  • Erebus Flight 901: Litany of Lies? Episode 9
    On November 28, 1979, an Air New Zealand DC10 crashed into the slopes of Mt Erebus in Antarctica, killing all 257 people on board. But who was really at fault, the pilot or the government-owned airline?
    On the 40th anniversary of the tragedy, presenter Garry McAlpine and writer/director John Keir interview the key players in a quest to discover the truth. They talk to the family of Jim Collins, the pilot who was initially blamed for the accident. For the first time we hear from an American pilot who tried in vain to send a warning to the doomed flight.

      You are correct about integrity but he had no idea about aviation

      Mahon may not have been trained to fly HS748s and so on, but he was intelligent enough to listen to advice from all directions. That some tried to lie about their actions is regrettable. As it is, I do know one of the persons who was secretly briefing Mahon. I have also exchanged hundreds of emails with a number of those who were involved in 1979 onwards and the truth is emerging. I will be publishing a book shortly which will include most of what I have been advised and the technical reports on the other cases which led to the Erebus saga. Watch this space!

      @@robinfautley8698 I look forward to reading it, however, there is no escaping the fact that Collins ( lead to a trap by AirNZ) failed in his most basic responsibility, safe terrain clearance

      Agreed but what caused the failure and how do we prevent repetition. As for apportioning blame, I am not going down that path as I am only interested in preventing repetition. However, judges do have to allocate blame in their court judgments. It would be interesting to know how you would allocate “blame” and what would be your formula to arrive at your allocation.
      I recollect Muldoon eventually said 3/5ths Air NZ and 2/5ths the flight deck. He just did not explain how he got to those numbers, but he was an accountant! Knew the price of everything but not the value.

      @@robinfautley8698 with almost all accidents to aircraft, there is an "error chain" and with careful analysis, the start of the chain can be determined. In Erebus case this started at AirNZ However, the issue of "safe terrain clearance " can avoid these issues, no matter how many errors there are in an organization. I dont "bame" Collins 100% but there was a serious lack of safety culture at airnz at the time.Many of their issues have been corrected since.

    Ted Thomas was almost right about Mahon being correct, other than the precise language “Orchestrated Litany etc” and it was not until 2016 when the “Natural Justice” afforded to the likes of liars in the past was deemed to be incorrect. The wording of an inquiry report had to be given with clarity, frankness and courage. It was Mahon’s duty to act with frankness and courage in his summing up. He was way ahead of all the judges who condemned his choice of words. His reputation within the New Zealand legal establishment has to be restored as one of the finest of all time.

      @@robinfautley8698 the lack of aviation understanding by Mahon was very obvious from the public inquiry. He doesn't understand the rule "the captain is solely responsible for safe terrain clearance " and the significant difference between IFR and going VFR with no radar support

    Erebus was pilot error, but AirNZ was guilty of many mistakes
