  • The 2022 Shkreli Awards have been released! Each year, the Lown Institute passes out awards as a way of reporting on dysfunction in the US health care system. Dysfunction in healthcare is one of our foundational pillars here at Healthcare Triage, and these awards highlight some of the worst examples.

    Wait.
    The dentist made 2 mil but only had 1 mil in fines?
    Please tell me he had to reimburse patients as well.

    Dr. Carroll: How do you maintain positivity working in a system rife with corruption and perverse incentives?

      Not about Dr. Carroll, but my psychiatrist and I talk about this sometimes. She sees how the healthcare system is essentially run by sociopaths. Thing is, she loves her patients and wants to help us improve. So she navigates it, speaks their language and uses their own tools to subvert them. She gets to have her moments of "Gotcha" with them too.
      Personally, I don't know how she does it. She's naturally optimistic, but this is a cynical system. She once told me though "If we're going down with the ship, I'd rather we go down holding hands". I've carried that tid-bit with me ever since. There's hope in there, with each other.

      @@netsquall Vote? Who said anything about voting? This is about working within the current healthcare system as it is (aka, dealing with the insurance companies, the pharmaceutical companies, the policies of the clinic), not what we all wish it was.
      Until everyone votes for the system we all deserve, she's trying to stand in the gap for us against this corruption.

    Cool. Cigna is up for renewal of it's contract with my union health plan this year. I'll be sure to congratulate them on this award when we hand the contract to one of the other mega conglomerates in control of our access to healthcare.

    Speaking of Medicare Advantage . . . I was trying to become a patient at a local non-profit medical foundation here in California. Although I have visited their urgent care facility a few times, I would be considered a new patient. And they do NOT take new patients who have Medicare A and B. They suggested I could change my insurance. Really. I am now talking to a local hospital who - so far - takes patients who have Medicare. I'm 76 and I don't appreciate this BS. But,, yes, hurray for the best healthcare system in , , , the USA.

    Geez, it's granted that we should expect maybe a few really bad actors per year, but the ease with which this top 10 gets filled out is . . . impressive.

    Huh. Ever since I got Medicaid on top of Medicare, I've had United Healthcare agents stalking me, trying to get me to sign up with their combined plan. Their big incentive is "Hey, we have this $2000 card you can use to buy medical stuff, OTC meds and food".
    Between being able to afford to give away two grand per patient and how persistent these agents are, I'm assuming they make a boatload of money off each sale. But in my state, when you have both, your healthcare needs are taken care of (lordy, can everyone have this??). Meaning, if you take away the two grand, there's no reason to sign-up.
    So the question is, what's the scheme they're pulling, where they can legally make a new Medicare Advantage Plan out of what's essentially already a Medicare Advantage Plan? What's the kickback where they make so much money off of us?

    Listening to this video just highlights the continued problem of monetary power from major pharmaceutical and insurance companies directing patient care in a negative light. It was crazy to me to hear about the Phillip Morris creating tobacco products and then investing in products to then try and help care for the respiratory issues that the tobacco users suffer from is just so incredibly outrageous. Additionally, insurance companies allowing individuals to be diagnosed with diseases that don’t apply to the person because they are married to the individual with the disease is outrageous. In regards to the examples focused on specific physicians or hospitals I would be interested to see if there is a pattern of where these issues occur whether it be wealthy areas, rural areas, large hospitals or small hospitals, etc.
    These few examples just prove the point that money unfortunately has a major hand in medical care for all patients regardless of if a physician has good intentions at first it seems that they are able to be corrupted simply for a cash grab. Physicians are held to the medical ethics pillars of beneficence, non-maleficence, justice and autonomy and these top 10 Shkreli Awards taint the profession and leave a bad taste in patient’s mouths with being able to trust doctors. As an aspiring physician it saddens me to see that pharmaceutical companies, insurance companies and money overall play a way too major role in the harming the care for patients. The goal should be to provide the best quality care at all times without jeopardizing a patient’s health simply for money or other selfish reasons. I hope to continue to see the new wave of medical students push for focusing on care for their patients and working to prevent large companies from jeopardizing that by working towards positive policy changes as well as focusing on the real reason we are working to be physicians: providing the best possible care for all our patients.

    That Catholic hospital doctor was probably motivated to keep his stats up but also it goes hand in hand with Catholic religious teachings to preserve life no matter at all costs things like forcing women to give birth to a child born without a brain, things like that.

    In this video about fraud and dysfunction in American healthcare, Aaron Carroll brings up this idea or recurring theme where physicians and pharmaceutical companies are causing harm to patients just to turn around and fix the problem for the patient, just to benefit financially. Physicians take a Hippocratic oath to apply beneficence (do good) and non-maleficence (do no harm) for their patients. Are physicians truly upholding beneficence if they are the reason or cause for their patient needing the beneficence in the first place due to the physician’s violation of non-maleficence? Either way the physician is violating their Hippocratic oath in which they are fined, do jail time, and have their license taken away. On the other hand, pharmaceutical companies are fined with possible jail time of some employees but if they have other employees and are finically stable enough to withstand a fine, what is stopping pharmaceutical companies from continuing to exploit patients for their own financial gain? However, to play devil’s advocate, the pharmaceutical companies are being ethically moral and practicing beneficence by offering treatments for the health complications caused by their previous products. So whether or not the pharmaceutical companies knowingly sold harmful products to patients, they are attempting to fix the problem they created by selling new products as treatment for said problems/health conditions at a profit. Ultimately, is it ethically and morally sound if someone or some company try to fix a problem they created, especially if they created said problem in the first place and if they are profiting on both ends?

      There's a phrase in Korean that roughly translates to "giving someone both the illness and the medicine", which isn't strictly about healthcare, but it definitely applies here.
