I'm having this exact problem. Have you figured out a solution? This mainly happens when using my PS5 and Switch. Workarounds: on PS5, go to the system audio settings and switch the audio output to Dolby then back to PCM. On Switch: go to the TV audio settings and change to Mono.
Even if the input volume is very low? That doesn't make sense. Connecting an HDMI from a PC to the Apple TV 4K and turning down the pre-amplification volume still shows this issue, as does turning down volume in games on my PS4 and Nintendo Switch. It's not an audio volume outputting too high of a signal. Not to mention that even loud signals can be fine for a moment, and then later be completely messed up later, same with quiet or even times when no audio is playing at all, so long as it's connected over eARC. Mystery still unsolved, unfortunately.
