of your other videos about TKR (thanks again for your replies), but have either of you ever performed TKR on a patient with transverse myelitis? Or do you know someone who has? My orthopedic surgeon who is performing my TKR in Oct. has not. I would like to get a better idea of what to expect post-surgery considering my unique and rare condition. For example, will the implant itself heat up or get colder with weather changes? Since I am already temperature-compromised I need to know if the implant will exacerbate that sensitivity.

Your videos have been very helpful. I've been dealing with arthritis in my right knee for a year, went in last Dec to have a tear fixed because of the pain and the dr said the tear was not causing the pain it was that I had grade 3 chondromalacia femoral condyle of the medial joint so been through cortisone shots 2 times and synvics and medication and nothing is working seeing my orthopedics doc in a few weeks to discuss surgery, is it normal for someone 51 to have to have a total knee replacement so young? I know the pain is horrible and getting worse.
Hi Lisa. So typically a knee replacement would not be the treatment of choice for grade 3 chondromalacia. Usually it is done for grade 4 - or bare bone. Knee pain is a difficult problem and it is important to accurately diagnose where the pain is coming from and then to exhaust all non knee replacement options before proceeding - especially at a young age. 51 is young but there is no perfect age. We recommend that people should wait as long as they possibly can to have a knee replacement because it is a good but not perfect operation and ultimately was designed for your parents and grandparents not for you. Younger people have the highest chance of not being totally satisfied due to their expectations. Have a long discussion with your surgeon. Good luck
This is a question for one of your shows and I need your thoughts I have a bad knee and hip on the same leg both with arthritis right now I am doing cortisone shots for both how would I go about having surgery on either one with pain in the other probably stupid question but I worry if it comes to that

Just wondering...have you ever done a TKR on a patient who had a stroke on one side, but a perfect knee there (can walk, foot drop with AFO, weakness , etc) but the others leg has the lateral aspect of the femur sitting on the medial of the tibia? Just wondering if there was any chance of a good outcome since the stroke side will never be optimal as a helper. I'm supposed to get this fixed although I have no pain in that knee, but my surgeon has never done it in this situation so I'm his guinea pig. If I can't rehab it, I'll never walk again, I suppose. Pretty scary.

It’s C What you need to do is to prepare people by telling them that they will basically be losing 6 months to a year of their lives. Minimizing the recovery is just a complete disservice to the patient and keeps them making an informed decision.

Hi Darlene. It may be that when they cross matched you for a possible blood transfusion they found antibodies so it means they have to find a more specific blood match. I have Complex Regional Syndrome.. but am bone on bone and in alot of pain because it. With my OA. It was advised by my Orthopedic doctor to have a total knee replacement, however he stated because of my CRPS he stated it's up to the orthopedic Surgeon he is referring me too.. What is your take on this.??? I feel I don't have any other options I want to proceed.
Hi Pamela. Ultimately the surgeon needs to agree to do it but yes if you have advanced arthritis and have exhausted non operative measures it sounds reasonable. Thank you I truly appreciate your advice and I am advance OA per MRI report along with other mild to moderate issues happening to the ACL now plus bursitis, however my meniscus was repaired last Nov. 2021, and is staying in tacked so far. It's just extremely difficult to walk, for any length of time. Am hoping for the best and in 2 mos. I hope to have my answers. I'm just starting my research on knee replacement as it's coming up for me in the near future. I know there is going to be pain after the surgery and I can deal with that after living with arthritis pain too long already, but after the pain from surgery passes and pain from muscles heals, what is causing pain after that? Sorry if that sounds like a really ignorant question, but if the knee replacement parts are not actually part of the body, (nerves and such) what is causing the pain? Thanks for your time.
Hi Juanita. Very good question to which there is not a clear answer - if you can figure that out you would solve a problem for a lot of patients and orthopaedic surgeons. The pain is likely coming from the soft tissues - the lining of the knee, the muscles, the tendons, etc. Some also think that when someone has pain for so long that the joint becomes wired for pain and that some of that is irreversible. Good luck with your procedure.

Hi Sandra. There is a lot of pain with total knee replacement surgery but with exercise, ice and pain medication you can get through it. It is painful but everyone gets through it. You just need a plan. Good luck and thanks for watching.

Hi bluesmanreturns. After total knee replacement there can often be sounds or clunking throughout range of motion. This can be related to the fact that you have an artificial knee made out of metal and plastic and that your patella (whether it is your own or has been resurfaced) clunks over the femoral component. If the clunk was related to soft tissue getting stuck in the notch arthroscopic treatment has the potential to reduce the clunk. The main indication to do this procedure would be if the clunk was significantly painful. Hope that helps.
The Main thing My Doctor did not tell me my implant would be hitting my patella with every step making it painful and swelling around my knee cap area , its also really annoying and loud If I new this before hand , I would not of had the operation.. And now my Doctor isn’t responding to my pleas, Any solution for this ?

It is important that you ensure that whatever damage to your skin that has happened doesn't get infected. Hi Docs I was assessed for total knee replacement on both knees three years ago I couldn't have it done then my right knee is grinding and I can't straighten it out so my foot turns out I am making a new appointment to have it assessed . It's preventing me from walking very far at all
Hi Charlotte. Often as arthritis progresses you will lose the ability to full extend - this is called a flexion contracture. If you have been suffering for a long period of time it is a great idea to get assessed and to discuss all of your options up to and including total knee replacement. Hope that helps

Hi Brian. That is pretty easy at this point. There is no scientific evidence to support stem cell injections for advanced arthritis at this point. TKR if you have severe pain and severe OA. Thanks for watching.
Talking With Docs , my doctor tells me that I need a TKR but I’m procrastinating. Someone in my exercise class was told that stem cell treatment was not going to help her. She found another doctor who gave it to her and now she swears by it. I’m just trying to be informed and exhaust all my options before going through with the surgery. Thanks for getting back to me.

Swelling? How much swelling will I have to endure? Do I need a brace for after a TKR?
Hi Sandra. Swelling is very common and will typically decrease over time. Usually you will not need a brace after a TKR but this can be discussed with your surgeon. Good luck! And thanks for watching.

Before deciding on a knee replacement surgery understand one thing: Surgeons do surgeries. Period. These guys are older in the tooth and seem to want to stay with old-fashioned painful traditional knee replacements when modern evidence DOES show that regenerative therapies like PRP actually work. I work out in a fitness class with a lady who worked as a registered nurse before retiring. Her husband is a physician. Before PRP she could hardly do a lunge move without wincing in pain She is now totally pain free and doing every move in the intermediate HIIT class. She had PRP therapy...not sure if they used fetal blood or her own blood. Downside is that in US Medicare wont pay for it because it's considered experimental. It can be quite expensive out-of-pocket. Due your own due diligence and don't listen to just one surgeon before deciding on knee replacement. Younger more open-minded docs will be more receptive to surgery alternatives like PRP therapy. Don't just listen to one doc who says surgery is the only answer.

Hi Joyce. There are many different approaches to the hip some of which involve cutting or releasing the insertion of some of the muscle and reattaching it at the end of the procedure. There are pros and cons to all approaches and recently there has been some enthusiasm for the anterior approach particularly because it gets people out of the hospital quicker. This seems like a somewhat short sighted goal but it does have some potential benefits depending on the patient and the surgeon performing the procedure. Our goal is that you have a hip replacement that functions well for 12-20 years rather than trying to get patients out of the hospital in 1 day rather than 2 or 3. All surgeons and centers are different. Hope that helps
