    🩺ABOUT DR. BREWER
    Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on to run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
    At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine. We find a lot of undiagnosed prediabetes or insulin resistance. Treating unrecognized risk factors like prediabetes allows reduction of risk and prevention of disease.

    For a number of reasons, primarily an elevated calcium score, two years ago I began low carb/keto . At the time my homocysteine level was 14.8. In addition to the diet I added methylated B-12 as well as folate ; my most recent homocysteine was 6.8. I was thrilled to see the improvement.

    Stan Getz had a beautiful estate overlooking the Hudson River in my home town of Irvington, NY. I went to school, played sports with his kids , and lived on the estate in one of the carriage houses for a while. Great guy and a huge talent. Too bad he's no longer with us.

    Sir my son age 3.10 having normal B12 and Folic acid levels but higher homocysteine 17.8( Normal less than 15)

    What to do if it homocysteine dropped to 3 after taking methylated form of b12? How to increase homocysteine? I feel terrible and aged fast.

    Doc, on this topic, I hope you can help me... I know that I have a high level of homocysteine (because I had the smartest hematologist EVER), and also, as a person with autoimmune disease, I definitely need my B vitamins. The problem is that I have hypercyanocobalaminemia, (I was actually told by my PCP to stop taking B12 for several months after a level check), AND that every B complex supplement on the market has B12 in excess of about 5000% of the RDA in it! What can I do?
    PS, when start accepting Medicare outright, and not just by reimbursement, I'm there!

      There are better forms of B-12 without the cyano group which the cheaper synthetic form of the vitamin. That said many (so-called experts) will call all forms B-12, cyanocobalamin even when it a more natural form so it maybe the term is problem and you just may need less of the vitamin??? I take methylcobalamin.

      @@man_at_the_end_of_time Yes, I do need less of the vitamin, but all of the B supplements have way too much B12 in them. That's why I was asking for help! Is there a good, recommend methylated B supplement with a normal amount of B12 in it? Recommendations?

      B12 is not toxic. There is no toxic level. Your body will take what it needs and pass the rest. It just looks high in the blood. This is not my opinion this is fact.

    There seems to be a relationship between high doses of B6 and tingling ( mainly in hands). I have this tingling and stopped the complex I've been taken. ( Methylated Bcomplex). It hasn't stopped the tingling and because I have high homecysteine, I want to continue it. Any comment from anyone?

      B Richie r-Lipoic acid, betaine (multigram dose), lower the carbs to 30 grams per day, fasting. methylated B's.....just things to research

    Dr.Brewer I started keto about a year ago and my total and ldl cholesterol showed to be high. I didn’t want to go on statin tried to ring down and it has but still my last test showed
    Total cholesterol 259
    Non HDL 185
    LDL 170
    Triglyceride 57
    Glucose 90
    A1c 4.3
    So compare to before lipid are going down should I continue with what I’m doing or start taking statin.

      Your Trigs and Glucose numbers look great. Your HDL is also likely good. Have you tried reducing saturated fat somewhat. I have an exaggerated response to it also and cutting back on red meat and dairy helps. Niacin would also likely to have a good impact. I would obtain more info before considering statin use. Advanced lipid testing such as particle size, Apob, lp(a), etc.. If you're over 40, you could get a calcium score scan or the CIMT as Dr Ford often recommends.
      Edit: it can be difficult to find a Dr who will prescribe advanced testing. Unfortunately most are stuck on the one size fits all approach. Being from a state that doesn't allow direct to consumer lab tests I had to find an alternative medicine Dr.. Many of these tests won't be covered by insurance. If I recall correctly the noted Lipidologist, Thomas Dayspring believes the 2 most important tests are LDL particle number and APOb. After several years of following the many "don't worry about LDL" voices on youtube, I started listening to and reading articles by Dayspring. He convinced me that perhaps I should error on the safe side and keep my numbers down, at least until more definitive evidence comes forward. My LDL has been as high as 289 and I have a moderate calcium score. I'm Statin intolerant so I take a pcsk9 inhibitor and very low dose of the Statin, crestor. I haven't noticed any side effects of the meds or of dropping my LDL down to well under 100. I hope to get some advanced blood work done soon to see if there's been any negative changes to my lipid profile etc.. BTW, I'd kill to have your A1c numbers. 5.3 is the lowest I've been able to achieve. Lastly, as a good doctor once told me, don't be in a rush to make a decision on the Statin. Heart disease is a slow process and you have plenty of time to research and sort out your best course of action.

      A statin is never warranted unless you have an artery blockage. High cholesterol by itself is not a reason to start a statin. Cholesterol and triglycerides will go up temporarily on Keto (I'm on it too), but it will eventually go back down. Don't try to micromanage your cholesterol, your body needs it for other things, brain health being at the top of the list. PS, I heard something from my cardiologist that I've never heard before in my life, not even in my 20's, and I only weighed 100 lbs then (I'm 53 now)... he said, your cholesterol is fine! I've been on Keto for 18 months!
