What they wont tell you about All-On-4/Implant Dentures
Board Certified Prosthodontist and Board-Certified implant Dentist, Dr. David McFadden, discusses several things dentists don’t - or won’t - tell you when you’re consulting with them regarding full mouth dental implant treatment.
The most popular treatment protocol currently for full arch or full mouth implant treatment is the All-On-4 protocol, (where they replace a full arch of teeth on just 4 implants.) While, on the surface, this sounds terrific, and it has become very popular treatment option due to heavy marketing efforts around promoting it, there ARE several reasons it is not the BEST option.

      Yes. With bone level implants we use straight abutments 95% of the time. 5% of the time we must use angled abutments because of bone anatomy. However, when you don't remove bone as in the all-on-4 process you can successfully use an angled abutment when needed.

    Hi, Dr McFadden, So I checked out some implant dentists in Brighton, UK, where I live. They quote 'from $40,000'...so I am just wondering if you can quote a similarly rough figure to see if I have enough money to come out to Dallas? Basically dentistry is still a bit of a Wild West scenario, with many not very ethical, not very skilled implant dentists offering substandard care. But for your videos I too would have gone down the 'All on Four' route, which is the standard offering in Brighton, [and indeed in London]. Judging by your videos I imagine you are swamped by people from all over the US [and indeed the world] trying to find your level of skill, ethics and knowledge where they live-which must be rather frustrating! Thanks for all your informative videos, setting out a standard of care which is outstanding. Cheers, Rosemary Rimmer-Clay

      Without at least reviewing a current panoramic X-ray and your medical and dental history, it’s difficult to give you a ballpark estimate or know which options would best suit your needs. With that said, if you can send (email) a current panoramic to my office, my staff can facilitate a zoom consultation and I can shed more light on what I would recommend and what the fees would be in my office. You can call 214-956-9100 and my staff will be able to take it from there.

    Hi, Dr McFadden, I was told by a dentist in the UK that it is not possible to place bridges directly on implants, that they require abutments to ensure stability. Did I misunderstand your videos in which you say that abutments are not essential? I feel confused! Thanks, Rosemary Rimmer Clay

    • @davidmcfadden7927
      @davidmcfadden7927 5 дней назад

      It depends on the implant. One kind you can avoid the abutments (Tissue level implants) and one kind you can't (bone level implants). I use both. The main thing about "abutments" is to avoid or minimize the use of "angles abutments" which are always used in All-on-4. It's part of what causes the problems with All-on-4. Better surgery without bone removal reduces the need for angled abutments. This is covered in my "Buyer Beware..." videos.

I remove just enough not to show transition of real gums plus gum prosthesis when patient is doing full smile or like gnarling smile that is why pre photographs are in a full smile and or sort of doing gnarling smile as one of the guides of course ct scan etc should be taken in consideration also. How much bone removal has no definite measurement it will all depend on how you think it will all end up pre surgery. So if you plan with end in mind you can come up with FP1, FP2 and FP3.

    Interesting video. I am trying to decide between bridge implants which are 3 bridges each on 2 implants or do the all in 6 procedure. My understanding is that if one or two implants fail with the all in 6 you should be okay. If one implant fails with a bridge then you would need to replace that section but at least it is only one section. Also, bridges do not need to be removed ever year to be cleaned and I have a lower price quote to do the bridges by 6K. Looking for any advice as it is a hard decision and I plan on doing next week.

    • @davidmcfadden7927

      We no longer do separate bridges. There are several reasons why which I believe would only muddy the waters. I'm glad you're doing your research but I believe you are missing the biggest point about the differences between implant bridges and all on 6. You will have massive bone removal with all on 6. You will not have any bone removal with implant bridges. This is the main reason to opt for implant bridges. Your precious bone is worth far more than $6,000.

    I have said this before, many thanks for saving me from a disaster. Unfortunately, a lot of people will only want to know what happened after damage is done.

    I have a family member who works at a dentist office, I think they offer the all on, I told her I want to have the implant bridges done. Her response was that of if I kicked a puppy. I told her a few reasons I don’t want to be cut into like that. She was in disbelief that just one of the negatives is hacking apart my jaw and losing that precious bone. She even said “I’ve never heard of that” I’m just curious how they sell people on the procedure? If someone who works there doesn’t know exactly what happens, I am shocked what they tell patients. These “professionals” need to have consequences for what they are getting away with. The dental industry needs more regulation,

      It's a travesty. Intense marketing and indoctrination. I am glad you are informed. Spread the word !!

    What the approximate cost for a full bridge like the one you did just for the top?

      That depends on a few factors. But it can range from $30-$40,000. I hope this helps :)

    Thanks for the presentation! I have a month old zirconia over zirconia prostheses with long face and just realized that sometimes I grind my teeth only on my left side closest to one of my implant. I don’t want to break my new arches. Do I have to order custom made night guard or I can get one from Amazon? Appreciate your response.

      A custom one would be a better bet for a proper fit. I wish you a lifetime of success and comfort with your new prostheses 😊

    @mcfaddendentalimplantcenter Did you say that All-in-4s are removable? How long will they last?

    • @mcfaddendentalimplantcenter
      @mcfaddendentalimplantcenter 3 месяца назад

      No, they are screwed in and only removable by a dentist. How long they last depends on several factors. Keep watching - I have several videos on this subject. They should help you understand all the variables involved, and different options available so you know just which questions to ask and which terms to use when discussing full mouth implant treatment options.

    GREAT JOB DOC! What if by changing the MUA to straight instead of 17 0r 30 degrees your access hole is on the buccal or in the tooth?

      In cases where we change the abutment for better tissue adaption and improved aesthetics which may place the screw hole in an undesirable position, we use angulated screw channels either from the manufacturer or Dynamic Abutment.
