Sentara Nursing Bundles: Hourly Rounding

This video shows example of Hourly Rounding by a nurse and ncp/tech.
1. Hourly rounding is when nurses and other hospital staff members work
together to check on each patient every hour and address any unmet
needs (round with a purpose).
2. Hourly rounding will include the 4P’s of Potty, Positioning, Possessions
and Pain.
3. All team members will take turns rounding. This will be discussed and
planned at the beginning of the shift.
4. The primary responsibility for hourly rounding will be assumed by the team
members of that unit (RN, LPN, and NCP). However, support staffs (i.e.
housekeepers, respiratory therapy, PT/OT, case managers, etc) are
expected to address the needs of the patient whenever there is patient
