I was expecting the same too but yk he had been through a lot and Chang ho at the beginning of the show could've reacted like this but big mouse changho had a journey and knowing about miho's condition and realising that he can do nothing to save her was a part of that journey so her death was not a shock to him but something he knew was coming and he was suffering with it.
