CTO PCI should be performed when the potential benefits outweigh the risks. The benefits (and the risks) depend on the patient's clinical condition, coronary anatomy, comorbidities, and the operator's expertise.

We didn't know who was beneficial for CTO PCI, but we tried to open all CTO vessels as we can !!

The sucess rate of CTO PCI is lower than other PCI. If CTO PCI is failed, I don't know whether collateral circulation may be decreased or not. Is it harmful for the patients? Of course I believe OPening CTO is better than only medication.

Average risk of complications is about 3% for CTO PCI
