    I’m a dentist that was considering offering all on 4 at my office. As soon as I saw a few live surgeries and witnessed the removal of bone to create space for the prosthesis , I decided that I could never offer such a procedure to my patients. We were taught to preserve hard and soft tissues in school and now doctors are removing shocking amounts of bone instead. Couldn’t sleep well at night knowing I’ve done such a thing . Implant bridges are the way to go.

      You are absolutely correct it should be more affordable. It will never happen. When there is non surgical means of replacing teeth. That are cost effective and low risk. Instead trade it for something that has risk of failure and infection. You very mistaken. Will use me and the trouble with my teeth. See if I should have implants. I have severe gum disease. Which is already causing bone loss. I am 51 years old and in good health. I have very severe tooth decay. Teeth that are broken at the gum line. Previous infections have have spread beyond the tooth. They can be seen on dental X-Ray. Now they are in the bone. Any dentist who was decent dentist. Would not put implants in my mouth. I would be at high risk for failure of the implant and infection. The impression molds for denture fitting. Would be in front me in second. How about we make it so everyone can see dentist. From the time they are child until they die. See doctors from the time people are kids until you die. Makes a lot more sense to me.

    I’m angry that so many dentists just see the $$ signs and suggest all-on-4 treatments and do not go into the detail of what possible failures look like. THAT IS NOT INFORMED CONSENT! I’ve had failed implants and now have snap-in lower and a full upper denture. But the tissue and bone loss continues. I still have inflammation and infection around the sharp edges of the screw ridges that are showing on retained implants. I think they will eventually also
    Have to be removed due to continuing problems. I really appreciate this informative video and your time in presenting.

    I am in tears after watching this. I had most of my teeth and was told by the sales person that all my teeth would eventually decay and need to come out. I saw a ton of videos, before and after pictures and fell hook, line a sinker for the pitch. I had one tooth missing on the side that showed when I smiled and I no longer smiled because of that hole, my smile had been my best feature. Now, after the surgery just three weeks ago, my entire lower part of my face looks like it was chopped off. My once full lips are sunken into my head and I have these itty bitty lips. I look like I am missing all my teeth until I smile and then my smile looks demented. I lisp terribly and spit when I talk, I don't even want to go out in public. I was pretty three weeks ago, now I just look like I am 90. My chin is gone. I was told after the fact that I had advanced periodontal disease. Why on earth would they tell me I had plenty of bone and was a perfect candidate for this procedure? I am devastated. I live in Fort Worth, I will be contacting your office for a consultation.

      Hi Jill ! How are you doing so far. I looked just like that, I felt my beautiful smile faded away, my chin looked like chopped off also , but now it recovered its natural shape. My dr took out my temps after 4 weeks due to the pain and discomfort I was feeling. My dentist did a bad job with the impression though. I was without the temps for one month and a half and with the exposed abutments. And I could see my chin was kind of flat and thought my chin bone was broken, and that was the cause of the excrutiang pain I was feeling. But it was not!! Something change in our physiognomy after surgery, but it doesn't stay 100% that way. But things will become to the natural look when we get our permanent teeth, no more bulky lips like with the temps, making us look someone different. I still have to deal with that a little bit. Eventhough I made it smooth out a lot with the lab tech guy. My lower lip still scabs 24/7 , sometimes I get hurt peeling it off. Yes I use lip balm and hidrate a lot, but still scabs. My smile looks wider than my natural one and that is because the temps are bulky and forwarded a little bit . And stretches my lower lip making it scabby and also making me look my smile completely different. But I am ok with that, I know everything will be different and normal or close to normal when I get my permanents and final work. Hopefully you are doing great and learning how to deal with all that discomfort you were going through. Everything is gonna be ok.
      Be safe! 🤗🤗

    I had an all on 4 on (both the upper and lower) about 8 years ago. There was no bone removal and no angular abutments involved. The upper has been completely trouble free but the lower has had repeated mechanical failures of the denture The dentures have been acryllic in all cases. The lower jaw sees much more mechanical impact loading than the upper The denture maker has redesigned the lower several times
    and each version is a tad better than the predicicator . Am I overall happy with my one on4s ? It has been a developmental process. The original work was done in Ukraine at a reasonable price.

    Thank you so much for providing this information. I have been considering all in for treatment and now and further educated on what that would mean in terms of bone reduction. Bone reduction is the whole reason why I wanted all on four because the bone loss I have experienced after having all of my teeth removed two years ago. I am shocked to hear that they remove a lot of the bone in order to make way for the prosthesis

    • @mcfaddendentalimplantcenter
      @mcfaddendentalimplantcenter  2 года назад +3

      Everyone is different. The amount of bone removal would be directly proportional to and relative to your bone resorption pattern and volume. If you've already lost a great deal of bone volume, there would be less bone removal involved. I hope this helps.

    To recognize all the possibilities and to choose the sustainable way to treat every individual is the responsibility of the medical service provider. We, dentists, should practice not only implant dentistry but also orthodontics, periodontology, endodontics...., and so on. Maximum preservation of tissues still is the mainstream principle. The dental implant does not have a periodontal ligament, so the perception of force is less sensitive, and to keep natural teeth may be the first order except making the reconstruction work compromised in esthetics or complex works in mass GBR. Thank you for your nice and justified presentation.

      Can you fit my dentures in lower, as I want to get maybe 2 clips. My 1st set of teeth had 4 bottoms to snap in and hold. My dentist told me I should have the metal snaps so that if my teeth went bad that were there for clips. That knowing these teeth were also bad and would actually be rotting eventually. So I got the metal lower set fitting... So if I would have got the plastic clips I would have had a big bill to pay. Where as the metal if my teeth were pulled the teeth missing In the lower denture would be free of cost to add fake teeth when I needed them pulled! Well I got them pulled, and my original Dr. Moved to a new country. So the new dentist said that my gums had changed so much that he wouldn't want to use someone else's work. He redid my dentures, and my teeth that were pulled took forever to heal. About 6 months! Then he fitted me for bottom dentures. When he was putting the piece in my mouth to pure in the solution to set where and how my teeth needed to be fitted. The piece they were trying to force it in. Instead of using maybe a child's mold piece as my mouth is so small. I could hardly keep my mouth stretched. They told me I needed to let them do this they forced my mouth open so bad my lips bled for days I had sores all over lips. And they said if I didn't let them continue, because it was so bad that I was crying! They said I wouldn't be able to get my new teeth. I've had them for 2 years and had not ever been able to wear my bottoms yet. I've been back to him several times he talks for 3 minutes and the assistant pores the mold to try to fix them. My set was never filed and or smoothed correctly. I had a bite that would catch my gums and bite Everytime my mouth would move. The back dentures are just white acrylic look like molars. He had to file them down so much it was only an eye illusion that I had. Molurs. So I have been through alit . My first time the dentist was pulling all bottom at once and all top at once so she started and my teeth were hooked so much and double rooted, that she was using her foot on the chair and her and assistant had such a hard time getting them out we were exhausted. She said if known she would've put me to sleep. Even my front teeth top and bottom were double rooted and hooked.

    • @darlenenanettehorn8918
      @darlenenanettehorn8918 2 года назад +1

      My mouth healed quickly but from the mouth going through unusual trauma from being hooked and double rooted, I thought there were teeth growing in on my top roof and bottom of gums. Well they did more x-rays so that entailed that my head face had more bone than others and it was from trauma that this bone was pooping out and it took a while summer 5 surgeries for removing all the bone that stuck out like teeth. So I believe that sitting all summer by the time I got my 1st set my gums had already changed, and to big. Also it had taken so long that my whole structures changed. When I went to surgeon they wouldn't put me to sleep because they said my mouth was to small and it would cut off my breathing with all the tools and 02.. so at the dentist I had a limit on numbing med which I felt a couple teeth being pulled worst pain. And also with the surgeries only so many doses are allowed for numbing. I had to get my gums sliced to get bone inside gum. But I really had a good run with tops and bottoms that clipped in. I am wondering if there are authorizations allowed for people to have the screw ins covered and paid by insurance. Beings I have slot of guy issues, I can't eat anything with gluten. Celiac disease. Then diroticilitis. IBS and Crohn's and colitis. So I already had a hard diet. Now I can hardly eat any food without having to smash it all up. Make it soft. But even now I wear my tops and they cut my bottom gums. Then I can't eat because I have sores all over bottom gums. I've lost 66 pounds between no nutrients with my gut diseases and only a soft limited diet because I get cuts all over.
      Then this is a bad problem, the furthest gum in the very back of mouth. My jaw bone sticks up so high and at times breaks through to skin, they are so painfully I can't even let soft stuff touch. And my denture on one side ribs it and then I get abseses. They told me they won't touch these gums because the bone controls the tougue and speech. Is there anything I can do is there maybe some rubber shield I can get to protect them. Because I can't even brush it. It's a toothache that's like phantom. But I never had teeth there. I did have my wisdom 4 teeth removed! So that was on the end why can't they just do something to alleviate the painfully back of mouth all the time
      I also have to use children's toothpaste because my mouth gets so dry and the sores burn so bad I also get the cracking in corners and sores. I really think maybe there's an authorization I can get beings my mouth will never stop forming differently because imm starving half the time. I don't get nutrients needed from food I can't eat because I can't chew. I need about 4 pints of blood every new season. And I'm weak and then I swell up all over. I either will have diarreaha so bad I can't leave toilet. Or constipation so bad it takes 2 days to empty out. It's ungodly huge. Which my GI Dr. Said my intestines are constricted from not having the correct nutrients. Is there any diagnosed I could maybe be authorized to get snaps. I'm tired of being hungry and chocking and I am starting to lose facial looks to a look of someone who doesn't care about there mouth.

    This is really sad. At 34:00 That lady had decently nice teeth and that office she went to destroyed that for her. This is why we all need to pray and really think hard about the decisions that we make. No matter what anyone tells you, study and get all the True knowledge that you can for yourself!
    In my line of work I always try to tell people the truth even if it makes them mad or cost the business money. At the end of the day I go to bed knowing that the company still made money and I gave the client the best and most correct answer that I could give them.
    Always try to do the right thing even if knowone see you. Even if it may hurt you or make you look bad.

    I will be seeing you on 4/27/22 from out-of-town. I hope you can replace multiple teeth which are in scattered parts of the jaw. From experience I've learned that root canals don't last forever. They eventually fail and you have to get implants or dentures. I also hope the price tag for replacing multiple teeth doesn't break the bank.

I was just about to start all on 6 procedure but reconsidered and will aim to do a bone reconstruction on the sides, sinus lift and try to get dental bridges instead. More painful, more time, more expensive but we’ll worth it. Too bad I’m in Canada, otherwise would definitely visit your clinic.

    I've already commented on the shorter version of this technique and it's very logical approach to tooth loss. This has nothing to do with implants, just the dentists out there who seem to have no grasp of what's healthy for teeth in general. Concave vs convex when it comes to basic dental fillings. Maybe others will have noticed the same issue. How many people who had fillings done decades ago where the filling was carefully shaped to mimic the biting surface of our tooth? Now, in the years since, dentist simply create a concave filling, leaving only the outer edges of our natural tooth to contact the matching tooth above or below. Personal experience is that these tend to fail. Either fracture the enamel or due to shrinkage of poorly installed amalgam... leak. Resulting in decay deep inside the tooth
    I had fillings last decades when they were shaped properly, and ones that failed within a couple years when done in this hollow cup shape. Destroying all my back teeth. Is it that easy to graduate dental school without basic common sense skills? Only 2 weeks ago, after having a filling repaired, I noitced at home with a cheap dental mirror that I had small dark spots at the gumline. Even in the tooth my dentist had just repaired. So I asked to have x-rays done. Results? He showed me that all my back teeth had decay below the gumline and would have to be pulled out!!! I'm still reasonably young and have taken excellent care of my teeth. Require fillings as at a very young age because my molars didn't "seal" and my old dentist said it would protect my teeth from decay. And so they did most of my adult life. Until I went to this clinic. It was at this point my new, straight out of college dentist tells me he only has basic dental skills and blames dry mouth for this rapid decay. Over a span of 2 years all this decay happened and would continue unchecked had I not insisted on the x-ray. Wish I had asked sooner. Before it was this bad. All I can do is get a second opinion before, at some point.... Lose all my molars and try to live with front teeth only. I'm disabled and don't have the resources to manage $30,000+ for what it might cost to replace. I'm a poor candidate for the dreaded dentures I so despised the very thought of due to oral Lichen Planus. Something that also began 3 years ago when I began having my dental work done at this clinic. I've been told it's due to a severe reaction to the large amount of amalgam I have in my mouth. Causes painful lesions on my cheeks and a very deep one on my tongue. 4 specialists and I still suffer from this. Was waiting for a patch test before we could move onto replacing the amalgam. I guess it's not going to be an issue once I lose all my back teeth. It's devastating. I've suffered excruciating pain from the lichen planus and now face the rest of my life without back teeth. It's already difficult to eat enough food to sustain me. I'm simply exhausted from fighting to protect my health in a profession that's profit driven. Not to help patients no matter what their financial circumstances.

      @mcfaddendentalimplantcenter  Год назад

      Yes, dental students of today are woefully unprepared for even basic dental care. All US systems of education have been dumbed down so much that it is SHOCKING that we can't change the course. It sounds like you have severe dry mouth. Dry mouth can be caused by over 1,000 medications. Perhaps you are taking one of them? It sounds like you are headed for dentures... I am sorry to say. Brush and floss like crazy and ask your dentist if you are a candidate for fluoride rinses or fluoride trays. It will help slow the progression of the decay on your good teeth.

    • @leianehiltz2486
      @leianehiltz2486 Год назад +4

      @@mcfaddendentalimplantcenter You'd be correct about the dry mouth but I've been fighting it with products like Theramelts and sugarless gum at all times. The biggest impact, I developed severe Lichen Planus 3 years ago. I've always invested in a good quality electric toothbrush and floss after each meal. Yet the erosive lesions also cause very irritated gums and gum recession. Despite the pain I haven't avoided my dental cleaning routine. My last specialist consult, she tried to express my saliva glands but nothing happened, yet while I have gun or the melts in my mouth it waters nicely. It's just been an excruciating mess. Special toothpaste (non flouride, xylitol, sls-free) as it's the only kind that doesn't burn like acid. Most things burn terribly. I've lost the lining of my mouth and taste buds. Havent tasted food in as many years. I have to stop and brush during meals as any food between my teeth causes pain. Can't say enough about having a good electric toothbrush.
      Also been told I'm not a candidate for dentures due to how reactive my mouth is. Seeing several specialists, many compounded oral rinses, high dose steroids...the fight goes on for a cure. Had one implant thats been a success. Best educated diagnosis so far is a reaction to amalgam but have been waiting a year for a patch test which may be pointless once the offending molars are extracted.
      It's been a very painful, long and expensive 3 years trying different compounded medications (pricey). 4 specialists and travel. Point being....this news was almost the last straw. I dreamed of maybe one day replacing the 3 other missing teeth with implant. NOT the entire back teeth, top and bottom. Exhausted, and frankly hope waning badly. Difficult enough dealing with the uncertainty of how long this Lichen Planus may go on, but add the loss of teeth as well and I don't want to imagine life ahead. Eating is hard enough now with a large lesion on the right side of my tongue, lacy patchwork of raw tissue on the cheeks. Only began watching RUclips videos like a child looking through the Sears Christmas Wishbook. Seeing what would work best for me, if it were possible
      Thanks for your valuable input. Excellent videos explain so clearly why other implant options aren't as safe/healthy. That's all most people want is a healthy mouth= thus a healthy body. Many may not understand why the mouth is the gateway to a long life with less risks for heart, lungs,digestive, neurological disease. One day perhaps a more enlightened society will include dental care as a right to all citizens and save so much money from all those other diseases.

      I also had my molars removed for holistic reasons - large amalgam fillings and root canals and asked my dentist to make me a simple flipper retainer ( just like you wear after braces ) and I had him put the missing molars into the retainer ( like they do for a denture) and I have been wearing this comfortably for 25 years with no problems, and I can chew everything .I used to work for an orthodontist and used to make these little flipper retainers and that is how I knew it could be done . This might be a perfect solution for you too instead of dentures or implants .

      @leianehiltz2486 Год назад +3

      @@dianebarron8362 That's a unique solution I hadn't heard of. The main issue is the same as what's attacked my teeth to begin with - erosive oral lichen planus. I walk a fine line keeping any bacteria cleaned out of my mouth or else it infects my tongue and cheeks. 5 specialists agreed that placing any device in my mouth. It'll trap bacteria in and around the device.
      I sure hate what this is doing to my health and facial appearance. Having lost half my back teeth already and my face is thinning down. Also, once you pull teeth, unless you get bone grafts you lose bone density. This also causes gums to recede further, which puts the other teeth at risk.
      Rural dentists are too quick to opt for pulling teeth rather than putting enough effort into saving natural teeth. Frankly I'm exhausted from trying to advocate for myself. How dare I assume I have any knowledge. Mocking "doctor google" when you're actually listening to professionals in the field, who are passing on their own knowledge. Not sure if doctors assume any and all information patients glean is from social media or half baked theories but the condescending attitudes is both insulting and dangerous

    I had all on 4 implant one month and a half and I was in excruciating pain for 4 days. The fifth day the pain was bearable but never disappeared until now and also I still feel numb on my chin area. Went to see my dr. He was kind of rude with me sayings and asking me why I am feeling pain , if I feel numb how can I feel pain. He was so angry at me as if I would be crazy inventing my pain. The stitches were not dissolving so he took them out together with my temps. I had to go back three times for the buried stitches to be removed that was causing me irritation and sensitivity and pain to my gum tissue. The throbbing pain I feel almost every day running over each implant sometimes is unbearable. Since the first day of surgery, I saw the abutment a little bit exposed and I don't know if that is the problem. My dr. said after he checked it out that my bone is ok, the implant are ok there is no infection that everything is ok. Then , why I feel this throbbing pain all over my the implants? He didn't put back my temps by the way, untill my next appt on March 31s. Was that ok? Do I need an optional treatment to eradicate the pain? I am afraid to keep saying to my dr that I am not feeling well with my implants, that I can't sleep well, that I have to be numbing day and night with Orajel and taking over the counter pain killers with temporary results.
    Dr What could be the problem or what is going on with implants? Could you please give me an idea? 😢😢 this throbbing pain it really sucks.

      I am familiar with that technique. Pros and cons just like every other treatment. In the past three years I have come to believe that on piece is better than multiple pieces (bridges) because we get "cross-arch stabilization", meaning, the the implants on the opposite side are providing stability to their counterparts. Also, screw retention is always better than cement retention. Still, 3-on-6 is far better than All-on-4.

    What a Godsend. Thank you so much Dr. McFadden for such an informative video. Tomorrow I will have a chat with a dentist regarding implant options. All of four will not be an option for me. Thanks to you I'm much more informed.

    Can this be done in sections. Like front 4 teeth on one day. No dentist wants to even hear about this . I had a dentist say what is it with you people in wanting teeth with no gums . I just wanted want i want and because it’s high priced to be able to do it in a reasonable financial way. Forget that in Jersey the just say here is the 19k bill and we’re gonna knock you out and grind all your bone down . People have no clue how frustrating it is to actually find someone to help and work with you .

      Yes, in many cases, it can be done in sections. I’ve done many of them. For instance: Surgery 1: Remove some teeth and if there’s adequate bone in some spaces to place a few of the total implants planned, go ahead and place them, and leave a few select teeth temporarily, to support a fixed temporary bridge, and not use those new implants immediately. Let them heal while the selected remaining teeth support the temporary. Then for surgery 2: remove the remaining teeth, place more implants and use the previously placed implants to support the revised (or new) temporary bridge, while the new ones heal. Then, once all have healed and integrated, get the final bridges. Typically in 3 or 4 sections, depending on whether there have been 6 or 8 implants.
      The benefits of this approach are that you don’t have to wear a temporary denture, you don’t have to immediately load the implants when they’re placed, (risking their success) and it allows for proper scalloping (shaping) of the gum tissues for the final teeth. It does involve more visits and work on the dentist’s part, because of the revised or new temporary bridges, but it can be done in many cases. I hope this helps.

    The biggest drawback of implant dentistry is ,the fixture is entirely differ from natural teeth roots. It's violated the law of physiology, anatomy and bio-mechanics. and occlusal principle, can not achieve the goal of complete dentistry .
    that is functional harmony, anatomic, bio-mechanical harmony. of masticatory system . with the result to have a predictable maintainable stable of each component of masticatory system in natural dentition.
    So no matter how implant prostheses designed , lots of late biological and bio-mechanical complications revealed in literatures, severe enough ,ultimately lead to failure.
    Master of Prosthodontics, Dr. Dawson said:[Unfortunately, it is unusual dentist graduated today have a workable knowledge of Temporomandibular joint and occlusion]
    [there is high confusion of occlusion. ] [ majority of dentists practiced incomplete dentistry. ] they do not know that they do not know.

    Who wouldn’t want a solution which is clearly superior in every way? Unfortunately the reality is that only a small percentage of the population can afford your elegant solutions, so I must ask what is the solution for everyone else who desperately needs a solution but can barely afford even the all on 4 sub par offerings?

      A skilled Prosthodontist who cares about outcomes can offer a wide range of good options for patients missing or soon to be missing their teeth. Complete dentures are a lower cost option that the vast majority of patients can afford. Unfortunately, the average dentist skips at least two important steps in making complete dentures whereby most patients have limited success or failure using them. One step up from complete dentures is also much less expensive are over dentures (snap-on). With as few as two dental implants, dentures can be stabilized and very functional.

      You do what my husband and I are doing. Look at your budget get the best denture you can afford. If you are low income individual. Google low income dental care. The city you live in. There should be clinics to call near you. Dentures for my husband and I. We ordered DYI denture kit. Our budget was $100. Took several months of me babysitting. In addition to caring for my husband who has dementia. When you don't have enough money to make ends meet. You get depressed which is something I can understand. The way to be poor and happy. Takes two things. Using the limited resources you have. Making them work and enjoying the results.

      The answer to your question is:
      You do implant bridges.
      I’ve got several videos on this topic if you’d like to learn more about them. Just click on my name under this video and it’ll take you to my channel. From there, you can scroll through all of my videos and find those on the topic of implant bridges.
      This is a big decision, so you are wise to take time and do your research. Best of luck to you.

    I found this video to be misleading. The fact that this "doctor" says the all on 4 are recommended for everyone is false. Most people who get this procedure are very happy with it. The first pictures of the restoration looked altered. Very yellow compared to the real thing. Compared to dentures and bridges which will need to be replaced someday, all on 4 is just as cost effective. I think this presentation was mostly one sided and biased. Also, if there were this many problems with all on 4 don't you think there would be a ton of lawsuits and people out there dissing all on 4? The only videos I see dissing all on 4 are doctors pushing their alternative products. Sound familiar?

    Unfortunately, most Dental doctors by using this dental implant technology, want to be a rich man in three days. According to doctor McFadden this procedure of All-on 4 is more faster than bridge dentures. So that, most Dental Doctors work on more than three patients in a single day, and they do not care about the patient economical situation. They are not interested at all in helping any person, who are confronting problems to eat, and don.t have enough money. this is our present world.
