The second Ender dragon is actually a bug fix incase if for some strange reason the Ender Dragon disappears while the end game portal is disabled. The first Ender dragon went flying away probably because of the level of cheating with the TNT knock back. Sandiction used this bug in his hardcore world to have multiple Ender dragons. It could of also been a command.
At 2:00 the third cloud is just because more clouds are rendering in the distance! If you watch the next few seconds, you can see it happening with the other clouds too.
