Not sure about Java edition, but the ender dragon escaping and wrecking your world was definitely true on bedrock some time ago (probably patched) I remember me and my cousin spending hours building an epic treehouse and building an end portal in it only for the stupid ass dragon going through the portal and destroying everything.
RIP i also don't think it happened on Java, I don't remember it ever happening to me.
