Interesting that you only briefly mentioned sugar, and not as a direct cause of raised triglyceride levels. Sugar, including those from carbs and particularly from fructose, which can ONLY be processed by the liver, is a major cause of high triglycerides, along with alcohol. I would imagine that the paradox with pancreatitis is that low carb diets, which replace sugars with fat as the main source of fuel, would presumably place too many demands on the pancreas? As pancreatitis is triggered by high triglycerides and therefore often by inappropriate diet it would nevertheless be interesting to see if low carb diets and/or fasting could help, at least with prevention if not as a cure.

Yes fructose is one of biggest reasons our country is plagued by fatty liver disease. The levels of triglycerides required to trigger pancreatitis are hard to achieve without genetics or alcohol mucking up the usual metabolism of triglycerides. So I don’t expect the low carb craze to cause much problem for pancreatitis (though yes it will for some).

Biochemical pathways of carbohydrates' metabolism DIRECTLY lead to lipid metabolism pathways ; where the lipids' production is directly proportional to the carbohydrates' intake .
That is achieved through 2 substrates in the carbohydrates' metabolism pathway :
First : Dihydroxyacetone phosphate (DHAP) .
Second : Acetyl coenzyme A (Acetyl coA) .
The first substrate : dihydroxyacetone phosphate (DHAP) is generated down the glycolytic pathway of both glucose & fructose in the cytosol ; where it undergoes a crucial metabolic step to enter the lipid metabolism pathway .
That involves its reduction to GLYCEROL 3 - PHOSPHATE providing the active form of glycerol , the backbone for the DE NOVO synthesis of triacyl glycerol (TAG) . That reaction is catalyzed by the key enzyme GLYCEROL 3 - PHOSPHATE DEHYDROGENASE (G3PD) & NADH + H as a cofactor .
NOTE : The rate of generation of dihydroxyacetone phosphate (DHAP) from fructose is higher than that generated from glucose . Where fructose generates dihydroxyacetone phosphate (DHAP) through ONLY 2 metabolic reactions down its glycolytic pathway , while glucose generates it through 4 metabolic reactions down its glycolytic pathway .
That might explain the association of fatty liver disease with high fructose intake .
The second substrate : acetyl coenzyme A (Acetyl coA) is generated through the oxidative decarboxylation of pyruvate - the end product of glycolysis ; where it undergoes a crucial metabolic step to enter the lipid metabolism pathway .
That involves its carboxyltion to MALONYL COA providing the substrate for the DE NOVO synthesis of fatty acids essential for the esterification of glycerol to generate triacyl glycerol (TAG) . That reaction is catalyzed by the key enzyme ACETYL COA CARBOXYLASE (ACC) & BIOTIN as a cofactor .

About 20 hours ago I had an homemade ketovore pizza. The crust was coconut flour, egg, cheese and galic powder. The sauce was primal kitchen unsweetened ketchup. I topped it with cheddar, mozzarella, uncured beef and pork pepperoni and organic spicy sausage. I normally eat carnivore but wanted some fiber to poo so I added the coconut flour. I drank orange muscat. Which I am sure was loaded with sugar. I drink periodically. No pattern or consistency. Some times I drink 3 days in a row and other times I don't drink for months. And sometimes I drink once a week. I'm alight weight so one glass, one shot or one serving is all I ever need. I have not had an appetite since. This morning first I drank RO water and after, about 18 hours since the pizza and wine I made a cup of espresso and added coconut oil and salted butter. I went to the gym and got pain where my pancreas is. This scared me. Added on the fact that I wasn't hungry and usually, in the past, coconut flour increases my appetite. So, I come home and check my sugar which was a normal low for me. Especially after fasting. I still have a dull discomfort in the area of my pancreas. I'm drinking water. But, I'm nervous and don't know why I have this. And what I should do.

I have the exact same shitty diet, and also drink 2-3 times a day some days of the week. You need to fix your diet and reduce sugars and carbs. No more drinking. Avoid unhealthy fats and red meat, only eat healthy fats.

Very helpful info! I had pancreatitis in 2018 due to high triglycerides and am still trying to figure out what to watch out for. I have DM-2 and recently tested positive for the antibodies confirming I’m also DM-1…aka LADA. For the last year my shoulder has started to have issues so I got an MRI, but they want to redo it with contrast because the first one was inconclusive. My PC/GP believes it to be a side effect of the diabetes and the shoulder issue is more prevalent in women (that’s me); so I’m still hunting to find answers.
    Thanks for the info!

I have a history of acute pancreatitis do to high triglycerides (dangerously high, the highest being close to 7,000. doctors didn’t understand how I was alive). The high triglycerides triggered high lipase and amylase. I’ve had 2 flare ups between 2018-2020. Multiple doctors have told me to eat lean meat, low fat dairy, low fat in general but I’ve also seen quite a bit about low carb diets like keto or carnivore actually improving pancreatic function. I’m so confused about what my diet should be like. I also have PCOS that has gotten out of control lately. Please help!

Polycystic ovarian syndrome (PCOS) is highly associated with type 2 diabetes mellitus (DM-2) .
You ought to check your blood sugar regularly to make sure that DM-2 is NOT the cause of your hypertriglyceridemia pancreatitis (HTGP) .

Thank you for another excellent video! I take the generic for Trilipix to keep my triglycerides under control. They started to rise after I began HIV medications in 1998. My cholesterol levels began their increase around that time as well. I take the generic Lipitor for them.

Edit: my triglycerides were 6,000+ when I had pancreatitis…scary to think people get into the tens of thousands!! 😮

The 4 times I had pancreatitis (the first time it put me in the hospital for 6 months) it was from alcohol.
