As for the teleporter explosion thing, there are later episodes of the series that show that when you get teleported there is like a field that goes around you... I guess when you are getting broken down and scanned to be reassembled elsewhere, that's what that is... In that field you are still conscious and able to react, which is the time frame they were referring to. So, somehow the dude just hulks through the field and it causes an explosion. No idea why, no idea how re ran past everyone either. When I first saw this as a kid, I just assumed that he messed up the teleport process and it sent him to a random part of the ship.
