Competition Between Providers-- Whole Systems Research in Parkinson's

What is Comparative Effectiveness Research (CER)? And why isn't it being done in Parkinson's?
Why are some clinics getting better results than others?
It is ethical, appropriate, and/or essential to report on the outcomes their patients?
Would physician egos permit it?
